Abstract Background. Clinical features indicating an ischemic infarction in the territory of posterior cerebral circulation require a comprehensive radiologic examination, which is best achieved by a multi-modality imaging approach (computed tomography [CT], CT-perfusion, computed tomography angiography [CTA], magnetic resonance imaging [MRI] and diffusion weighted imaging [DWI]). The diagnosis of an acute ischemic infarction, where the damage of brain tissue may still be reversible, enables selection of appropriate treatment and contributes to a more favourable outcome. For these reasons it is essential to recognize common neurovascular variants in the territory of the posterior cerebral circulation, one of which is the artery of Percheron. Case report. A 69 year-old woman, last seen awake 10 hours earlier, presented with two typical clinical features of the artery of Percheron infarction, which were vertical gaze palsy and coma. Brain CT and CTA of neck and intracranial arteries upon arrival were interpreted as normal. A new brain CT scan performed 24 hours later revealed hypodensity in the medial parts of thalami. Other imaging modalities were not performed, due to the presumption that the window for the application of effective therapy was over. The diagnosis of an artery of Percheron infarction was therefore made retrospectively with the re-examination of the CTA of neck and intracranial arteries. Conclusions. A multi-modality imaging approach is necessary in every patient with suspicion of the posterior circulation infarction immediately after the onset of symptoms, especially in cases where primary imaging modalities are unremarkable and clinical features are severe, where follow-up examinations are indicated.

Bilateral Tessier no. 7 clefts are rarely reported in the literature. Here, we describe the presence of accessory maxilla with supernumerary teeth in a patient who exhibited bilateral Tessier no. 7 clefts; the diagnosis was established based on the patient’s history, clinical presentation, and computed tomography images. A review of the available literature revealed 24 patients with Tessier no. 7 clefts from 2000 to 2020, including our patient. The most common clinical manifestation in patients with Tessier no. 7 clefts comprises bilateral facial clefts. Additionally, Tessier no. 7 clefts are more frequently found in boys or men, rather than in girls or women. The presence of an accessory maxilla with supernumerary teeth in a patient with bilateral Tessier no. 7 clefts is extremely rare. Early detection of craniofacial abnormalities is important, because it may influence patient prognosis and management.

Introdução: Apendagite epiplóica (AE) é uma causa incomum de dor abdominal causada por alterações inflamatórias e isquêmicas relacionadas à torção ou trombose venosa dos apêndices epiplóicos. Estas estruturas consistem em projeções de tecido adiposo que emergem da superfície serosa do cólon e apêndice cecal, sendo maiores e mais numerosas no cólon descendente e sigmóide. A apendagite epiplóica do apêndice cecal (AEA) é uma forma ainda mais rara de apresentação, tendo poucos casos relatados em literatura. Os sinais e sintomas da AE comumente mimetizam outras causas de abdome agudo, que varia dependendo da sua localização, devendo ser considerada no diagnóstico diferencial de dor abdominal localizada. O diagnóstico correto é fundamental para evitar gastos e procedimentos desnecessários, pois se trata de uma condição benigna e autolimitada, com tratamento conservador sendo suficiente na grande maioria dos casos. Atualmente a tomografia computadorizada é o método de escolha na avaliação de pacientes com abdome agudo. Objetivo: Relatar um caso raro de dor abdominal aguda causada por apendagite epiplóica do apêndice cecal e a importância do seu diagnóstico correto. Relato do caso: Paciente com quadro de dor abdominal aguda no quadrante inferior direito, afebril e sem outras queixas gastrointestinais associadas. Ao exame físico referiu dor à palpação profunda no quadrante inferior direito. Exames laboratoriais sem alterações. Foram solicitados exames de imagem complementares, evidenciando apêndice cecal nos limites superiores da normalidade, inflamação periapendicular e uma imagem com densidade de gordura adjacente ao apêndice. Os diagnósticos diferenciais foram de apendicite inicial e apendagite epiplóica. Foi realizado tratamento operatório e exame anatomopatológico, que confirmou o diagnóstico de apendagite epiplóica do apêndice cecal. As informações foram obtidas por meio de revisão do prontuário, entrevista com o paciente, registro dos métodos diagnósticos, incluindo exames laboratoriais, exames de imagem como tomografia computadorizada e anatomopatológico, aos quais o paciente foi submetido e uma breve revisão da literatura. Conclusão: Relatamos um caso raro de apendagite epiplóica do apêndice cecal, demonstrando um desafio diagnóstico e a importância dos métodos de imagem. These structures consist of projections of adipose tissue that emerge from the serous surface of the colon and cecal appendix, being larger and more numerous in the descending and sigmoid colon. Epiploic appendagitis of the appendix (EAA) is an even rarer form of presentation, with few cases reported in the literature. The signs and symptoms of EAA commonly mimic other causes of acute abdomen, which varies depending on their location, and should be considered in the differential diagnosis of localized abdominal pain. Correct diagnosis is essential to avoid unnecessary expenses and procedures, as it is a benign and self-limited condition, with conservative treatment being sufficient in the vast majority of cases. Currently, computed tomography is the method of choice in the evaluation of patients with acute abdomen. Objectives: To describe a rare cause of abdominal pain due acute epiploic appendagitis of the appendix and reinforce the importance of the CT to diagnosis this condition and rule out other causes of acute abdominal pain. Case report: Patient with acute abdominal pain in the lower right quadrant, afebrile and without other associated gastrointestinal complaints. On physical examination, she reported pain on deep palpation in the lower right quadrant. Laboratory tests without changes. Complementary imaging exams were requested, showing the appendix at the upper limits of normal, periapendicular inflammation and an image with fat density adjacent to the appendix. The differential diagnoses were of initial appendicitis and epiplatic appendagitis. Operative treatment and anatomopathological examination were performed, which confirmed the diagnosis of appendagitis of the appendix. Information was obtained by reviewing medical records, interviewing the patient, recording diagnostic methods, including laboratory tests, imaging tests such as computed tomography, anatomopathology, and a brief review of the literature. Conclusion: We report a case of acute epiploic appendagitis of the appendix, demonstrating a diagnostic challenge and the importance of imaging methods.Keywords: Abdominal pain; Abdomen, acute; Appendix, Diagnostic imaging

