Patients who undergo surgery of femur fracture suffer the excruciating pain. Dexmedetomidine (DEX) is a unique α2-adrenergic receptor agonist with sedative and analgesic properties, whose efficacy and safety are still unclear for surgery of femur fracture. Randomized controlled trials comparing the effects of addition of DEX to general or local anesthesia in surgery of femur fracture were searched from MEDLINE, EMBASE, and the Cochrane Library database. Patients who received DEX infusion had a significant longer time to rescue analgesia compared with those without DEX coadministration. DEX treatment seemed to reduce the visual analog score; however, the significance did not reach any statistical difference. DEX as an analgesic adjuvant did not reduce the onset of sensory block time, shorten the time to achieve maximum sensory block level, and provide a longer duration of sensory block. The difference in mean sedation scores between 2 groups was not statistically significant. As for adverse effects, DEX therapy significantly increased the rate of hypotension. In conclusion, dexmedetomidine as a local anesthetic adjuvant in femur fracture surgery had a longer duration of rescue analgesia. However, the incidence of hypotension was markedly increased in these patients. It was worth noting that the evidence was of low to moderate quality.

A proof-of-work blockchain uses a retargeting algorithm, also termed a difficulty adjustment algorithm, to manage the rate of block production in the presence of changing hashrate. To derive the parameters that guide the search for the next block, nearly all such algorithms rely on averages of past inter-block time observations, as measured by on-chain timestamps. We are motivated to seek better responsiveness to changing hashrate, while improving stability of the block production rate and retaining the progress-free property of mining. We describe a class of retargeting algorithms for which the sole inter-block time input is that of the block being searched for, and whose response is nonlinear in that time. We discuss how these algorithms allow the other consensus rules that govern allowable timestamps to be tightened, which may improve the blockchain’s effectiveness as a time-stamping machine.

Tujuan dari penelitian ini adalah mengetahui pengaruh latihan plyometrics terhadap hasil tolakan start dalam olahraga renang terutama pada block time phase. Sampel diambil sebanyak delapan orang atlet putra dari UKM AQUATIK UPI dengan menggunakan teknik Purposive Sampling. Metode penelitian ini menggunakan penelitian kuantitatif dengan pendekatan eksperimental one group pretest posttest desingn. Pengambilan data dilakukan menggunakan Digital-Kamera sebanyak dua kali pada saat pretest dan pada saat postest. Test terdiri atas tiga jenis phase yaitu block time phase, flight time phase, dan glide time phase. Tahap yang diukur adalah teknik grab start meliputi data tolakan start  terutama pada block time phase. Masing-masing tahap penilaian pada block time phase yaitu jarak tempuh, kecepatan rotasi sendi lutut, impuls, leg power, ground reaction force (GRF). Kemudian data berupa rekaman di input menggunakan software frame dias IV. Perhitungan statistik menggunakan SPSS dengan sub menu Independent Sample T-test. Uji hipotesis keseluruhan digabungkan dengan T-Score t = 21,903, p = 0,000 < 0,05 maka Ho ditolak, artinya Terdapat pengaruh yang nyata (signifikan) Setelah dilakukan analisis data diperoleh nilai (p) < 0,05. Berdasarkan hasil tersebut penelitian ini menyimpulkan bahwa terdapat pengaruh latihan plyometrics terhadap hasil tolakan start dalam olahraga renang terutama pada block time phase

Standard scheduled flight block time (SBT) setting is of great concern for Civil Aviation Administration of China (CAAC) and airlines in China. However, the standard scheduled flight block times are set in the form of on-site meetings in practice and current literature has not provided any efficient mathematical models to calculate the flight block times fairly among the airlines. The objective of this paper is to develop and solve a mathematical model for standard SBT setting with consideration of both fairness and reliability. We use whale optimization algorithm (WOA) and an improved version of the whale optimization algorithm (IWOA) to solve the SBT setting problem. A novel nonlinear update equation of convergence factor for random iterations is used in place of the original linear one in the proposed IWOA algorithm. Experimental results show that the suggested approach is effective, and IWOA performs better than WOA in the concerned problem, whose solutions are better compared to the flight block times released by CAAC. In particular, it is interesting to find that MSE, RMSE, MAE, MAPE and Theil of the reliability in 60%–70% range are always the smallest and the average fairness of airlines is better than that of 60%–75% range. The model and solving approach presented in this article have great potential to be applied by CAAC to determine the standard SBTs strategically.

Blockchain is emerging as a promising candidate for the uberization of Internet services. It is a decentralized, secure, and auditable solution for exchanging, and authenticating information via transactions, without the need of a trusted third party. Therefore, blockchain technology has recently been integrated with industrial Internet-of-things (IIoT) networks to help realize the fourth industrial revolution, Industry 4.0. Though blockchain-enabled IIoT networks may have the potential to support the services and demands of next-generation networks, the gap analysis presented in this work highlights some of the areas that need improvement. Based on these observations, the article then promotes the utility of reinforcement learning (RL) techniques to address some of the major issues of blockchain-enabled IIoT networks such as block time minimization and transaction throughput enhancement. This is followed by a comprehensive case study where a Q-learning technique is used for minimizing the occurrence of forking events by reducing the transmission delays for a miner. Extensive simulations have been performed and the results have been obtained for the average transmission delay which relates to the forking events. The obtained results demonstrate that the Q-learning approach outperforms the greedy policy while having a reasonable level of complexity. To further develop the blockchain-enabled IIoT networks, some future research directions are also documented. While this article highlights the applications of RL techniques in blockchain-enabled IIoT networks, the provided insights and results could pave the way for rapid adoption of blockchain technology.
