ΕΙΣΑΓΩΓΗ: Ο Διαβήτης τύπου 2 είναι από τα πιο δαπανηρά χρόνια νοσήματα τόσο για το ίδιο το άτομο όσο και την κοινωνία. Ανάγκη αποτελεί η μεταφορά των μεγάλων κλινικών δοκιμών πρόληψης της νόσου με τροποποιήσεις του τρόπου ζωής, στην καθημερινή πρακτική και η προαγωγή εφικτών, αποτελεσματικών και οικονομικά αποτελεσματικών παρεμβάσεων στην κοινότητα. Πρόκληση αποτελεί η υλοποίηση ενός κοινοτικού προγράμματος πρόληψης του διαβήτη σε ενήλικες μεγαλύτερης ηλικίας.ΣΚΟΠΟΣ: Σκοπός της παρούσας μελέτης είναι να εξετάσει την επίδραση ενός διετούς κοινοτικού προγράμματος παρέμβασης στον τρόπο ζωής σε σχετικά υγιείς ηλικιωμένους 55-75 ετών στις αλλαγές των διατροφικών συνηθειών, της σωματικής δραστηριότητας και τις κλινικές και μεταβολικές παραμέτρους.ΥΛΙΚΟ ΚΑΙ ΜΕΘΟΔΟΣ: Συνολικά 34 συμμετέχοντες μέσης ηλικίας 66±6 ετών υψηλού κινδύνου εμφάνισης διαβήτη οι οποίες εντοπίστηκαν με τη συμπλήρωση του ερωτηματολόγιου Finnish Type 2 Diabetes Risk Score χωρίστηκαν τυχαία σε δύο ομάδες στα δύο Κέντρα Ανοιχτής Προστασίας Ηλικιωμένων στην ελληνική αγροτική πόλη του Αιγίου. Η διετής παρέμβαση περιελάμβανε 16 ομαδικές συναντήσεις για την ομάδα παρέμβασης και μια αρχική συνάντηση με τους συμμετέχοντες της ομάδας ελέγχου από μια διαιτολόγο. Μετρήσεις των κλινικών και μεταβολικών παραγόντων, των διατροφικών συνηθειών και της σωματικής δραστηριότητας πραγματοποιηθήκαν στην αρχή του προγράμματος, στον πρώτο και δεύτερο χρόνο. Η αξιολόγηση των διατροφικών συνηθειών και της σωματικής δταστηριότητας έγινε με την συμπλήρωση του Ερωτηματολογίου Καταγραφής Κατανάλωσης Τροφίμων και του Διεθνούς Ερωτηματολογίου Σωματικής Δραστηριότητας.ΑΠΟΤΕΛΕΣΜΑΤΑ: Οι αναλύσεις των διαφορών μεταξύ των δύο ομάδων στον πρώτο και δεύτερο χρόνο (σε σύγκριση με την αρχή) έδειξαν ότι οι συμμετέχοντες της ομάδας παρέμβασης πέτυχαν σημαντικότερες μειώσεις του σωματικού βάρους (-0.8±1.9 έναντι 1.7±3.8kg, p=0.021), της περιμέτρου μέσης [-1(-2 -1) έναντι 2 (0 – 4) cm, p=0.027], της διαστολικής πίεσης αίματος (-2.89±14.71 έναντι 15.85±20.19mmHg, p=0.044), της ημερήσιας ενεργειακής πρόσληψης (-202.6±409.2 έναντι 194.3±453.7kcal/day, p=0.015) και του προσλαμβανόμενου κορεσμένου λίπους (-4.8±13.7 έναντι 5.1±11.8g/day, p=0.044), καθώς και σημαντική αύξηση της πρόσληψης φυτικών ινών (3.7±2.2 έναντι -0.1±2.8gr/day, p=0.001) συγκρινόμενες με την ομάδα ελέγχου, ενώ διαγνώστηκαν 2 άτομα με διαβήτη στο τέλος του πρώτου χρόνο. Στο τέλος του δεύτερου χρόνου, στατιστικά σημαντικές διαφορές παρατηρήθηκαν στην περίμετρο μέσης (-2.3±3.5 έναντι 2.9±7.1cm, p=0.019), στη διαστολική πίεση αίματος (-1.38±14.29 έναντι 13.5±16.06mmHg, p=0.021), στην ημερήσια ενεργειακή πρόσληψη (-228.6±513.4 έναντι 264.5±462.1kcal/day, p=0.021) και στο προσλαμβανόμενο λίπος (-16±30.2 έναντι 10.8±30.3g/day, p=0.037) στην ομάδα παρέμβασης συγκρινόμενες με την ομάδα ελέγχου. Καμιά στατιστικά σημαντική διαφορά δεν παρατηρήθηκε στους άλλους κλινικούς και μεταβολικούς παράγοντες. Τέλος, η υψηλή συμμετοχή στις συναντήσεις δεν συσχετίστηκε με μεγαλύτερη συμμόρφωση. Αντίθετα, οι συμμετέχοντες της ομάδας παρέμβασης με χαμηλότερη συμμετοχή στις συναντήσεις σημείωσαν στατιστικά σημαντική συμμόρφωση στις αλλαγές του τρόπου ζωής συγκρινόμενη με εκείνων με μεγαλύτερη ανταπόκριση.ΣΥΜΠΕΡΑΣΜΑΤΑ: Αυτή η μελέτη αποδεικνύει ότι η υλοποίηση ενός κοινοτικού προγράμματος παρέμβασης στον τρόπο ζωής για την πρόληψη του διαβήτη σε ενήλικες άνω των 55 ετών είναι εφικτό, χωρίς να απαιτεί εντατικές παρεμβάσεις.

