AbstractThe Oncology Care Model (OCM) is a US Centers for Medicare & Medicaid Services (CMS) specialty model implemented in 2016, to provide higher quality, more highly coordinated oncology care at the same or lower costs. Under the OCM, oncology clinics enter into payment arrangements that include financial and performance accountability for patients receiving chemotherapy treatment. In addition, OCM clinics commit to providing enhanced services to Medicare beneficiaries, including care coordination, navigation, and following national treatment guidelines. Nutrition is a component of best-practice cancer care, yet it may not be addressed by OCM providers even though up to 80% of patients with cancer develop malnutrition and poor nutrition has a profound impact on cancer treatment and survivorship. Only about half of US ambulatory oncology settings screen for malnutrition, registered dietitian nutritionists (RDNs) are not routinely employed by oncology clinics, and the medical nutrition therapy they provide is often not reimbursed. Thus, adequate nutrition care in US oncology clinics remains a gap area. Some oncology clinics are addressing this gap through implementation of nutrition-focused quality improvement programs (QIPs) but many are not. What is needed is a change of perspective. This paper outlines how and why quality nutrition care is integral to the OCM and can benefit patient health and provider outcomes.

Purpose – This study aims to address how and why do formal key account management (KAM) programmes hinder effective KAM management, and how can the problems of formalization in KAM be overcome. Recent empirical studies have reported an unexpected negative relationship between KAM formalization and performance. Design/methodology/approach – An 18-month (340 days) ethnographic investigation was undertaken in the UK-based subsidiary of a major US sports goods manufacturer. This ethnographic evidence was triangulated with 113 in-depth interviews. Findings – This study identifies how and why managerial reflexivity allows a more effectively combining of formal and post-bureaucratic KAM practices. While formal KAM programmes provide a means to initiate, implement and control KAM, they have an unintended consequence of increasing organizational bureaucracy, which may in the long-run hinder the KAM effectiveness. Heightened reflexivity, including “wayfinding”, is identified as a means to overcome many of these challenges, allowing for reflexively combining formal with post-bureaucratic KAM practices. Research limitations/implications – The thesis of this paper starts a new line of reflexive KAM research, which draws theoretical influences from the post-bureaucratic turn in management studies. Practical implications – This study seeks to increase KAM implementation success rates and long-term effectiveness of KAM by conceptualizing the new possibilities offered by reflexive KAM. This study demonstrates how reflexive skills (conceptualized as “KAM wayfinding”) can be deployed during KAM implementation and for its continual improvement. Further, the study identifies how KAM programmes can be used to train organizational learning regarding KAM. Furthermore, this study identifies how and why post-bureaucratic KAM can offer additional benefits after an organization has learned key KAM capabilities. Originality/value – A new line of enquiry is identified: the reflexive-turn in KAM. This theoretical position allows us to identify existing weakness in the extant KAM literature, and to show a practical means to improve the effectiveness of KAM. This concerns, in particular, the importance of managerial reflexivity and KAM wayfinding as a means to balance the strengths and weaknesses of formal and post-bureaucratic KAM.

Background: There remains significant opportunities to reduce door-to-needle (DTN) times for stroke despite regional and national efforts. In Chicago, Quality Enhancement for the Speedy Thrombolysis for Stroke (QUESTS) was a one year learning collaborative (LC) which aimed to reduce DTN times at 15 Chicago Primary Stroke Centers. Identification of barriers and sharing of best practices resulted in achieving DTN < 60 minutes within the first quarter of the 2013 initiative and has sustained progress to date. Aligned with Target: Stroke goals, QUESTS 2.0, funded for the 2016 calendar year, invited 9 additional metropolitan Chicago area hospitals to collaborate and further reduce DTN times to a goal < 45 minutes in 50% of eligible patients. Methods: All 24 hospitals participate in the Get With The Guidelines (GWTG) Stroke registry and benchmark group to track DTN performance improvement in 2016. Hospitals implement American Heart Association’s Target Stroke program and share best practices uniquely implemented at sites to reduce DTN times. The LC included a quality and performance improvement leader, a stroke content expert, site visits and quarterly meetings and learning sessions, and reporting of experiences and data. Results: In 2015, the year prior to QUESTS 2.0, the proportion of patients treated with tPA within 45 minutes of hospital arrival increased from 21.6% in Q1 to 31.4% in Q2. During the 2016 funded year, this proportion changed from 31.6% in Q1 to 48.3% in Q2. Conclusions: Using a learning collaborative model to implement strategies to reduce DTN times among 24 Chicago area hospitals continues to impact times. Regional collaboration, data sharing, and best practice sharing should be a model for rapid and sustainable system-wide quality improvement.

