The paper is an attempt to describe the forecast on the risk of damages resulting from failures to the means of transport. It has been assumed that the product of the probability of failure (fault) occurrence and measures of effects thereof are to be used to estimate the risk. The below presented dependences that describe the risk of damages have been based on the failure rate. With the available literature as the basis, a preliminary description of the probability of a failure (fault) and the level of losses has been proposed. The paper gives dependences on short- and long-range risk forecasts. To determine the relationship for the probability of a failure (fault), the failure rate has been used.

Environmental engineers are frequently faced with uncertainty in making design decisions because the true value of many process parameters is unknown. In this study, the design of countercurrent air stripping towers was modeled using fuzzy numbers, taking into account uncertainties in mass transfer and Henry's constant. It was found that, in addition to cost, the risk of failure is an important design consideration for stripping tower design. A significant over-design is both cost-effective and results in less risk of design failure. The air-to-water ratio that yielded the least risk of failure switched from low to high as the removal efficiency of the tower increased. An important result is that at lower removal efficiencies, tower design and operation is most sensitive to uncertainties in mass transfer and at higher removal efficiencies, tower design and operation is most sensitive to uncertainties in Henry's constant . The implication is that low air-to-water ratios are best when the regulatory target removal efficiency is low and/or when the uncertainty in the value of the contaminant's Henry's constant is larger than the uncertainty in the mass transfer coefficient value. Otherwise a high air-to-water ratio results in the least risk of process failure.

Background: Concomitant lesion of the medial collateral ligament (MCL) is associated with a greater risk of anterior cruciate ligament (ACL) graft failure. Purpose: The aim of this study was to compare two medial stabilization techniques in patients with revision ACL reconstruction (ACLR) and concomitant chronic medial knee instability. Study Design: Cohort study; Level of evidence, 3. Methods: In a retrospective study, we included 53 patients with revision ACLR and chronic grade 2 medial knee instability to compare medial surgical techniques (MCL reconstruction [n = 17] vs repair [n = 36]). Postoperative failure of the revision ACLR (primary aim) was defined as side-to-side difference in Rolimeter testing ≥5 mm or pivot-shift grade ≥2. Clinical parameters and postoperative functional scores (secondary aim) were evaluated with a mean ± SD follow-up of 28.8 ± 9 months (range, 24-69 months). Results: Revision ACLR was performed in 53 patients with additional grade 2 medial instability (men, n = 33; women, n = 20; mean age, 31.3 ± 12 years). Failure occurred in 5.9% (n = 1) in the MCL reconstruction group, whereas 36.1% (n = 13) of patients with MCL repair showed a failed revision ACLR ( P = .02). In the postoperative assessment, the anterior side-to-side difference in Rolimeter testing was significantly reduced (1.5 ± 1.9 mm vs 2.9 ± 2.3 mm; P = .037), and medial knee instability occurred significantly less (18% vs 50%; P = .025) in the MCL reconstruction group than in the MCL repair group. In the logistic regression, patients showed a 9-times elevated risk of failure when an MCL repair was performed ( P = .043). Patient-reported outcomes were increased in the MCL reconstruction group as compared with MCL repair, but only the Lysholm score showed a significant difference (Tegner, 5.6 ± 1.9 vs 5.3 ± 1.6; International Knee Documentation Committee, 80.3 ± 16.6 vs 73.6 ± 16.4; Lysholm, 82.9 ± 13.6 vs 75.1 ± 21.1 [ P = .047]). Conclusion: MCL reconstruction led to lower failure rates in patients with combined revision ACLR and chronic medial instability as compared with MCL repair. MCL reconstruction was superior to MCL repair, as lower postoperative anterior instability, an increased Lysholm score, and less medial instability were present after revision ACLR. MCL repair was associated with a 9-times greater risk of failure.

In an environment where the availability of resources sought by a forager varies greatly, individual foraging is likely to be associated with a high risk of failure. Foragers that learn where the best sources of food are located are likely to develop risk aversion, causing them to avoid the patches that are in fact the best; the result is sub-optimal behaviour. Yet, foragers living in a group may not only learn by themselves, but also by observing others. Using evolutionary agent-based computer simulations of a social foraging game, we show that in an environment where the most productive resources occur with the lowest probability, socially acquired information is strongly favoured over individual experience. While social learning is usually regarded as beneficial because it filters out maladaptive behaviours, the advantage of social learning in a risky environment stems from the fact that it allows risk aversion to be circumvented and the best food source to be revisited despite repeated failures. Our results demonstrate that the consequences of individual risk aversion may be better understood within a social context and suggest one possible explanation for the strong preference for social information over individual experience often observed in both humans and animals.