Generally, cochlear implants (CIs) are effective in helping patients improve their hearing performance; however, some patients have poor hearing performance owing to facial nerve stimulation (FNS), which is often associated with cochlear anomalies. We report a case with a normal cochlea and severe and persistent FNS owing to cochlear–facial dehiscence (CFD) that affected the CI outcomes. Preoperatively, a careful review of the computed tomography images before CI surgery is necessary not only for patients with otosclerosis and inner ear malformations but also for patients with normal cochlear structures because facial nerve anomalies could be present.

ABSTRACT Introduction The purpose of this article was to emphasize the importance of understanding root canal anatomy and its variations for successful endodontic treatment. Methods This case report presents the detection and endodontic management of eccentrically placed second mesiobuccal (MB) canal in maxillary first molar with the help of dental operating microscope and cone-beam computed tomography (CBCT) scanning. Results Cone-beam computed tomography images showed broad MB root with second MB canal located closer to palatal orifice. Conclusion This report describes a variation in canal location and complex anatomy of maxillary first molar with posttreatment apical periodontitis which was managed successfully with the aid of dental operating microscope and CBCT. Abstract Objective Application of extended computed tomography scale (ECTS) reconstruction to diagnose metal implant failure has been described in a single case report. The purpose of this study was to compare the features and visibility of humeral transcondylar screw fractures in standard CT scale (SCTS) and ECTS images. Study Design Case series: CT images of dogs with fractured transcondylar screws were retrospectively reviewed and described in both SCTS and ECTS images. Results Five dogs with a total of six transcondylar screw failures (five right and one bilateral) were reviewed. All cases had an ongoing humeral intercondylar fissure with varying degrees of stress remodelling. The fracture was seen in all screws on ECTS images, however only in three implants on SCTS images. The measured fracture gap was larger in ECTS images in all cases (range: + 0.14 mm to + 0.28mm). The three smallest fracture gaps were not seen on SCTS images. A subtle hypoattenuating streak (artefact) was visible adjacent to the screw fracture in 5/6 of cases using SCTS images. All screw fractures occurred parallel and often slightly medial to the humeral intercondylar fissure. Conclusion Implant failure is only seen with larger fracture gaps in SCTS images, with 3/6 screw fractures not visible in SCTS compared with ECTS. A hypoattenuating streak extending perpendicular to the implant in SCTS images is suggestive of screw fracture even if this is not directly visible.

Ligamentum arteriosum calcification is the calcification or ossification of arterial ligaments. However, on computed tomography images, ligamentum arteriosum calcification is often mistaken for esophageal perforation when a patient has a medical history of foreign body ingestion. Ligamentum arteriosum calcification is uncommon in clinical practice. In this case report, we confirm the presence of this condition intraoperatively, which has seldom been reported previously. Increased awareness of the clinical characteristics of this uncommon disease can help thoracic surgeons with proper patient management.

Abstract The outbreak of coronavirus disease 2019 (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) was initially reported in Wuhan, China since December, 2019. Here, we reported a timely and comprehensive resource named iCTCF to archive 256,356 chest computed tomography (CT) images, 127 types of clinical features (CFs), and laboratory-confirmed SARS-CoV-2 clinical status from 1170 patients, reaching a data volume of 38.2 GB. To facilitate COVID-19 diagnosis, we integrated the heterogeneous CT and CF datasets, and developed a novel framework of Hybrid-learning for UnbiaSed predicTion of COVID-19 patients (HUST-19) to predict negative cases, mild/regular and severe/critically ill patients, respectively. Although both CT images and CFs are informative in predicting patients with or without COVID-19 pneumonia, the integration of CT and CF datasets achieved a striking accuracy with an area under the curve (AUC) value of 0.978, much higher than that when exclusively using either CT (0.919) or CF data (0.882). Together with HUST- 19, iCTCF can serve as a fundamental resource for improving the diagnosis and management of COVID-19 patients.Authors Wanshan Ning, Shijun Lei, Jingjing Yang, and Yukun Cao contributed equally to this work.
