Számtalan nemzetközi vizsgálat igazolta, hogy az intézetekből örökbefogadott gyerekek, bár sok területen jelentős fejlődést mutatnak, még évekkel családba kerülésük után is sok szocioemocionális problémával küzdenek. Nagyon keveset tudunk azonban azokról a folyamatokról, amelyeken keresztül a korai élmények ezekhez a fejlődési problémákhoz vezetnek. Az itt bemutatott vizsgálat célja az érzelemregulációs képességek fejlődésének jobb megismerése örökbefogadott gyermekeknél, mivel ezen képességek alapvető feltételei a pszichés egészségnek, a hatékony társas működésnek. A vizsgálat központi hipotézise, hogy azok a gyerekek, akik életük első (minimum) 6 hónapját intézetben töltötték, fejletlenebb érzelemregulációs képességgel rendelkeznek. A vizsgálatban 90 4 és 6 év közötti gyerek vett részt, akik a 3 vizsgálati csoport egyikébe tartoztak: 1. olyan gyerekek, akik születésükkor gyermekotthonba kerültek, és ott éltek örökbefogadásukig, de leg¬alább 6 hónapos korukig; 2. csecsemőkorban (6 hetes koruk előtt) örökbefogadott gyerekek; 3. (kontroll) vér szerinti családjukban élő gyerekek. A vizsgálat során az érzelemregulációs képességet a játék-narratívák elemzésével (MacArthur Story Stem Battery) vizsgáltuk. A három vizsgálati csoport összehasonlításának eredményeit összefoglalva elmondható, hogy igazolódott az a hipotézis, miszerint az intézeti gondozás egyik fontos következménye az elmaradás az érzelemregulációs képességekben. Ugyanakkor az eredmények arra is felhívják a figyelmet, hogy az újszülött korban örökbefogadott gyerekek bizonyos érzelemregulációs képességei is, bár jóval kevésbé, de eltérnek a vér szerinti kontrollcsoportétól. Ez az eredmény összefüggésbe hozható egyrészt pre- és perinatális tényezőkkel, másrészt pedig az örökbefogadó szülők és család jellegzetességeivel, valamint az örökbefogadottság tényéből következő vulnerabilitással.

Early adversity, including institutional orphanage care, is associated with the development of internalizing disorders. Previous research suggests that institutionalization can disrupt emotion regulation processes, which contribute to internalizing symptoms. However, no prior work has investigated how early orphanage care shapes emotion regulation strategy usage (e.g., cognitive reappraisal, expressive suppression) and whether said strategy usage contributes to internalizing symptoms. The present study probed emotion regulation strategy usage and internalizing symptoms in a sample of 36 previously institutionalized and 58 comparison youth. As hypothesized, previously institutionalized youth exhibited higher rates of internalizing symptoms than comparison youth, and more frequent use of suppression partially accounted for the relationship between early institutional care and elevated internalizing symptoms. Contrary to our initial hypotheses, reappraisal use did not buffer previously institutionalized or comparison youth against internalizing symptoms. Our findings highlight the potential utility of targeting emotion regulation strategy usage in adversity-exposed youth in future intervention work.

The participation of patients in their treatment and care is perceived as desirable; however, patients with mental illnesses experience limited opportunities to participate in their own care. As nurses play a key role in taking care of patients with mental illnesses, this study aimed to investigate how nurses within psychiatric care settings experience patients' participation and how they act to increase it. Semi-structured interviews were conducted with eight registered nurses, four of whom worked in a psychiatric institutional care setting and four of whom worked with outpatients. Data were analysed using a qualitative content analysis approach. The analysis identified a theme: a caring relationship is a prerequisite for patient participation. This theme was further developed through five subthemes. The findings illustrate critical aspects of the caring relationship by which the nurse–patient relationship can either facilitate or impede patient participation. A caring relationship builds trust and increases the patient's sense of responsibility for their own condition. Because patients with mental illness are cared for in many different contexts, the results of the present study have implications for a broad range of healthcare environments.

AbstractChildren reared in orphanages typically experience the lack of stable, reliable caregivers and are at increased risk for deficits in regulatory abilities including difficulties in inhibitory control, attention, and emotion regulation. Although adoption results in a radical shift in caregiving quality, there remains variation in postadoption parenting, yet little research has examined postadoption parenting that may promote recovery in children experiencing early life adversity in the form of institutional care. Participants included 93 postinstitutionalized children adopted between 15 and 36 months of age and 52 nonadopted same-aged peers. Parenting was assessed four times during the first 2 years postadoption (at 2, 8, 16, and 24 months postadoption) and children's regulation was assessed at age 5 (M age = 61.68 months) and during kindergarten (M age = 71.55 months). Multiple parenting dimensions including sensitivity/responsiveness, structure/limit setting, and consistency in routines were examined. Both parental sensitivity and structure moderated the effect of preadoption adversity on children's emotion regulation while greater consistency was associated with better inhibitory control and fewer attention problems. Results support the notion that postadoption parenting during toddlerhood and the early preschool years promotes better regulation skills following early adversity.

Alzheimer's disease is neurodegenerative disorder which affects a growing number of older adults every year. With an understanding of auditory dysfunction in Alzheimer's disease, the speech-language pathologist working in the health care setting can provide better service to these individuals. The pathophysiology of the disease process in Alzheimer's disease increases the likelihood of specific types of auditory deficits as opposed to others. This article will discuss the auditory deficits in Alzheimer's disease, their implications, and the value of clinical protocols for individuals with this disease.
