AbstractSaliva and urine are the two main body fluids sampled when breast milk intake is measured with the 2H oxide dose-to-mother technique. However, these two body fluids may generate different estimates of breast milk intake due to differences in isotope enrichment. Therefore, we aimed to assess how the estimated amount of breast milk intake differs when based on saliva and urine samples and to explore whether the total energy expenditure of the mothers is related to breast milk output. We used a convenience sample of thirteen pairs of mothers and babies aged 2–4 months, who were exclusively breastfed and apparently healthy. To assess breast milk intake, we administered doubly labelled water to the mothers and collected saliva samples from them, while simultaneously collecting both saliva and urine from their babies over a 14-d period. Isotope ratio MS was used to analyse the samples for 2H and 18O enrichments. Mean breast milk intake based on saliva samples was significantly higher than that based on urine samples (854·5 v. 812·8 g/d, P = 0·029). This can be attributed to slightly higher isotope enrichments in saliva and to a poorer model fit for urine samples as indicated by a higher square root of the mean square error (14·6 v. 10·4 mg/kg, P = 0·001). Maternal energy expenditure was not correlated with breast milk output. Our study suggests that saliva sampling generates slightly higher estimates of breast milk intake and is more precise as compared with urine and that maternal energy expenditure does not influence breast milk output.

Adherence to healthy behaviors is a protective factor in the disease progression of chronic kidney disease (CKD). Measuring adherence can lead to the recognition of unhealthy behaviors and the suggestion of programs to prevent poor health outcomes. An assessment measurement for patients with CKD not requiring dialysis was developed and psychometrically tested. A convenience sample ( n = 330) of patients with CKD attending a nephrology clinic in southern Taiwan completed the 13-item Adherence to Healthy Behaviors Scale (AHBS). A principal axis factor analysis and a parallel analysis demonstrated a three-factor structure accounting for 47.16% of the total variance. Confirmatory factor analysis indicated a good model fit. The criterion-related validity was adequate ( r = .51; p < .000), with a Cronbach’s alpha of .70; the test-retest reliability demonstrated good stability ( r = .70; p < .000). The AHBS is a valid, reliable instrument to assess adherence to healthy behaviors among patients with CKD.

ABSTRACT Objective: To assess the physical activity level (PAL) and the total daily energy expenditure (EE-TDEE) in a sample of ≥60y subjects from Niterói, Rio de Janeiro, Brazil. Methods: A convenience sample of 88 subjects recruited from recreational physical activity programs wore an accelerometer around the waist for seven consecutive days for at least 10h/day. Minute-by-minute EE was estimated from the counts per minute (CPM) data, and the daily sum yielded the TDEE. PAL (TDEE/BMR) with BMR calculated with the FAO/WHO predictive equation and a population-specific equation. Body composition was assessed by DXA. Results: Mean age (SD) was 69.2 (5.8) years, the prevalence of overweight and obesity was 36.4 and 25.0%, respectively, and excess body fat was 39.8%. The subjects spent 600min/day engaged in sedentary activities (CPM<100). Men engaged in 30min of moderate-to-vigorous physical activity (CPM≥1,952) daily, on average. The subjects were active on 34.5 and 18.0% of the weekdays and weekend days with a 1,400 steps/day difference between these days. TDEE was 1,731.5 (348.7) and 1,356.3 (223.7) kcal/day depending on the BMR prediction equation used. Mean PAL was lower than the maintenance level. Conclusions: The high prevalence of sedentary activities and the low percentage of subjects who met the physical activity recommendations indicate that physical activity programs must be adjusted so that the enrolled subjects can meet the physical activity recommendations, preferably with the activities objectively monitored. Population-specific equations improve the final estimation of TDEE and PAL. Level of Evidence I; Diagnostic studies - Investigating a diagnostic test.

Abstract Objective This study aimed to investigate characteristics of human cytomegalovirus (HCMV) glycoprotein H (gH) genotypes in urine, throat swab, and serum from children and breast milk from children's mothers. Methods Fresh urine samples, throat swabs, or serum samples from children and breast milk samples from children's mothers were collected for HCMV DNA detection. The positive samples of HCMV DNA were further detected by fluorescent quantitative polymerase chain reaction (PCR) with gH typing. Results Of 1,703 HCMV DNA-positive samples, the highest proportion (83.3%, 85/102) of children aged between 21 days and 3 months was detected positive in breast milk samples (p = 0.002), and the highest proportion (70.5%, 110/156) of children aged above 3 months was detected positive in throat swab samples (p = 0.002). HCMV in throat swab specimens is mainly high copy (p < 0.0001), and low-copy HCMV is prevalent in breast specimens (p < 0.0001). Among them, 1,059 samples were identified as gH1 genotype, 530 samples were gH2, and 114 samples were coinfection (gH1/2). There had the highest gH2 rates (32.3%) and lowest gH1 (61.0%) rates in urine samples (p = 0.041), whereas the highest gH1 rates (71.6%) and lowest gH2 rates (19.6%) were found in breast milk samples (p = 0.032). Concerning age groups, patients aged between 21 days and 3 months had the highest gH1 proportion (p = 0.017), while patients aged above 3 months had the highest gH1 and gH2 HCMV coinfection proportion (p = 0.002). Among 43 pairs of maternal and child samples corresponding to positive samples, gH genotype of 35 pairs of samples was consistent with a rate of 81.4%. Conclusion gH1 is the predominant genotype of HCMV in each kind of sample in China. However, the distribution of the HCMV gH genotype is different among different samples.
