Molecular identification of the first local dengue fever outbreak in Shenzhen city, China: a potential imported vertical transmission from Southeast Asia?

SUMMARYA suspected dengue fever outbreak occurred in 2010 at a solitary construction site in Shenzhen city, China. To investigate this epidemic, we used serological, molecular biological, and bioinformatics techniques. Of nine serum samples from suspected patients, we detected seven positive for dengue virus (DENV) antibodies, eight for DENV-1 RNA, and three containing live viruses. The isolated virus, SZ1029 strain, was sequenced and confirmed as DENV-1, showing the highest E-gene homology to D1/Malaysia/36000/05 and SG(EHI)DED142808 strains recently reported in Southeast Asia. Further phylogenetic tree analysis confirmed their close relationship. At the epidemic site, we also detected 14 asymptomatic co-workers (out of 291) positive for DENV antibody, and DENV-1-positive mosquitoes. Thus, we concluded that DENV-1 caused the first local dengue fever outbreak in Shenzhen. Because no imported case was identified, the molecular fingerprints of the SZ1029 strain suggest this outbreak may be due to vertical transmission imported from Southeast Asia.

The aim of the study was in the differential specific diagnosis of viral infections of unknown etiology in patients who have recently returned from a trip to the countries of Southeast Asia and India. In 2013, SRC VB "Vector" examined single serum samples from 131 patients with suspected Dengue Fever. Dengue viral RNA was detected by means of polymerase chain reaction, NS dengue virus antigen and antibody classes M and G to Dengue virus - with the use of immunochromatographic methods and enzyme immunoassay. In the same sera the presence of specific IgM to measles, rubella and cytomegalovirus virus antigen was revealed by enzyme immunoassay and enzyme-enhanced chemiluminescence. As a result of examination 87 (66.4%) ofsamples were diagnosed as "Dengue fever". In seven out of the 87 samples there were detected specific class-M antibodies to cytomegalovirus. 44 sera (33.6%) out of 131 did not contain markers for Dengue fever. But 4 out of them contained IgM to measles virus, the other 5 sera contained IgM to rubella virus. Regional Centre of WHO laboratory confirmed three cases of measles, the one positive result for the content of IgM to rubella virus antigens results was interpreted as "equivalent." Identified in 2013 cases of importation ofmeasles and rubella by tourists from Southeast Asia and India have not resulted in the transmission of the disease through contact with others persons in Russia. In seven detected Dengue Fever cases opportunistic cytomegalovirus infection was revealed.

105 sera from 101 patients with a confirmed diagnosis of dengue fever (LD) were examined using monospecific ELISA-IgM test. Monospecific results were observed in 39 samples (37.1%). 27 sample of them were taken during the first 7 days of the disease. In those systems were examined 23 serum samples of 23 patients, in which etiological role of one of the four dengue viruses was been established by the RT-PCR method. In this case, the coincidence-IgM ELISA results and the RT-PCR took place in 14 sera (60.9%).

In this work, a two-strain dengue model with vertical transmission in the mosquito population is considered. Although vertical transmission is often ignored in models of dengue fever, we show that effective control of an outbreak of dengue can depend on whether or not the vertical transmission is a significant mode of disease transmission. We model the effect of a control strategy aimed at reducing human-mosquito transmissions in an optimal control framework. As the likelihood of vertical transmission increases, outbreaks become more difficult and expensive to control. However, even for low levels of vertical transmission, the additional, uncontrolled, transmission from infected mosquito to eggs may undercut the effectiveness of any control function. This is of particular importance in regions where existing control policies may be effective and the endemic strain does not exhibit vertical transmission. If a novel strain that does exhibit vertical transmission invades, then existing, formerly effective, control policies may no longer be sufficient. Therefore, public health officials should pay more attention to the role of vertical transmission for more effective interventions and policy.

SUMMARYChikungunya virus (CHIKV) and Ross River virus (RRV) of the genusAlphavirus, family Togaviridae are mainly transmitted byAedesmosquitoes and the symptoms they cause in patients are similar to dengue. A chikungunya (CHIK) outbreak re-emerged in several Asian countries during 2005–2006. This study aimed to clarify the prevalence of CHIKV infection in suspected dengue patients in six countries in South Asia and Southeast Asia. Seven hundred forty-eight serum samples were from dengue-suspected patients in South Asia and Southeast Asia, and 52 were from patients in Fiji. The samples were analysed by CHIKV IgM capture ELISA, CHIKV IgG indirect ELISA and focus reduction neutralization test against CHIKV or RRV. CHIK-confirmed cases in South Asia, particularly Myanmar and Sri Lanka, were 4·6%, and 6·1%, respectively; and in Southeast Asia, particularly Indonesia, the Philippines and Vietnam, were 27·4%, 26·8% and 25·0%, respectively. It suggests that CHIK was widely spread in these five countries in Asia. In Fiji, no CHIK cases were confirmed; however, RRV-confirmed cases represented 53·6% of suspected dengue cases. It suggests that RRV is being maintained or occasionally entering from neighbouring countries and should be considered when determining a causative agent for dengue-like illness in Fiji.

