Neutrophil lymphocyte ratio vs total leukocyte count as a predictor of adverse outcomes in Clostridium difficile associated diarrhea (CDAD)

Osteoarthritis, rheumatoid arthritis, and gout are frequently encountered diseases in the orthopedic clinic. CRP is done to evaluate and monitor these disease processes. This study aimed to evaluate total leukocyte count with differential count, neutrophil/lymphocyte ratio, platelet, platelet/lymphocyte ratio, mean platelet volume (MPV), platelet distribution width (PDW), and plateletcrit in patients with arthritis. This was a cross-sectional study conducted at KIST Medical College, Teaching Hospital from 15 Ashad 2075 to 14 Ashad 2076. The patient’s demographic data and laboratory findings: total leukocyte count, differential count, hemoglobin, platelet count, plateletcrit, mean platelet volume, platelet distribution width, and plateletcrit. CRP, ESR, RA factor/anti-CCP, and Uric acid tests were also performed. Among 67 patients, osteoarthritis was seen in 53 (79.1%), rheumatoid arthritis among 11(16.4%), and Gout among 3(4.5%). The ROC area under the curve was within acceptable limits for the neutrophil count, neutrophil/lymphocyte ratio, mean platelet volume, platelet distribution width. The sensitivity and specificity of the neutrophil count were 75% and 62% respectively (cut-off: 64.5%). Neutrophil/Lymphocyte ratio was increased and correlated with CRP (p-value: <0.001). PDW had a sensitivity of 90% and specificity of 83.0% (cut-off: 16.8 CV). For MPV, the sensitivity was 80% and specificity was 81% (cut-off: 9.1 fL). At 1.92 cut-off value, the sensitivity and specificity of neutrophil/lymphocyte ratio were 80.0% and 62.0% respectively. CBC parameters can provide an important clue to the treating physician, which helps to manage a patient with arthritis effectively. Neutrophil count, Neutrophil/Lymphocyte ratio, MPV, and PDW correlated with other inflammatory biomarkers and can be used to assess the patient with arthritis.

Abstract 1. Rhesus monkeys fed purified rations supplemented with adequate amounts of the B vitamins, ascorbic acid, and whole liver substance maintained the following average blood picture: See PDF for Table 2. Natural diets or purified rations supplemented with liver extract do not support the above blood picture. The hemoglobin is lower and there is an increase in the range of the total leukocyte count and in the neutrophil-lymphocyte ratio to 2.0 ([unknown]). These figures are similar to the values in the literature and generally accepted as the normal. 3. Previous reports have shown the characteristic blood dyscrasias which develop when monkeys are fed certain B vitamin-deficient diets. These changes are summarized graphically in this paper. 4. The importance of determining the concentration of hemoglobin and the formed elements of the blood as a diagnostic test in nutritional studies has been shown.

Abstract We retrospectively collected data from a large sample size of population and explore the relationship between neutrophil–lymphocyte ratio (NLR) and adverse outcomes, and assessed the clinical prognostic utility of NLR in patients with chronic obstructive pulmonary patients with acute exacerbation (AECOPD). We reviewed 3 years medical case records, 622 patients were identified including 48 died and 574 alive. Compared with alive group, the died group had significantly elevated neutrophils, lymphocyte, and NLR level (P<0.001). The high-sensitive C-protein level of died group was also higher compared with alive group (7.48 ± 4.2 vs 1.26 ± 0.56, vs P<0.001). The univariate logistic regression indicated that elevated NLR level was associated with the increased of adverse outcome (odds ratio [OR] = 4.59, 95% CI: 2.27–8.94, P<0.001). After adjusted potential confounding factors, the elevated NLR level was still associated with adverse outcomes in the chronic obstructive pulmonary patients with acute exacerbation (OR = 2.05, 95% CI: 1.21–3.48, P=0.008). The area under the receiver operating characteristic curve for death at 90 days was 0.742 (95% CI: 0.554–0.881). NLR cutoff of >4.19 had a sensitivity of 71.4% and specificity of 74.2%. Our results suggested that NLR, as a rapid, inexpensive and easily obtained blood routine index was associated with short-term adverse outcomes in patients with AECOPD. The elevated NLR predicted the increased the risk of 90-day mortality in patients with AECOPD.

Background. High value of neutrophil lymphocyte ratio (NLR) is a strong independent predictor and biomarker of ongoing vascular inflammation in various cardiovascular disorders. Objective. The main focus of the study is to investigate the effect of nebivolol on NLR in mild to moderate hypertensive patients in comparison with metoprolol. In addition, BMI, blood pressure, TLC count, blood sugar, and lipid profile were also assayed before and after treatment. Materials and Methods. In this 12-week prospective double-blinded randomized study, 120 patients with mild to moderate hypertension were randomly divided into two groups to prescribed daily dose of tab nebivolol 5–10 mg and metoprolol 50–100 mg, respectively, for 12 weeks. The data were analyzed using SPSS 16 software. Results. A total of 100 patients completed the study. Both drugs lowered blood pressure significantly, nebivolol 20.5/10.5 and metoprolol 22.5/11.2 (p<0.001) from baseline. Regarding inflammation, nebivolol reduced total leukocyte count (p=0.005) and neutrophil count (p=0.003) and increased lymphocyte count (p=0.004) as compared to metoprolol. Similarly, nebivolol but not metoprolol significantly reduced NLR ratio (p=0.07). Nebivolol improved lipid profile and blood sugar compared to metoprolol, but values were nonsignificant. Conclusion. Nebivolol has a strong impact on reducing NLR, a marker of subclinical inflammation in hypertensive patients. Moreover NLR can be used as a disease and drug monitoring tool in these patients.
