AbstractObjectivesThe goal of this study was to examine the mental health needs of children and youth who present to the emergency department (ED) for mental health care and to describe the type of, and satisfaction with, follow-up mental health services accessed.MethodsA 6-month to 1.5-year prospective cohort study was conducted in three Canadian pediatric EDs and one general ED, with a 1-month follow-up post-ED discharge. Measures included 1) clinician rating of mental health needs, 2) patient and caregiver self-reports of follow-up services, and 3) interviews regarding follow-up satisfaction. Data analysis included descriptive statistics and the Fisher’s exact test to compare sites.ResultsThe cohort consisted of 373 children and youth (61.1% female; mean age 15.1 years, 1.5 standard deviation). The main reason for ED presentations was a mental health crisis. The three most frequent areas of need requiring action were mood (43.8%), suicide risk (37.4%), and parent-child relational problems (34.6%). During the ED visit, 21.6% of patients received medical clearance, 40.9% received a psychiatric consult, and 19.4% were admitted to inpatient psychiatric care. At the 1-month post-ED visit, 84.3% of patients/caregivers received mental health follow-up. Ratings of service recommendations were generally positive, as 60.9% of patients obtained the recommended follow-up care and 13.9% were wait-listed.ConclusionsChildren and youth and their families presenting to the ED with mental health needs had substantial clinical morbidity, were connected with services, were satisfied with their ED visit, and accessed follow-up care within 1-month with some variability.

Wellness and resilience promotion efforts are often an underutilized opportunity to address the mental health needs of children and youth. Organizations and individuals with state-level reach are ideally poised to develop and disseminate infrastructure, training, and resources to support wellness and resilience promotion efforts. This chapter highlights 8 key systems and practices that can inform and support such statewide efforts. Examples within Ohio, a recipient of 2 federal grants to support behavioral and mental health promotion, are used to illustrate the eight systems and practices. The chapter incorporates recommendations for others seeking to promote wellness and resilience in their respective contexts.

AbstractChildren in armed conflict are frequently deprived of basic needs, psychologically supportive environments, educational and vocational opportunities, and other resources that promote positive psychosocial development and mental health. This article describes the mental health challenges faced by conflict-affected children and youth, the interventions designed to prevent or ameliorate the psychosocial impact of conflict-related experiences, and a case example of the challenges and opportunities related to addressing the mental health needs of Rohingya children and youth.

Recent years have seen a surge in interest in mental healthcare and some reduction in stigma. Partly as a result of this, alongside a growing population and higher levels of societal distress, many more people are presenting with mental health needs, often in crisis. Systems that date back to the beginning of the National Health Service still form the basis for much care, and the current system is complex, hard to navigate and often fails people. Law enforcement services are increasingly being drawn into providing mental healthcare in the community, which most believe is inappropriate. We propose that it is now time for a fundamental root and branch review of mental health emergency care, taking into account the views of patients and the international evidence base, to ‘reset’ the balance and commission services that are humane and responsive – services that are fit for the 21st century.

BACKGROUND Electronic health (eHealth) tools have the potential to meet the mental health needs of individuals who have barriers to accessing in-person treatment. However, most users have less than optimal engagement with eHealth tools. Coaching from peer specialists may increase engagement with eHealth. OBJECTIVE This pilot study aimed to 1) test the feasibility and acceptability of a novel, completely automated online system to recruit, screen, enroll, assess, randomize and then deliver an intervention to a national sample of military veterans with unmet mental health needs, 2) investigate whether phone-based peer support increased usage of an online problem-solving training compared to self-directed use and 3) generate hypotheses about potential mechanisms of action for problem-solving and peer support for future full-scale research. METHODS Veterans (n=81) with unmet mental health needs were recruited via social media advertising and enrolled and randomized to self-directed use of an online problem-solving training called Moving Forward (n=28), peer supported Moving Forward (n=27), or a waitlist control (n=26) (ClinicalTrials.gov NCT03555435). Participants completed pre and post study measures (8 weeks later) of problem-solving skills and confidence as well as mental health symptoms. Satisfaction was assessed at post-treatment and objective use of Moving Forward was measured with number of log-ins. RESULTS Automated recruitment, enrollment and initial assessment methods were feasible and resulted in a diverse sample of veterans with unmet mental health needs from 38 states. Automated follow-up methods resulted in 46% retention. Peer support was delivered with high fidelity and was associated with favorable patient satisfaction. Participants randomized to receive peer support had significantly more Moving Forward logins than self-directed Moving Forward participants, and those who received peer support had greater decreases in depression. Problem-solving confidence was associated with greater Moving Forward use and improvements in mental health symptoms among participants both with and without peer support. CONCLUSIONS Enrolling and assessing individuals in eHealth studies without human contact is feasible, but different methods or designs are necessary to achieve acceptable participant engagement and follow-up rates. Peer support shows potential for increasing engagement in online interventions and in reducing symptoms. Future research should investigate when and for whom peer support of eHealth is helpful. Problem-solving confidence should be further investigated as a mechanism of action for online problem-solving training. CLINICALTRIAL ClinicalTrials.gov NCT03555435
