Background Currently, residency programs lack objective predictors for passing the sequenced American Board of Anesthesiology (ABA) certification examinations on the first attempt. Our hypothesis was that performance on the ABA/American Society of Anesthesiologists In-Training Examination (ITE) and other variables can predict combined success on the ABA Part 1 and Part 2 examinations. Method The authors studied 2,458 subjects who took the ITE immediately after completing the first year of clinical anesthesia training and took the ABA Part 1 examination for primary certification immediately after completing residency training 2 yr later. ITE scores and other variables were used to predict which residents would complete the certification process (passing the ABA Part 1 and Part 2 examinations) in the shortest possible time after graduation. Results ITE scores alone accounted for most of the explained variation in the desired outcome of certification in the shortest possible time. In addition, almost half of the observed variation and most of the explained variance in ABA Part 1 scores was accounted for by ITE scores. A combined model using ITE scores, residency program accreditation cycle length, country of medical school, and gender best predicted which residents would complete the certification examinations in the shortest possible time. Conclusions The principal implication of this study is that higher ABA/ American Society of Anesthesiologists ITE scores taken at the end of the first clinical anesthesia year serve as a significant and moderately strong predictor of high performance on the ABA Part 1 (written) examination, and a significant predictor of success in completing both the Part 1 and Part 2 examinations within the calendar year after the year of graduation from residency. Future studies may identify other predictors, and it would be helpful to identify factors that predict clinical performance as well.

Hemoglobin A1c (HbA1c) is used routinely to monitor long-term glycemic control in people with diabetes mellitus, as HbA1c is related directly to risks for diabetic complications. The accuracy of HbA1c methods can be affected adversely by the presence of hemoglobin (Hb) variants or elevated levels of fetal hemoglobin (HbF). The effect of each variant or elevated HbF must be examined with each specific method. The most common Hb variants worldwide are HbS, HbE, HbC, and HbD. All of these Hb variants have single amino acid substitutions in the Hb β chain. HbF is the major hemoglobin during intrauterine life; by the end of the first year, HbF falls to values close to adult levels of approximately 1%. However, elevated HbF levels can occur in certain pathologic conditions or with hereditary persistence of fetal hemoglobin. In a series of publications over the past several years, the effects of these four most common Hb variants and elevated HbF have been described. There are clinically significant interferences with some methods for each of these variants. A summary is given showing which methods are affected by the presence of the heterozygous variants S, E, C, and D and elevated HbF. Methods are divided by type (immunoassay, ion-exchange high-performance liquid chromatography, boronate affinity, other) with an indication of whether the result is artificially increased or decreased by the presence of a Hb variant. Laboratorians should be aware of the limitations of their method with respect to these interferences.

During the last two decades we have witnessed great improvements in ICT hardware and software technologies. Three-dimensional content is starting to become commonplace now in many applications. Although for many years 3D technologies have been used in the generation of assets by researchers and experts, nowadays these tools are starting to become commercially available to every citizen. This is especially the case for smartphones, that are powerful enough and sufficiently widespread to perform a huge variety of activities (e.g. paying, calling, communication, photography, navigation, localization, etc.), including just very recently the possibility of running 3D reconstruction pipelines. The REPLICATE project is tackling this particular issue, and it has an ambitious vision to enable ubiquitous 3D creativity via the development of tools for mobile 3D-assets generation on smartphones/tablets. This article presents the REPLICATE project’s concept and some of the ongoing activities, with particular attention being paid to advances made in the first year of work. Thus the article focuses on the system architecture definition, selection of optimal frames for 3D cloud reconstruction, automated generation of sparse and dense point clouds, mesh modelling techniques and post-processing actions. Experiments so far were concentrated on indoor objects and some simple heritage artefacts, however, in the long term we will be targeting a larger variety of scenarios and communities.

PROXIMA 2A is a new micro-focus and energy tunable beamline dedicated to biological macromolecular crystallography at Synchrotron SOLEIL. The beamline officially opened in March 2013, and its first year of user operation has yielded excellent results. The X-ray source is a powerful in-vacuum U24 undulator coupled to a cryo-cooled Si[111] channel-cut monochromator and a pair of focussing bimorph mirrors in Kirpatrick-Baez configuration. This combination delivers a photon flux of over 10**12 ph/s into a focal spot of 10 μm × 5 μm (H×V FWHM), which is tunable over 6 – 15 keV. The supports of the optical elements have been designed to minimise the effects of vibrations and thermal dilations on the X-ray beam position, which is stable to within 5 microns over a day. The experimental station consists of a high performance micro-diffractometer, a cryostream, an area detector (ADSC Q315r), and an X-ray fluorescence detector. The X-ray energies for MAD experiments are directly calibrated on the sample. A robot equipped with a large 9 uni-puck dewar (CATS Irelec) is available to users for the automated transfer and screening of cryo-cooled samples. The users launch their experiments via an MXCuBE interface [1], which permits the centering of the sample, collecting of diffraction images, recording of X-ray spectra and the transfer of samples. The X-ray diffraction data are of an excellent quality, and the users readily exploit the micro-focused X-rays to select the best zones of their crystals. The first year of results from users has yielded a variety of success stories including novel protein structures resolved from crystals as small as 5 microns, as well as those solved by SAD & MAD methods. The future perspectives include automated helical and grid scans, in situ plate screening and multi-crystal merging techniques.

