The aim of our review is to critically analyze the urban agriculture and urban food systems literature in order to understand the impact of urban-produced foods on community food security. We examine the role of city planning, food policy, and civic engagement in creating spaces for urban agriculture in cities across the United States, and whether (and how) these spaces promote food justice and food security. Bringing together multidisciplinary literature on access to urban agriculture and the distribution of urban-produced foods in a thematic, systematic review, we point out gaps in the academic research that would benefit from further study. The review integrates academic literature generated from Web of Science searches with gray literature identified through Google Alerts. We find that while there is a strong focus on elucidating the multiple benefits of urban agriculture, there are few studies that robustly measure the impact of urban farms on improving food security in low-income communities. Much of the literature is theoretical, focused on the production potential of urban agriculture, while more work is needed to understand and overcome barriers to access and distribution among communities in need. We conclude with a set of recommendations for researchers, practitioners, and policymakers who seek to create spaces in cities for food justice, equity, access, and sovereignty.

Abstract Background In the United States, approximately 11% of households were food insecure prior to the COVID-19 pandemic. The present study aims to describe the prevalence of food insecurity among adults and households with children living in the United States during the pandemic. Methods This study utilized social media as a recruitment platform to administer an original online survey on demographics and COVID-related food insecurity. The survey was disseminated through an advertisement campaign on Facebook and affiliated platforms. Food insecurity was assessed with a validated six-item United States Department of Agriculture (USDA) Household Food Security Survey Module, which was used to create a six-point numerical food security score, where a higher score indicates lower food security. Individual-level participant demographic information was also collected. Logistic regressions (low/very-low compared with high/marginal food security) were performed to generate adjusted odds ratios (AOR) and 95%CIs for food insecurity and select demographic characteristics. Results Advertisements reached 250,701 individuals and resulted in 5,606 complete surveys. Overall, 14.7% of participants self-identified as having low or very low food security in their households, with higher prevalence (17.5%) among households with children. Unemployment (AOR:1.76, 95%CI:1.09–2.80), high school or lower education (AOR:2.25, 95%CI:1.29–3.90), and low income (AOR[$30,000-$50,000]:5.87, 95%CI:3.35–10.37; AOR[< $30,000]:10.61, 95%CI:5.50–20.80) were associated with higher odds of food insecurity in multivariable models among households with children (and the whole sample). Conclusions These data indicate exacerbation of food insecurity during the pandemic. The study will be instrumental in guiding additional research and time-sensitive interventions targeted towards vulnerable food insecure subgroups.

This research highlights the mismatch between food security and climate adaptation literature and practice in the Global North and South by focusing on nested case studies in rural India and the United States during the COVID-19 pandemic. The United States is one of the wealthiest countries in the world, but also has one of the largest wealth gaps. Comparatively, India has one of the largest populations of food insecure people. To demonstrate how adaptive food security approaches to climate change will differ, we first review the unique climate, agricultural, demographic, and socio-economic features; and then compare challenges and solutions to food security posed by the COVID-19 pandemic. While both countries rely on rural, low-income farmworkers to produce food, the COVID-19 pandemic has highlighted how agricultural and food security policies differ in their influence on both food insecurity and global hunger alike. Emphasis on agricultural production in developing regions where a majority of individuals living in rural areas are smallholder subsistence farmers will benefit the majority of the population in terms of both poverty alleviation and food production. In the Global North, an emphasis on food access and availability is necessary because rural food insecure populations are often disconnected from food production.

ABSTRACT Background Nonalcoholic fatty liver disease (NAFLD), considered a “barometer” of metabolic health, is the leading cause of liver disease in the United States. Despite established associations between food insecurity and obesity, hypertension, and diabetes, little is known about the relation between food insecurity and NAFLD. Objective We sought to evaluate the association of food insecurity with NAFLD among low-income adults in the United States. Methods We conducted a cross-sectional analysis of a nationally representative sample of adults from the NHANES (2005–2014 waves). Participants included adults in low-income households (≤200% of the federal poverty level) without chronic viral hepatitis or self-reported heavy alcohol use. Food insecurity was measured using the Household Food Security Survey. Our primary outcome was NAFLD, as estimated by the US Fatty Liver Index, and our secondary outcome was advanced fibrosis, as estimated by the NAFLD fibrosis score. The association between food insecurity (defined as low and very low food security) and hepatic outcomes was assessed using multivariable logistic regression, adjusting for sociodemographic factors. Results Among 2627 adults included in the analysis, 29% (95% CI: 26%, 32%) were food insecure. The median age was 43 y, 58% were female, and 54% were white. The weighted estimated prevalence of NAFLD did not differ significantly by food security status (food secure 31% compared with food insecure 34%, P = 0.21). In the multivariable model, food-insecure adults were more likely to have NAFLD (adjusted OR: 1.38; 95% CI: 1.08, 1.77) and advanced fibrosis (adjusted OR: 2.20; 95% CI: 1.27, 3.82) compared with food-secure adults. Conclusions Food insecurity may be independently associated with NAFLD and advanced fibrosis among low-income adults in the United States. Future strategies should assess whether improved food access, quality, and healthy eating habits will decrease the growing burden of NAFLD-associated morbidity and mortality among at-risk adults.