This was a cross-sectional study designed to assess the validity of the Finnish Diabetes Risk Score for detecting undiagnosed type 2 diabetes among general medical outpatients in Botswana. Participants aged ≥20 years without previously diagnosed diabetes were screened by (1) an 8-item Finnish diabetes risk assessment questionnaire and (2) Haemoglobin A1c test. Data from 291 participants were analyzed (74.2% were females). The mean age of the participants was 50.1 (SD = ±11) years, and the prevalence of undiagnosed diabetes was 42 (14.4%) with no significant differences between the gender (20% versus 12.5%,P=0.26). The area under curve for detecting undiagnosed diabetes was 0.63 (95% CI 0.55–0.72) for the total population, 0.65 (95% CI: 0.56–0.75) for women, and 0.67 (95% CI: 0.52–0.83) for men. The optimal cut-off point for detecting undiagnosed diabetes was 17 (sensitivity = 48% and specificity = 73%) for the total population, 17 (sensitivity = 56% and specificity = 66%) for females, and 13 (sensitivity = 53% and specificity = 77%) for males. The positive predictive value and negative predictive value were 20% and 89.5%, respectively. The findings indicate that the Finnish questionnaire was only modestly effective in predicting undiagnosed diabetes among outpatients in Botswana.

The New Chinese Diabetes Risk Score (NCDRS) is one of the recommended tools for screening undiagnosed type 2 diabetes in China. However, its performance in detecting undiagnosed diabetes needs to be verified in different community populations. Also, it is unknown whether NCDRS can be used in detecting prediabetes. In the present study, we aimed to evaluate the performance of NCDRS in detecting undiagnosed diabetes and prediabetes among the community residents in eastern China. We applied NCDRS in 7675 community residents aged 18-65 years old in Jiangsu Province. The results showed that the participants with undiagnosed diabetes reported the highest NCDRS value, followed by those with prediabetes (P<0.001). The best cut-off points of NCDRS for detecting undiagnosed diabetes and prediabetes were 27 (with a sensitivity of 78.0% and a specificity of 57.7%) and 27 (with a sensitivity of 66.0% and a specificity of 62.9%). The AUCs of NCDRS for identifying undiagnosed diabetes and prediabetes were 0.749 (95% CI: 0.739~0.759) and 0.694 (95% CI: 0.683~0.705). These results demonstrate the excellent performance of NCDRS in screening undiagnosed diabetes in the community population in eastern China and further provide evidence for using NCDRS in detecting prediabetes.

Abstract Background: Risk scores were mainly proved to predict undiagnosed type 2 diabetes mellitus (UT2DM) in a non-invasive manner and to guide earlier clinical treatment. The objective of the present study was to assess the performance of the Finnish Diabetes Risk Score (FINDRISC) for detecting three outcomes: UT2DM, prediabetes, and the metabolic syndrome (MS).Methods: This was a prospective, cross-sectional study during which employees aged between 30 and 64, with no known diabetes and working within the faculties of the Lebanese University (LU) were conveniently recruited. Participants completed the FINDRISC questionnaire and their glucose levels were examined using both fasting blood glucose (FBG) and oral glucose tolerance tests (OGTT). Furthermore, they underwent lipid profile tests with anthropometry.Results: Of 713 subjects, 397 subjects (55.2% female; 44.8% male) completed the blood tests and thus were considered as the sample population. 7.6% had UT2DM, 22.9% prediabetes and 35.8% had MS, where men had higher prevalence than women for these 3 outcomes (P = 0.001, P = 0.003 and P = 0.001) respectively. The AUROC value with 95% Confidence Interval (CI) for detecting UT2DM was 0.795 (0.822 in men and 0.725 in women), 0.621(0.648 in men and 0.59 in women) for prediabetes and 0.710 (0.734 in men and 0.705 in women) for MS. The correspondent optimal cut-off point for UT2DM was 11.5 (sensitivity = 83.3% and specificity = 61.3%), 9.5 for prediabetes (sensitivity = 73.6% and specificity = 43.1%) and 10.5 (sensitivity = 69.7%; specificity = 56.5%) for MS.Conclusion: The FINDRISC can be considered a simple, quick, inexpensive, and non-invasive instrument to use in a Lebanese community of working people who are unaware of their health status and who usually report being extremely busy because of their daily hectic work for the screening of UT2DM and MS. However, it poorly screens for prediabetes in this context.
