This article addresses the question of to what extent conventional theories of high reliability organizations and normal accidents theory are applicable to public bureaucracy. Empirical evidence suggests precisely this. Relevant cases are, for instance, collapsing buildings and bridges due to insufficient supervision of engineering by the relevant authorities, infants dying at the hands of their own parents due to misperceptions and neglect on the part of child protection agencies, uninterrupted serial killings due to a lack of coordination among police services, or improper planning and risk assessment in the preparation of mass events such as soccer games or street parades. The basic argument is that conceptualizing distinct and differentiated causal mechanisms is useful for developing more fine-grained variants of both normal accident theory and high reliability organization theory that take into account standard pathologies of public bureaucracies and inevitable trade-offs connected to their political embeddedness in democratic and rule-of-law-based systems to which belong the tensions between responsiveness and responsibility and between goal attainment and system maintenance. This, the article argues, makes it possible to identify distinct points of intervention at which permissive conditions with the potential to trigger risk-generating human action can be neutralized while the threshold that separates risk-generating human action from actual disaster can be raised to a level that makes disastrous outcomes less probable.

AbstractIn the present study we report a case of thyroglobulin (TGB) measurement interference in a total thyroidectomized and radio-ablated 61-year old woman with papillary thyroid cancer. We investigated possible interference in the measurement of TGB due to discordant TGB in relation to clinical condition during the follow-up period. Serum TGB was measured with the chemiluminescence method using Beckman Coulter Unicel DxI 800 instrument. To investigate possible interference in TGB measurement serial dilutions, polyethylene glycol precipitation (PEG), treatment with heterophile blocking tube (HBT), rheumatoid factor level determination and retesting of TGB with an alternative method were performed. Serial dilutions of the serum sample revealed linearity but a remarkable decrease in TGB in the patient’s serum samples post PEG and post HBT treatments. Also, TGB results under functional sensitivity level obtained with a different method suggested that TGB interference developed due to heterophile antibody presence in the serum sample. The patient had unnecessarily undergone expensive imaging techniques, and invasive procedures such as lymph node fine needle aspiration biopsy, before the analytical interference was suspected by the clinician. This report illustrates the importance of early communication and close collaboration between clinicians and laboratorians in order to avoid unnecessary clinical intervention.

The number of buildings renovated following the introduction of European energy-efficiency policy represents a small number of buildings in Spain. So, the main Spanish building stock needs an urgent energy renovation. Using passive strategies is essential, and thermal characterization and predictive tests of the energy-efficiency improvements achieving acceptable levels of comfort for their users are urgently necessary. This study analyzes the energy performance and thermal comfort of the users in a typical Mediterranean dwelling house. A transient simulation has been used to acquire the scope of Spanish standards for its energy rehabilitation, taking into account standard comfort conditions. The work is based on thermal monitoring of the building and a numerical validated model developed in TRNSYS. Energy demands for different models have been calculated considering different passive constructive measures combined with real wind site conditions and the behavior of users related to natural ventilation. This methodology has given us the necessary information to decide the best solution in relation to energy demand and facility of implementation. The thermal comfort for different models is not directly related to energy demand and has allowed checking when and where the measures need to be done.

Coastal communities face heightened risk to coastal flooding and erosion hazards due to sea-level rise, changing storminess patterns, and evolving human development pressures. Incorporating uncertainty associated with both climate change and the range of possible adaptation measures is essential for projecting the evolving exposure to coastal flooding and erosion, as well as associated community vulnerability through time. A spatially explicit agent-based modeling platform, that provides a scenario-based framework for examining interactions between human and natural systems across a landscape, was used in Tillamook County, OR (USA) to explore strategies that may reduce exposure to coastal hazards within the context of climate change. Probabilistic simulations of extreme water levels were used to assess the impacts of variable projections of sea-level rise and storminess both as individual climate drivers and under a range of integrated climate change scenarios through the end of the century. Additionally, policy drivers, modeled both as individual management decisions and as policies integrated within adaptation scenarios, captured variability in possible human response to increased hazards risk. The relative contribution of variability and uncertainty from both climate change and policy decisions was quantified using three stakeholder relevant landscape performance metrics related to flooding, erosion, and recreational beach accessibility. In general, policy decisions introduced greater variability and uncertainty to the impacts of coastal hazards than climate change uncertainty. Quantifying uncertainty across a suite of coproduced performance metrics can help determine the relative impact of management decisions on the adaptive capacity of communities under future climate scenarios.

Abstract An ultrasound method for simultaneous extraction of Cr, Cu, Zn, Cd, and Pb from sediment, and determination by flame atomic absorption spectrometry (FAAS) and graphite furnace atomic absorption spectrometry (GFAAS) was proposed. The experimental results obtained using analytical curves and the method of standard additions agreed at a confidence level of 95% for all the analytes, as determined by FAAS and GFAAS, indicating no significant matrix effects. Recoveries ranged from 80.1 to 93.7% (certified reference material) and from 89 to 107% (spike tests). The LOD and LOQ results from the method were consistent with the techniques used (FAAS and GFAAS), with high analytical throughput. The proposed method was then used to determine Cr, Cu, Zn, Cd, and Pb in river sediment samples from Rio Doce, Minas Gerais, Brazil. The results indicated levels below those permitted by Brazilian legislation for all the analytes, with the exception of Cr.
