Autonomic activity, posttraumatic and nontraumatic nightmares, and PTSD after trauma exposure

Abstract Background Nightmares are a hallmark symptom of posttraumatic stress disorder (PTSD). This strong association may reflect a shared pathophysiology in the form of altered autonomic activity and increased reactivity. Using an acoustic startle paradigm, we investigated the interrelationships of psychophysiological measures during wakefulness and PTSD diagnosis, posttraumatic nightmares, and nontraumatic nightmares. Methods A community sample of 122 trauma survivors were presented with a series of brief loud tones, while heart rate (HRR), skin conductance (SCR), and orbicularis oculi electromyogram (EMGR) responses were measured. Prior to the tone presentations, resting heart rate variability (HRV) was assessed. Nightmares were measured using nightmare logs. Three dichotomous groupings of participants were compared: (1) current PTSD diagnosis (n = 59), no PTSD diagnosis (n = 63), (2) those with (n = 26) or without (n = 96) frequent posttraumatic nightmares, and (3) those with (n = 22) or without (n = 100) frequent nontraumatic nightmares. Results PTSD diagnosis was associated with posttraumatic but not with nontraumatic nightmares. Both PTSD and posttraumatic nightmares were associated with a larger mean HRR to loud tones, whereas nontraumatic nightmare frequency was associated with a larger SCR. EMGR and resting HRV were not associated with PTSD diagnosis or nightmares. Conclusions Our findings suggest a shared pathophysiology between PTSD and posttraumatic nightmares in the form of increased HR reactivity to startling tones, which might reflect reduced parasympathetic tone. This shared pathophysiology could explain why PTSD is more strongly related to posttraumatic than nontraumatic nightmares, which could have important clinical implications.

The literature indicates stress as a response of the organism to a stimulation that requires enormous efforts to adapt to the changes in the environment and the body. When an individual is subjected to stress, the autonomic nervous system is triggered, the sympathetic pathway is activated, and the parasympathetic system is suppressed, which exerts several effects on the cardiovascular system and affects heart rate variability. This research aimed to conduct a literature review to find and analyze the studies that address clearly the implications of stress on heart rate variability. The methodology employed was an active search in the databases SciELO, PubMed and Lilacs. The results were five articles, most of which suggest a relationship between stress and heart rate variability. We observed that the majority of the studies indicated a strong association between stress and cardiac autonomic activity. The stress is present in the daily activities of the population, especially in labor. The subject is vast, however, were observed in the references the effects of stress on the body making it vulnerable to diseases. Thus, this information may contribute to the aid of preventive strategies against stress and diseases of the cardiovascular system.

Abstract. We hypothesized that in healthy subjects differences in resting heart rate variability (rHRV) would be associated with differences in emotional reactivity within the medial visceromotor network (MVN). We also probed whether this MVN-rHRV relationship was diminished in depression. Eleven healthy adults and nine depressed subjects performed the emotional counting stroop task in alternating blocks of emotion and neutral words during functional magnetic resonance imaging (fMRI). The correlation between rHRV outside the scanner and BOLD signal reactivity (absolute value of change between adjacent blocks in the BOLD signal) was examined in specific MVN regions. Significant negative correlations were observed between rHRV and average BOLD shift magnitude (BSM) in several MVN regions in healthy subjects but not depressed subjects. This preliminary report provides novel evidence relating emotional reactivity in MVN regions to rHRV. It also provides preliminary suggestive evidence that depression may involve reduced interaction between the MVN and cardiac vagal control.

This study aimed to describe habitual sleep and nocturnal cardiac autonomic activity (CAA), and their relationship with training/match load in male youth soccer players during an international tournament. Eighteen elite male youth soccer players (aged 14.8 ± 0.3 years; mean ± SD) participated in the study. Sleep indices were measured using wrist actigraphy, and heart rate (HR) monitors were used to measure CAA during night-sleep throughout 5 consecutive days. Training and match loads were characterized using the session-rating of perceived exertion (s-RPE). During the five nights 8 to 17 players slept less than <8 h and only one to two players had a sleep efficiency <75%. Players’ sleep duration coefficient of variation (CV) ranged between 4 and 17%. Nocturnal heart rate variability (HRV) indices for the time-domain analyses ranged from 3.8 (95% confidence interval, 3.6; 4.0) to 4.1 ln[ms] (3.9; 4.3) and for the frequency-domain analyses ranged from 5.9 (5.6; 6.5) to 6.6 (6.3; 7.4). Time-domain HRV CV ranged from 3 to 10% and frequency-domain HRV ranged from 2 to 12%. A moderate within-subjects correlation was found between s-RPE and sleep duration [r = −0.41 (−0.62; −0.14); p = 0.003]. The present findings suggest that youth soccer players slept less than the recommended during the international tournament, and sleep duration was negatively associated with training/match load.