Background: An adequate level of muscular fitness is related to occupational performance in military personnel, including pilots flying high performance aircraft. The aim of this study was to describe the baseline level and the change in muscular fitness between the first and the second years of the Air Force Academy among fighter pilot cadets. Methods: The muscular strength and endurance test results of 182 male fighter pilot cadets were analyzed during their first year in the Air Force Academy and one year after. Maximal isometric strength tests included trunk flexion, trunk extension and bilateral leg extension tests, whereas muscle endurance was measured with modified a sit-up test and seated alternative dumbbell press. Results: The maximal isometric bilateral strength of the leg extensor muscles increased from 220 ± 42 to 232 ± 42 kg. The maximal isometric trunk extension strength increased from 117 ± 21 to 120 ± 19 kg and trunk flexion from 82 ± 16 to 86 ± 17 kg. Muscle endurance increased from 68 ± 13 to 75 ± 15 repetitions/min in seated dumbbell press and from 47 ± 12 to 51 ± 13 repetitions/min in sit-up test. Conclusions: Both maximal strength and muscular endurance improved among fighter pilot cadets, which indicates that occupational performance is well maintained or improved from the perspective of physical fitness during the early phase of academy studies. Education in the Air Force Academy, including physical education, seems beneficial in improving muscular fitness among military pilots.

Abstract Hip fracture patients experience heterogenous pathways of recovery. Knowing about these different groups can guide rehabilitation efforts. The sample was 339 hip fracture patients age 65+ recruited to the Baltimore Hip Studies 7th cohort [2006-2011; men(n=168) women(n=171)] assessed at baseline, 2.6, and 12 months. Group based trajectory modeling assessed participants’ cognitive and functional recovery trajectories as measured by the Modified Mini-Mental State Examination(3MS;0-100), Hooper Visual Organization Task(HVOT,0-30), Trail-Making Task(TrailsA,B:0-301s), Short Physical Performance Battery(SPPB,0-12), Lower Extremity Physical Activities of Daily Living(LPADL,0-12), and physical activity (YALE, hours/week). Participants were on average 80.91(7.88) years old with 13(3.41) years of education. Two groups were identified for Trails A [Consistent high performance (85.3%), declined performance (14.6%)] and YALE [low exercise(94.9%), small decline(5.0%)]. Three groups were identified for 3MS [consistently high(76.3%), consistently borderline(17.1%), consistently low-performing(6.4%)] and SPPB [failure(19.3%), low-performing with small improvement(55.7%), moderate improvement(24.9%)]. Four groups were identified for HVOT[low scoring(14.2%), mid-range(31.9%), borderline(32.4%), well-performing(21.3%)], Trails B [consistent high-performance(40.2%), improved(18.6%), declined(12.0%), consistent failure(29.1%)], and LPADL[low-performing(27.0%), small improvement(50.0%), dramatic improvement(17.0%), high-performing(5.7%)]. The polynomial functions of the trajectory groups were primarily quadratic with some cubic functions best representing the recovery trajectories. Many of the groups have similar trajectories but at different magnitudes. For most tests performance at baseline was able to differentiate performance at 1 year. These data show that there are very few changes in high or low performing groups, but mid-range participants can experience volatility.

Abstract Background The objective of this study was to detect the urinary levels of dimethoate, benzo(a) pyrene (BaP), and bisphenol A (BPA) in first-year Hohai University students with different geographic origins. Methods First-morning urine samples were collected from 540 healthy freshmen aged 17 to 19 years. Chemical levels were measured using β-glucuronidase hydrolysis followed by a high-performance liquid chromatography-tandem mass spectrometry-based method. Geometric means (GMs) of these three chemicals are presented by body mass index (BMI) and location in a volume-based and creatinine-standardized way. Results GM concentrations of omethoate, BPA and 3-OHBaP were 9.47 μg/L (10.80 μg/g creatinine), 3.54 μg/L (4.04 μg/g creatinine) and 0.34 ng/L (0.39 ng/g creatinine), respectively. The GM concentration of omethoate in males was significantly higher than that in females. The individuals with a BMI higher than 23.9 had higher GM concentrations of omethoate, BPA, and 3-OHBaP. The inhabitants of Southwest China had significantly lower GM concentrations of omethoate, BPA, and 3-OHBaP than those who lived in other locations in China. Conclusion The average level of environmental chemical accumulation in freshmen is lower in Southwest China and differs in youth who live in different regions. In addition, obesity is correlated with higher toxin levels in youth.

The goal of the study was the identification of epidemiological features of whooping cough and assessment of economic losses due to this disease in Moscow.Materials and methods. The retrospective descriptive epidemiological research is conducted. The statistical data on incidence of whooping cough in the Russian Federation and in Moscow from 2009 to June, 2018, the reports on registration of whooping cough cases as well as the results of laboratory testing on whooping cough in Moscow in 2017 have been studied. The general losses from whooping cough in Moscow in 2017 have been calculated.Results. The whooping cough incidence in Moscow considerably influences a situation on this infection in the country in general as more than 20% of all cases of whooping cough in Russia are registered in the capital, where the incidence exceed by 1.5–2.5 times the average country level. The incidence of whooping cough among children of 7–14 years and the share of this group of population in the structure of the whooping cough cases have increased in Moscow, and outbreaks of pertussis at schools are observed. In Moscow the general losses due to whooping cough in children of 7–14 years in 2017 were estimated as 19.72 million rubles. High performance of vaccinal prevention of whooping cough is confirmed: in Moscow in 2017 the coefficient of epidemiological efficiency of vaccination among children of 6-12 months was 97.7%, among children at the age of 1 year – 82.6%, 2 years -79.9%, 3 years – 74.9%. Weighted average of direct loss from a case of whooping cough was estimated as 31182.2 rubles. The general losses due to whooping cough in Moscow in 2017 were 67.88 million rubles.Conclusion. It is necessary to take additional measures for vaccinal prevention of whooping cough at children of school age and to support the high level of coverage by timely immunization of children of the first year of life. For assessment of economic feasibility of vaccinal prevention of whooping cough it is necessary to consider the size of economic losses due to this disease.
