Expression and coordinated regulation of matrix metalloproteinases in chronic hepatitis C and hepatitis C virus-induced liver cirrhosis

Matrix metalloproteinases (MMPs) are central to tissue remodelling; however, little is known about the temporal pattern and differential regulation of hepatic MMP expression in the course of chronic human liver disease. Using quantitative reverse transcription–PCR ELISA assays, we studied hepatic mRNA expression of MMP-1, -2, -3, -7, -9, -10, -11, -13 and -14 in patients with chronic hepatitis C and hepatitis C virus-induced end-stage liver cirrhosis and controls. Results were compared with histology, hepatic expression of tissue inhibitor of metalloproteinases (TIMP)-1, -2 and -3, procollagen types I and IV, laminin, and with circulating protein levels of hyaluronate, TIMP-1 and -2 and MMP proenzymes, as measured by ELISA. The impact of the MMP-3(-1171) promoter polymorphism on hepatic MMP-3 expression was analysed. Hepatic mRNA expression data identified differentially regulated groups of MMPs during the course of chronic hepatitis C, showing either steadily increasing mRNA expression with disease progression (MMP-1, -2, -7 and -14) or transiently elevated expression (MMP-9, -11 and -13). The first group closely correlated to the parameters of fibrogenesis. Hepatic MMP-3 expression was unrelated to disease stage, but was determined by the MMP-3(-1171) promoter polymorphism. In conclusion, MMP expression during the course of chronic hepatitis C appears to be a closely regulated process, with different clusters of coordinately regulated MMP genes being identified.

Introduction. The incidence of chronic hepatitis C and its consequences, liver cirrhosis and hepatocellular carcinoma is growing rapidly. The aim of this study was to evaluate clinical characteristics of patients with hepatocellular carcinoma and chronic hepatitis C treated at the Clinical Centre of Vojvodina and therapy options. Material and Methods. This retrospective study included 51 patients (52.9% male and 47.1% female) with chronic hepatitis C and hepatocellular carcinoma treated between 2000 and 2014. The average age of patients was 61.6 years (SD=10.8) and the average duration of hepatitis C virus infection was 30.2 years (SD=11.7). All patients had liver cirrhosis and 43.1% had previously been treated with pegylated interferon and ribavirin. According to the Barcelona Clinic Liver Cancer criteria, stadium A, B, C and D were found in 15.7%, 52.3%, 19.6% and 11.8% of the patients, respectively. The average value of alpha fetoprotein at the moment of making diagnosis of hepatocellular carcinoma was 397.56 ng/ml and the level of alpha fetoprotein was below 20mg/ml in 26% of patients. Tumor resection, radiofrequency ablation, chemoembolization, systemic chemotherapy and liver transplantation were performed in 13.7%, 3.9%, 1.9%, 1.9% and 5.9% of patients, respectively. Average survival time after the diagnosis of hepatocellular carcinoma among patients included in the study was 1.39 (SD=1.61) years. Conclusion. Ultrasound examinations of the patients with liver cirrhosis caused by hepatitis C virus infection are obligatory every 3 months. Etiology of every focal lesion in the liver must be clarified, which could increase the possibility of administration of available therapeutic methods.

By 01.01.2013 in the Republic of Tatarstan there were 23 970 Hepatitis B Surface Antigen carriers and 4546 patients with active Hepatitis B out of 28 516 registered patients who were infected with Hepatitis B. Our experience shows that not only inactive chronic Hepatitis B Surface Antigen carriers, but also patients with different clinical forms of Hepatitis B might be estimated as Hepatitis B Surface Antigen carriers. The feature of chronic viral hepatitides is a discrepancy between the scarce clinical picture and presence (in 15-30% of cases) of active inflammation in liver tissue. Since 2009, patients with positive ELISA test for antibodies to Hepatitis C virus are no longer registered in the national registry of infective diseases of Russian Federation. In the Republic of Tatarstan, such patients with antibodies to Hepatitis C virus were still registered up to 2012 (50 266 patients were registered by 01.01.2012). According to our own data, Hepatitis C viral RNA was found at polymerase chain reaction to Hepatitis C virus in 61% of cases out of 957 patients tested. According to our own data, when using the combined treatment (interferon alpha and ribavirin), lasting virologic response was obtained in 65.7% cases of type 1 genotype of Hepatitis C and almost in 100% of cases of other genotypes out of 163 cases of chronic Hepatitis C, which corresponds to worldwide standards. By 01.01.2013, 295 patients with liver cirrhosis as the result of chronic Hepatitis C were followed up at the consultative and diagnostic center of Republican Clinical Hospital of Infective Diseases with the upward trend. The retrospective analysis of 130 case histories of patients with liver cirrhosis revealed that Hepatitis C was firstly diagnosed at the stage of established liver cirrhosis in 57.7% of cases. Timely diagnosis and periodic medical examination in cases of chronic Hepatitis C allows to implement the individual, personalized approach in such patients and to choose the adequate treatment and follow-up.
