The extent to which government should partner with business interests such as the alcohol, food, and other industries in order to improve public health is a subject of ongoing debate. A common approach involves developing voluntary agreements with industry or allowing them to self-regulate. In England, the most recent example of this was the Public Health Responsibility Deal (RD), a public–private partnership launched in 2011 under the then Conservative-led coalition government. The RD was organised around a series of voluntary agreements that aim to bring together government, academic experts, and commercial, public sector and voluntary organisations to commit to pledges to undertake actions of public health benefit. This paper brings together the main findings and implications of the evaluation of the RD using a systems approach. We analysed the functioning of the RD exploring the causal pathways involved and how they helped or hindered the RD; the structures and processes; feedback loops and how they might have constrained or potentiated the effects of the RD; and how resilient the wider systems were to change (i.e., the alcohol, food, and other systems interacted with). Both the production and uptake of pledges by RD partners were largely driven by the interests of partners themselves, enabling these wider systems to resist change. This analysis demonstrates how and why the RD did not meet its objectives. The findings have lessons for the development of effective alcohol, food and other policies, for defining the role of unhealthy commodity industries, and for understanding the limits of industry self-regulation as a public health measure.

Review of the literature on public health services shows that virtually no information is available on how the state-supported networks of STD clinics now function, what the possibilities are for achieving efficiency in service delivery, or the implications of local, state, and federal funding and staffing changes. This article describes models of STD services now offered and thus allows one to project impending changes in the public health STD clinic system. The description includes a brief recount of how the imposition of HIV testing and counseling has taxed clinic resources and has sharpned the need for more efficient, technology-supported management. The status of federal staff is also summarized, with consideration of how decreases in staff will affect partner notification, the cornerstone of traditional STD-clinic-based services. Data on clinic function and staffing trends frame suggestions for the placement of computer technology in the system.

Abstract Background New developments and technologies require skilled professionals to manage and use public health information in most efficient ways. However, currently training in specific fields is lacking and large inequalities exist in Europe in terms of the availability and training of qualified professionals. Methods In order to set up a sustainable EU HI training program for HI professionals, core modules need to be selected to provide adequate training across Europe. This study a scoping review and is doing a survey on current existing HI programmes and aimes to identify the main educational components needed to train HI professionals. The databases of Pubmed and Ebsco were searched for the scoping review. The survey is being circulated to representatives of InfAct in 28 countries. Based on the results a baseline HI training programme for practicing public health professionals will be proposed. Results Initial search identified 249 papers. After the exclusion of duplicates and unavailable documents, 53 papers were selected for review. Only 5 papers were found related to the European context. However the results from other papers could be extrapolated. The results were organised in 3 domains: organisation, people and technology dimensions. The development of HI capacities is often done through multiple and sequential interventions, which are adapted to the needs, conditions and resources available within each HIS. Thus, the importance of aligning training with working processes and usability in professional practice is key. This can be challenging, as HI functions are diverse across Europe. The results of the survey will be presented when processed by the time of the workshop. Conclusions There is a need for qualified skilled professionals in HI taking into account a changing global environment. Most of HI capacity building programmes are yet to reach maturity, and this review and survey identify a set of important areas for further development of the HI training programme.

Abstract Category A agents are biological pathogens that could pose a threat to health and human safety if used as bioweapons. The exploration and possibility of these threats must be comprehensively reviewed to create a preparedness plan to recognize outbreaks, to educate the public, and to offer vaccinations and/or treatment options, if available. A scoping review using PRISMA guidelines was performed to categorize current information on Category A biological agents as well as understand their potential for future threats. The results used 34 articles and found that while botulin neurotoxins were the most lethal, anthrax posed the most likely threat for use as a bioweapon. Most research was conducted on plague, though it is not the most likely threat. Smallpox is the most likely agent to vaccinate against as there is already a working vaccine that has proven effective and the issue at hand is the need for a larger stockpile. Ultimately, preparedness efforts should include vaccinations and continued research and development of them. Category A agents are a serious public health concern; updated and reformed bioterrorism preparedness plans could greatly minimize panic and mortality.