Across the world, many people remain food insecure, and unhealthy dietary patterns are driving up health costs. Solutions to these problems will impact food production and water scarcity. This chapter reviews the history of government interventions to improve food security and nutrition in the United States, which have enjoyed a “broad coalition” of support over seven decades. More recently, nutrition incentive programs have emerged, aiming to increase purchases and consumption of fresh fruits and vegetables by low-income consumers who benefit from the federal nutrition programs. This chapter explores the development and programmatic success of nutrition incentives, impacts on consumers’ food security and nutrition, impacts on farmers and farmers’ markets, and potential contributions to shifting dietary patterns. As they mature, evolve, and scale, nutrition interventions may deliver shorter supply chains, increase fruit and vegetable consumption, and contribute to a shift toward more sustainable and nutritious foods.

Abstract Background Home food procurement (HFP) (i.e. gardening, fishing, foraging, hunting, backyard livestock and canning) have historically been important ways that people obtain food. Recently, some HFP activities have grown (e.g. gardening), while other activities (e.g. hunting) have become less common in the United States. Anecdotally, COVID-19 has sparked an increase in HFP evidenced by increased hunting licenses and shortages in seeds and canning supplies. HFP may have positive benefits for food security and diet quality, though research beyond gardening is especially limited in high-income countries. Methods We examine HFP activities since the COVID-19 pandemic began, and their relationship to food security and dietary quality using multivariable logit models and matching analysis with a statewide representative survey (n = 600) of residents of Vermont, United States. Results We find 29% of respondent households classified as food insecure since COVID-19, and higher prevalence of food insecurity among those experiencing a negative job change since COVID-19, households earning less than $50,000 annually, Hispanic and multi-race respondents. Nearly 35% of respondents engaged in HFP activities since the COVID-19 pandemic began; the majority of those gardened, and more than half pursued HFP activities more intensely than before the pandemic or for the first time. Food insecure households were more likely to pursue HFP more intensely, including more gardening, fishing, foraging, and hunting. Respondents who were food insecure, Black, Indigenous, People of Color, those with a negative job disruption, and larger households all had greater odds of increased intensity of HFP during the COVID-19 pandemic. HFP was significantly associated with eating greater amounts of fruits and vegetables; however, this effect was only significant for food secure households. Conclusion Overall, these results suggest that HFP activities have increased since the start of the COVID-19 pandemic, and may be an important safety net for food insecure households. However, HFP for food insecure households does not translate into the same higher fruit and vegetable intake as found among food secure HFP households, suggesting this population may be trying to maintain intake, or that they may have potential important resource or technical assistance needs. Long-term, HFP activities may have important food security and diet quality impacts, as well as conservation implications, which should be more thoroughly explored. Regardless, the increased interest and intensity of HFP demonstrates opportunities for educational and outreach efforts.

The proportion of children in the United States without private or public health insurance increased from roughly 13 percent to 18 percent between 1977 and 1987, according to a new study by the Agency for Health Care Policy and Research (AHCPR). The growth in the proportion of uninsured children in poor and low-income families over the decade was even more dramatic—it rose from 21 percent to 31 percent.

Definition Drowning is defined as death caused by submersion, whereas near-drowning connotes survival for some time period following submersion. The following remarks pertain to the near-drowning victim who presents for acute medical management. Epidemiology Because reporting of near-drowning incidents is incomplete, most of the available epidemiologic information focuses on drowning deaths, which number more than 6500 per year in the United States. Data from King County, Washington, however, suggest that near-drownings slightly out-number drownings. Drowning rates are highest for children under the age of 5 years and between the ages of 15 and 24 years. Males drown 4 times more frequently than females. African-Americans and low-income groups also are affected disproportionately, except for those drownings involving boats and residential swimming pools, which more often are owned by middle class groups. Drownings peak during the summer months and are most common in the southern and western United States and Alaska. Forty to 45% of all drownings occur while the victim is swimming and 12% to 29% are boat-related. Alcohol plays a substantial role in these deaths. Between one half and three quarters of all drownings occur in lakes, ponds, rivers, and the ocean. More than 40% of all submersions in these bodies of water involve older adolescents or young adults.