The pipeline industry continues to look for ways to improve its compliance and performance. Management systems have increased prevalence in the pipeline industry, with recognition that carefully designed and well-implemented management systems are the fundamental method that should be used to keep people safe, protect the environment and align organizational activities. Experience has shown significantly better success rates with management system implementation, both in terms of the quality and speed, when the person responsible for the design, implementation and sustainment of the management system has an integrated set of technical and enabling competencies. However, there is currently no standardized competency model that can be used to support a Management Systems Professional’s specialized knowledge and skills. The paper outlines the competencies needed by individuals to be effective in the design, implementation, measurement and evaluation of management systems. Applying a ‘whole-person’ perspective, the model includes business, relational and technical competencies that contribute to performance excellence for management system practitioners, including outlining example behaviours at target level performance and proficiency, and supported by a defined body of knowledge. This paper describes the Management System Competency Model, including how it can be used to create a position-specific development program for application within various organizations. This research establishes a basis for the creation of a practical, systematic and easy to use development road map for individuals and organizations who use or leverage a management system.

The design of a product largely predetermines its cost and quality, and there are therefore limits to the benefits that can be obtained by the application of best practice in manufacturing and quality control. The paper introduces a general model of design for quality and describes a systematic quality evaluation methodology to aid the development of quality competitive products. The application and performance of the methodology are described and its integration with techniques in design for manufacture and assembly is discussed.

Using Facebook Groups to connect otherwise anonymous people that live in a single urban development is a relatively new phenomenon. Within residential developments, there are a number of common comfort, management, and performance issues experienced by many isolated inhabitants that are identified through building performance evaluation studies. Facebook is a ubiquitous social network tool and powerful communication platform, particularly popular among young adults. This article explores the use of closed Facebook Groups in relation to collective learning about home use in two residential communities in the United Kingdom. Data were collected through longitudinal digital and physical visits to case study residential developments and to the Facebook Group sites. Group development, dynamics, and the quality of knowledge sharing is evaluated. Findings are presented in relation to home use learning, as it proved to be a vital theme of each Group’s activity. We propose that weak-tie urban communities can develop collective efficacy through communicating on a Facebook Group that enables quality learning based on reciprocal sharing of experiences and knowledge by its members. This helps tackle comfort issues experienced, lower the cost of living, and share bespoke, context-specific home use best practice. Strong engagement and leadership of group administrators limited to early stages of the Groups’ formation followed by high rate of activity by the majority of members was key. There was a clear overlap observed between social media narrative and the physical experiences of daily life, which helps support residents. The analysis suggests the positive effect of the learning environment created bottom-up would not be easily transferable to professional applications.

The aim is to achieve top-quartile financial results by accessing new value streams, encapsulating business processes of best practice and motivating employees of businesses to work in a collaborative atmosphere within the global and cross-functional business cycle to make smarter, quicker and more competitive choices.

Sport organizations regulate athletes' use of social media for many reasons including the protection of the organization's reputation. Several strategies have been introduced to minimize issues related to the negative consequences athlete social media use may present, yet whether these strategies also work to address social media distractions experienced by athletes during major sport events is not well known. Utilizing communication privacy management (CPM) theory, the purpose of the current research was to examine the aspects of social media that sport administrators perceive to be distracting to athletes and what support and management mechanisms are utilized to address such concerns during major sport events. Semistructured interviews ( N = 7) with Australian national sport organization (NSO) administrators were conducted. Sport administrators reported several aspects of social media that are perceived to distract athletes including personal and performance criticism and a fixation with social media profiles. Social media could also be used to manage athlete temperament. As a result, organizations highlighted both proactive and reactive communication boundaries and mechanisms that could be used to address concerns including content restrictions, best practice case studies, engaging in conversations, and monitoring. Opportunities for sport practitioners are described including conducting consultation sessions with athletes to better understand their needs regarding their social media use.

This paper addresses the methodology used to design the layout of the tip cooling nozzles of a high pressure rotor blade turbine. The methodology used is through a complete CAE approach, by means of a parametric CFD model which is run several times for the exploration of several designs by an optimizer. Hence the design is carried out automatically by parallel computations, with the optimization algorithms taking the decisions rather than the design engineer. The engineer instead takes decision regarding the physical settings of the CFD model to employ, the number and the extension of the geometrical parameters of the blade tip holes and the optimization algorithms to be employed. From CFD validation the final design of the tip cooling geometry found by the optimizer has proved to be better than the base design, which used mean values of all input parameters, and than the design proposed by an experienced heat transfer AVIO engineer, who used standard best practice methods. Furthermore the large number of experiences gained by the simulations run by the optimizer allowed the designer to find laws, functions and correlation between input parameters and performance output, with a further and deeper insight on this specific design problem.
