SummaryA prospective study compared real-time B-mode ultrasound examination with bilateral ascending phlebography in the diagnosis of postoperative deep vein thrombosis in 60 patients undergoing elective total hip replacement. The overall sensitivity and specificity of the ultrasound method were 54 and 91%, respectively, and the positive and negative predictive values were 83 and 69%, respectively. The rather low overall sensitivity of the ultrasound method in this study was due to difficulty in detecting thrombi smaller than 1 cm wherever they were located in the deep veins, and in diagnosing thrombi in the calf, regardless of their size. We conclude that real-time B-mode ultrasonography is a technique that can easily be used routinely for detection of postoperative DVT in hip surgery, but its sensitivity for proximal thrombosis (63%) is too low for it to be used alone.

SummaryTo determine their ability to diagnose postoperative deep vein thrombosis (DVT) D-dimer – by three methods – fibrinogen degradation products (FgDP) and fibrinogen levels were measured in 68 consecutive patients before elective surgery for hip replacement and on postoperative day 1, 3, 6, and 10. All patients received prophylaxis and underwent compression real-time B-mode ultrasonography (C-US) on postoperative day 5 and 9, and bilateral ascending venography on day 10. Twenty-two out of 68 patients developed asymptomatic postoperative DVT, which was limited to the calf veins in 14 and involved the proximal veins in 8 patients. C-US was negative in all patients on day 5. On day 9, C-US sensitivity and specificity for proximal DVT were 63% (95% confidence interval: 26%-90%) and 98% (89%-100%) respectively. Postoperative changes in the laboratory parameters evaluated were not different in patients with or without DVT until day 10. On day 10, mean D-dimer, FgDP and fibrinogen levels were significantly higher in patients with DVT than in those without DVT (p values between 0.006 and 0.032), but only D-dimer was higher with DVT involving two or more venous segments than with thrombosis involving one venous segment only (p <0.05). Stepwise logistic regression analysis identified D-dimer and fibrinogen on day 10 as predictors of postoperative DVT. In a receiver operator curve and after weighing for the coefficients generated by logistic regression analysis, the combination of a latex photometric immunoassay and of PT-derived fibrinogen yielded - at a cut-off value of 7.0 - a sensitivity of 100% (73%-100%) and a specificity of 58% (39%-75%) for DVT, with a negative predictive value of 100% (78%-100%), a positive predictive value of 52% (32%-71%) and an overall accuracy of 71% (55%-83%). These results suggest that two simple, fast and reproducible tests may permit the identification of patients at low risk of having postoperative DVT and that a combination of sensitive laboratory assays and of the highly specific C-US may select patients requiring anticoagulant treatment. Efficacy and cost-effectiveness of this approach should be evaluated in large clinical management studies.

Abstract Abstract 2578 Background: Thromboprophylaxis is the top challenge to patient safety practice in hospitals. Postoperative Deep vein thrombosis (DVT) and pulmonary embolism (PE) are the major cardiovascular killers in the surgical setting. In October 2008, Medicare designated DVT and PE occurring after total knee or hip replacement as ‘never events', and indicated that they will not pay the incremental cost to manage the complication and has made the hospital acquired DVT/PE unacceptable and serious. There are however, limited data on factors contributing to DVT/PE in-patients undergoing total knee or hip replacement. Aim: To ascertain nationwide health care utilization and associated co-morbidities in Total knee replacement (TKR) recipients who do or do not develop DVT/PE. Methods: We used the year 2007, National Inpatient Sample (NIS) to analyze the post operative occurrence of DVT/PE after TKR. We used an analysis similar to AHRQ's Patient Safety Indicator number – 12 (PSI 12) which is Postoperative Pulmonary Embolism or Deep Vein Thrombosis but restricted our analysis only to TKR. We intended to capture cases of postoperative venous thromboses and embolism - specifically, pulmonary embolism and deep venous thrombosis. For our analysis we separated TKR into 2 groups; one without DVT/PE and the other with DVT/PE. We analyzed all surgical discharges age 18 years and older with an ICD-9-CM code for an operating room procedure TKR (ICD-9 8154). From this we excluded those who have principal diagnosis of DVT/PE, as these patients are likely to have had PE/DVT present on admission and not because of TKR and also where a procedure for interruption of vena cava (IVC filter) (ICD-9 387) occurs before or on the same day as the first operating room procedure as these patients likely had DVT/PE even before TKR. We then created a subset from the first group, with discharges ICD-9-CM codes for deep vein thrombosis or pulmonary embolism in any secondary diagnosis field and thus defined the group of patients who developed DVT/PE after TKR. We used the following ICD-9 codes to represent DVT (ICD-9 codes 451.11, 451.19, 451.2, 451.81, 451.9, 453.40, 453.41, 453.42, 453.8, 453.9) and PE (ICD-9 codes 415.1–415.19). IBM SPSS Statistics 18 was used for data mining and analysis. Result: In the year 2007, there were 550,770 discharges with a procedure for TKR. After excluding primary diagnosis of DVT/PE and IVC filter, we had 550228 as our working number. Of these, 5454 discharges had a secondary diagnosis of DVT/PE (Rate - 10 new cases per 1000 TKR procedures). Demographics and health care utilization between those who did or did not develop are described in Table 1. Co-morbidities associated with those who did or did not develop DVT are described in (Table 2). Conclusion: DVT & PE are major avoidable complications of Total Knee replacement and are associated with significant mortality and health care costs. These data demonstrate that there may not be any significant differences in age and associated co-morbidities between those who do or do not develop DVT/PE following total knee replacement except for UTI which can be attributed to the difference in length of stay. The absence of serious co-morbidities like AF and CHF in both groups suggest those with serious co-morbidities may not be receiving total knee replacement. That no differences were noticed in associated co-morbidities among those who did or did not develop DVT/PE following TKR provide the rationale for further study of factors contributing to this serious complication of TKR. Such studies may inform future strategies for prevention of post-operative DVT/PE. Disclosure: No relevant conflicts of interest to declare.

INTRODUCTION The merit of using D-dimer estimations in patients following total hip and knee replacement has been previously questioned. A survey of radiology protocols in 70 hospitals ascertained that the test continues to be mandatory in 51 prior to formal radiological investigation of suspected deep vein thrombosis (DVT) in this patient population. PATIENTS AND METHODS In this study, D-dimer levels were measured pre-operatively and during the first 7 days following primary total hip and knee replacement to estimate the range of normal values in these patients. RESULTS All 78 patients were ultrasonographically shown to be clear of DVT. D-Dimer levels were significantly raised in all patients at all measurements during the first week. Levels after total knee replacement were higher than after total hip replacement. Comparison was made with an age-matched group who had a proven DVT. There was no difference in D-dimer levels between patients with or without a DVT. CONCLUSIONS The use of this test in this patient group is a waste of resource and merely delays appropriate radiological investigation and treatment.
