ZusammenfassungDie ersten Impfversuche gegen das Hepatitis-B-Virus (HBV) erfolgten 1970, noch bevor die Natur des dafür verwendeten „Australia-Antigens“ bekannt war. Bald darauf wurde dieses Antigen als Hüllprotein des HBV erkannt (HBV Surface Antigen, HBsAg), dann aus HBV-haltigem Plasma gereinigt und später gentechnisch in Hefezellen hergestellt. Die hohe Wirksamkeit des HBsAg-Impfstoffs wurde vielfach bewiesen, insbesondere bei Neugeborenen von HBV-infizierten Müttern, die sonst fast immer chronische HBV-Träger werden. Auch bei älteren Kindern und Erwachsenen schützt die Impfung und wird seit 1984 weltweit angewendet, was zu einer ungefähr 10-fachen Abnahme der HBV-Infektionen bei den Geimpften geführt hat.Es gibt dennoch verschiedene Herausforderungen bei der Hepatitis-B-Impfung. Bei Neugeborenen von hochvirämischen Müttern kann die Impfung versagen. Bei verringerter Immunkompetenz kann die Bildung schützender Antikörper ausbleiben, aber auch bei Risikofaktoren wie höherem Alter, Rauchen oder Übergewicht. Frühe Impfstudien belegten, dass Impfstoffe mit dem HBsAg-Subtyp adw2 auch gegen HBV mit anderen HBsAg-Subtypen schützen, neuere Beobachtungen zeigen aber, dass die Schutzwirkung gegen heterologe Subtypen schwächer ist. Gelegentlich werden auch Escape-Mutationen beobachtet.Die meisten jetzigen Impfstoffe beruhen auf dem Kenntnisstand vor 40 Jahren und könnten wesentlich verbessert werden. Eine Einbeziehung der bislang fehlenden PräS-Domänen der HBV-Hülle in die Impfstoffe würde die wichtigsten schützenden T‑ und B‑Zellepitope einbringen. Die Expression in Säugerzellkulturen verbessert die native Faltung der neutralisierenden HBsAg-Epitope und die Verwendung von regional vorherrschenden HBsAg-Subtypen würde die Schutzwirkung erhöhen. Optimale Adjuvanzien oder Epitopträger könnten die Immunogenität auch für eine HBV-Immuntherapie steigern.

Objectives. The organisms of Toxoplasma gondii, Rubella virus, Cytomegalovirus, and Herpes simplex virus as an acronym of TORCH are major pathogens in prepregnancy and reproductive-age women. These microorganisms are considered a serious problem and cause 2-3% of all birth defects in the fetus. Our study was aimed at screening the seroprevalence of TORCH antibodies among prepregnancy and reproductive-age women in Tabriz, Iran. Design and Setting. This study was carried out in 2726 prepregnancy and reproductive-age women, who were referred to the laboratory for prenatal TORCH screening. To detect the presence of IgG, IgM antibodies and Hepatitis B surface antigen against these microorganisms were carried out using a chemiluminescence immunoassay analyzer (CLIA). Results. In the current study, the rates of anti-Toxoplasma gondii IgG, anti-Rubella virus IgG, and anti-Cytomegalovirus IgG were found in 722 cases (26.5%), 2579 cases (94.6%0), and 2718 cases (99.7%), respectively. Moreover, the rates of anti-Toxoplasma gondii IgM, anti-Rubella virus IgM, and anti-Cytomegalovirus IgM were discovered in 10 cases (0.4%), 13 cases (0.5%), and 16 cases (0.6%), respectively. The Hepatitis B surface antigen was found in 32 cases (1.2%). The dissemination of positive TORCH in various ages was different ( P < 0.05 ). Conclusions. In our study, the seroprevalence of acute TORCH infections was relatively low. Due to the probability of vertical transmission to the fetus during pregnancy and the unpleasant complication of these pathogens, it is essential to be screened for detection of specific IgG and IgM antibodies in reproductive ages.

