ObjectivesTo evaluate the subjective impact on quality of life and burden of caregivers of chronic psychotic patients and correlate with the impact that the type of patient’s symptoms according to PANSS (negative, positive, mixed or none).MethodsWe used the Caregiver's Burden scale to measure quality of life and subjective burden of caregivers of chronic psychotic patients followed up by the Psychotic Disorders Clinic of the Hospital Mental Health Messejana, in Fortaleza, Ceará, and the PANSS for the patients of the relatated caregiver. Then we correlated the data in order to observe whether the patient’s symptoms interfere with the quality of life and the burden of the caregivers.ResultsWe observed that most caregivers were women, mothers of patients who had incomplete primary schooling, married, with incomes between one and two minimum wages. In most cases also with incomplete primary degree and mean disease duration was 16.51 (± 9.64) years. We found that higher scores on scales of positive and PANSS general psychopathology in the patient, the worse the quality of life and caregiver burden. In this sample, negative symptoms measured by the PANSS negative scale does not seem to interfere with quality of life and caregiver burden.OuvirLer foneticamente Dicionário - Ver dicionário detalhadoConclusionDemographic data did not affect the quality of life of caregivers; the main factors that contributed to a poorer quality of life and caregiver's burden were high scores in positive and general psychopathology of PANSS.

Abstract Background The objective of this study was to investigate the effects of group arts therapy based on emotion management training on emotional expression, positive emotion and negative emotion, alexithymia, depression and quality of life in patients with schizophrenia. Methods 24 of 160 in-patients with schizophrenia, according to DSM-IV from H Mental Health Hospital, were randomly assigned to either an experimental or control group. Each group were consisting of 12 patients. Group arts therapy was conducted on the experimental group twice a week, 60 minutes per session, for a total of 16 sessions, while the control group was left untreated. The following scales were used for assessment: Berkeley Expressivity Questionnaire (BEQ), Positive Affective and Negative Affect Schedule (PANAS), Toronto Alexithymia Scale-Korean vesion (TAS-20K), Depression Scale for Schizophrenia (K-CDSS), Positive and Negative Syndrome Scale (PANSS), and Schizophrenia Quality of Life Scale (SQLS-R4K). Independent t-test was conducted to confirm the homogeneity, and to find the effects of group arts therapy, Repeated measures ANOVA was conducted to confirm the differences for scores of each scales regarding groups, measuring timing, and also the interaction between groups and measuring timing by pre, post and follow-up test. Results The result of this study are as follows. First, total score, expressivity factors and impulse strength factors of emotional expressivity were significantly increased after group arts therapy compared to the control group. Second, positive emotion was significantly increased and negative emotion was significantly decreased after group integrative therapy compared to the control group. Third, total score, difficulty identifying feelings of Alexithymia were significantly decreased after group arts therapy compared to the control group. Fourth, depression was significantly decreased after group integrative arts therapy compared to the control group. Fifth, negative syndromes and general psychopathology were significantly decreased after group arts therapy compared to the control group. Sixth, quality of life was significantly increased after group arts therapy compared to the control group. Discussion The group arts therapy have significantly improved the emotional expression, positive emotion, negative emotion and Alexithymia in patients with schizophrenia and also improved negative syndromes, general psychopathology, depression and quality of life. These result suggest that group arts therapy based on emotion management training could be a useful intervention for emotional disturbance treatment for in patients with schizophrenia.

Abstract Psychotic experiences (PEs) are common in the general population in preadolescence. The implications of PEs on socioeconomic outcomes, including educational attainment, are scarcely described. We aimed to estimate how preadolescent PEs were associated with later healthcare costs, school performance, and health-related quality of life (HRQoL) in adolescence. A total of 1607 preadolescents from the general population Copenhagen Child Cohort 2000 were assessed for PEs at age 11–12 years and followed up over 5 years using register-based data on mental and somatic healthcare costs, and school performance at age 16. Furthermore, HRQoL was assessed for a subsample of the children at age 16–17. We adjusted for perinatal and family sociodemographic adversities, prior parental mental illness and healthcare use, child IQ-estimate at age 11–12, and parent-rated general psychopathology of their child. PEs were associated with slightly poorer school performance. However, preadolescents with PEs more often reported HRQoL within the lowest 10th percentile (OR = 2.74 [95% CI 1.71–4.37]). Preadolescents who reported PEs had higher average total healthcare costs over the following 5 years. The costs for individuals with PEs were higher for mental healthcare services across primary to tertiary care, but not for somatic care. After adjustments, PEs remained independently associated with higher costs and poorer HRQoL, but not with poorer school performance. In conclusion, PEs are important in mental health screening of preadolescents and identify a group of young people with increased healthcare service-use throughout adolescence and who report poorer HRQoL in adolescence, over and above parent-rated general psychopathology of their child.

AbstractWe assessed the subjective quality of life (QOL) of 30 deficit schizophrenic patients compared to 112 nondeficit schizophrenic patients. The deficit patients did not differ in term of QOL, total score of positive symptoms, general psychopathology from the nondeficit patients. This result suggested an absence of impact of primary negative symptoms on the subjective QOL in schizophrenic patients.

Se presentan cinco casos de pacientes con hipotiroidismo primario y altos requerimientos de levotiroxina, que luego de haber tenido un abordaje convencional en el ajuste de las dosis necesarias de levotiroxina por el peso y etiología de hipotiroidismo, recomendación y educación para el consumo adecuado de levotiroxina, optimización de la calidad del medicamento recibido, eliminación de interferencias con la absorción por medicamentos o alimentos, reposición de deficiencia de vitamina B12, desparasitación, descartar enfermedad celíaca y gastritis autoinmune atrófica, persisten con TSH elevada pese al uso de dosis altas de levotiroxina (mayores de 2,5 µg/kg/día). Se les practicó prueba de absorción de levotiroxina con 1.000 µg en dosis única de carga y evaluación de los incrementos de T4 libre a las 2 y 4 horas en cuatro pacientes y en uno con suministro semanal de la dosis de levotiroxina y evaluación semanal de T4 libre. Se confirmó en los cinco pacientes la capacidad de absorción de levotiroxina, constituyendo cinco casos de pseudomalabsorción de levotiroxina.Abstract Five patients with primary hypothyroidism and high levothyroxine dose requirements (>2.5 µg/kg/day) are presented. All patients had usual adjustment of the levothyroxine dose according to weight and hypothyroidism etiology. Patients were educated about the appropriate way of drug intake, the quality of the hormone replacement was assessed and withdrawal of all medications potentially interrupting with the levothyroxine absorption was done. All patients had vitamin B12 replacement, were treated for gastrointestinal parasites and celiac disease and autoimmune atrophic gastritis was ruled out. Despite correction of all this factors these patients had elevated TSH. To evaluate a potential mala-absorptive case all patients underwent a levothyroxine absorption test. In 4 cases, 1000 µg of levothyroxine was administered and free T4 was evaluated at 2 and 4 hours after intake of the drug. In 1 case, the levothyroxine was administered weekly with free t4 evaluation. Appropriate levothyroxine absorption was confirmed in all cases and the diagnosis of pseudo mal-absorption of levothyroxine was done.