The melanocortin 1 receptor (MC1R) plays an important role in determining plumage color, and the variants of MC1R have been found to be associated with the color of plumage and skin in both domestic and wild birds. However, the molecular and genetic mechanism for plumage color variation has not been reported in silky fowl, which is a unique subspecies in China with high nutritive value. We sequenced and analyzed the encoding region of MC1R from silky fowl. The predicted coding region of MC1R is 945 bp, which is the same size as the one in Gallus gallus. Six nucleotide polymorphisms that lead to four protein mutations were detected, which were M71T, E92K, S124G and H215P, respectively. Among the four mutations, the S124G mutation is found to be unique to silky fowl. A phylogenetic tree analysis of MC1R from silky fowl and other species of chicken shows a close relationship between silky fowl and Gallus gallus. Furthermore, the eukaryotic expression vector pEGFP-N1-MC1R was constructed, and transfected into goat fibroblasts by means of electroporation. The success of MC1R gene expression in transfected goat fibroblasts makes it possible to develop transgenic animals for further studies.

Abstract A unique gain-of-function mutation of the Janus kinase 2 (JAK2) gene has been recently described in patients with polycythemia vera (PV), essential thrombocythemia and chronic idiopathic myelofibrosis [N Engl J Med. 2005 Apr 28;352(17):1779–90]. Although the currently available data clearly demonstrate that the JAK2 V617F mutation participates in the pathogenesis of myeloproliferative disorders, the mutation’s precise place in the hierarchical order of pathogenetic events remains to be established. We have recently reported that altered gene expression in myeloproliferative disorders correlates with activation of signaling by the V617F mutation of JAK2 (Blood. 2005 Aug 4; Epub ahead of print). Granulocyte CD177 (PRV1) mRNA overexpression has been initially reported as a potential marker of PV but later shown by us to rather be a marker of neutrophil activation [Br J Haematol. 2004 Sep;126(5):650–6]. In this study, we analyzed the relationship between JAK2 V617F mutation status, granulocyte CD177 mRNA expression and CD177 soluble protein level in 72 patients with PV. We also investigated the ontogeny of CD177 expression by hematopoietic cells with the aim of defining the stage of mRNA expression during myeloid, erythroid and megakaryocytic cell differentiation. Finally we studied the effect of soluble CD177 protein on hematopoietic cell proliferation and differentiation. Granulocyte CD177 mRNA expression and percentage of JAK2 V617F alleles were evaluated by quantitative Real Time PCR (qRT-PCR), while serum CD177 protein level was measured by a flow cytometry-based competitive antibody-binding assay. Liquid cultures were performed by culturing peripheral blood mononuclear cells obtained from healthy individuals and PV patients in the presence of high CD177-expressing, low CD177-expressing or CD177-depleted sera. After 12 days of culture, cells were collected, counted and evaluated for colony growth, and for flow cytometry analysis of myeloid, erythroid, megakaryocytic and CD34-positive cell subpopulations. qRT-PCR studies showed a close relationship between CD177 mRNA level and percentage of JAK2 V617F alleles (r=0.412, P<0.001). CD177 mRNA expression was almost undetectable in cell populations other than granulocytes. Studies of CFU-GM growth and differentiation indicated that CD177 mRNA expression is a late event restricted to the neutrophil stage of differentiation. Analysis of serum samples showed variable values for mean fluorescence intensity (MFI), indicating variable levels of the soluble CD177 protein in the patients studied. A very close relationship was found between granulocyte CD177 mRNA expression and soluble CD177 protein level (r=0.56, P=0.02). Incubation of mononuclear cells with serum samples showing high levels of soluble CD177 protein resulted in increased numbers of CD34-positive cells (P<0.02) and of erythroid progenitors (P<0.03). This effect was not detectable when low CD177-expressing or CD177-depleted sera were employed. These observations clearly indicate that the JAK2 V617F mutation is associated with enhanced granulocyte CD177 mRNA expression, and that this latter results in high levels of soluble CD177 protein. These elevated levels might contribute to the increased red cell production that characterizes polycythemia vera.

SUMMARYWe determined the genetic relationships and origin of the dengue virus (DENV) responsible for an outbreak of dengue fever (DF) in Guangdong province, China, in 2006. Five DENV type 1 (DENV-1) isolates were obtained from human serum samples collected from DF patients during the outbreak. The nucleotide sequences of the E (envelope) gene were compared with those of 48 previous DENV-1 isolates: 18 from Guangdong province, one from Fujian province, one from Zhejiang province, and 28 from other countries in the South Asian region. The results suggested that four DENV-1 isolates identified in Guangdong province in 2006 might be in general circulation there, although these DENV-1 viruses may have been originally introduced into the province from other countries. In contrast, one isolate from Guangzhou city in 2006, may have been introduced by a recently imported case from Cambodia.