Background: Hepatitis B Viral (HBV) infection is a potentially fatal infection of public health importance globally. Pregnant women and their unborn children are at risk of this infection due to its mode of transmission especially when in contact with infected body fluids and from mother to child transmission respectively. The prevalence of HBV infection in Nigeria is 13.6%. Aim: To determine the prevalence of HBV infection amongst pregnant women in rural community in southern Nigeria. Methods: This was a retrospective cross sectional study of pregnant women at Ogoni Primary Health Centers and General Hospitals in Southern Nigeria carried over a 4-month period. Serological test results for Hepatitis B surface antigen (HBsAg) of subjects were retrieved from the laboratories. Permission was gotten from Director of Public Health Rivers State Ministry of Health. The information gotten was analyzed using independent t test, chi square and fisher exact tests. Significance level was set at 0.05. Result: One hundred and eighty-five pregnant women were recruited for the study. The mean age of the antenatal clinic attendees was 28 ± 2 years, the mean gestational age at booking was 25 weeks and the mean parity was 2. Majority had secondary level of education represented by 122 (66%). Fifteen (8.1%) of the pregnant women were positive for Hepatitis B surface antigen. Conclusion: The prevalence of HBV infection was 8.1% in Ogoni southern Nigeria. This is worisome, hence preventive measures including HBV vaccines should be made available to the populace.

Abstract Serum hepatitis B surface antigen (HBsAg) is a component of hepatitis B virus (HBV) virions and non-infectious subviral particles (SVPs). O-glycosylation of the PreS2 domain of middle HBsAg protein is a distinct characteristic of genotype C HBV virions versus SVPs. We measured serum O-glycosylated HBsAg levels in 47 treatment-naïve patients with genotype C chronic hepatitis B (CHB) at baseline and after 48 weeks of nucleos(t)ide analog (NA) therapy by immunoassay using a monoclonal antibody against the O-glycosylated PreS2 domain of middle HBsAg, and analyzed their correlations with conventional HBV marker levels. At baseline, serum O-glycosylated HBsAg levels were significantly correlated with HBV DNA, HBsAg, and hepatitis B-core related antigen (HBcrAg) levels. HBV DNA and O-glycosylated HBsAg levels were decreased after 48 weeks of NA therapy. The correlation between O-glycosylated HBsAg and HBsAg or HBcrAg levels was lost in patients who achieved undetectable HBV DNA. HBV DNA and RNA were detected in the O-glycosylated HBsAg-binding serum fraction, and the proportion of HBV RNA increased during NA therapy. In conclusion, serum O-glycosylated HBsAg levels change during NA therapy and may reflect combined serum HBV DNA and RNA virion levels, and an O-glycosylated HBsAg-based immunoassay may allow monitoring viral kinetics during NA therapy.

Zusammenfassung Hintergrund und Ziel Trotz niedriger Prävalenz der Hepatitis-B-Virus-(HBV-)Infektion in Deutschland ist es wichtig, vulnerable Gruppen und Ansatzpunkte für die Prävention zu identifizieren. In ersten Analysen der „Studie zur Gesundheit Erwachsener in Deutschland“ (DEGS1, 2008–2011) waren HBV-Infektion und -Impfung mit sozidemografischen Determinanten assoziiert. In dieser Arbeit werden die Ergebnisse im Detail untersucht. Material und Methoden In DEGS1 lag für 7046 Teilnehmende (Alter: 18–79 Jahre) eine HBV-Serologie vor. Die stattgehabte HBV-Infektion war durch Antikörper gegen das Hepatitis-B-Core-Antigen (Anti-HBc) definiert, die impfinduzierte Immunität durch alleinigen Nachweis von Antikörpern gegen das Hepatitis-B-Surface-Antigen (Anti-HBs). Seroprävalenzen von HBV-Infektions- und -Impfstatus wurden geschlechtsstratifiziert geschätzt und Assoziationen mit Alter, Gemeindegröße, Einkommen, formaler Bildung, Krankenversicherung und Migrationsgeneration in logistischen Regressionen analysiert. Ergebnisse Die HBV-Infektion war bei Männern und Frauen unabhängig mit den Altersgruppen 34–64 und ≥ 65 Jahre, erster Migrationsgeneration und Leben in größeren Gemeinden assoziiert, zudem bei Männern mit niedrigem Einkommen und bei Frauen mit niedriger Bildung. Die impfinduzierte Immunität war bei Männern und Frauen unabhängig mit den Altersgruppen 18–33 und 34–64 Jahre, mittlerer und hoher Bildung und hohem Einkommen assoziiert, darüber hinaus bei Männern mit mittlerem Einkommen und privater Krankenversicherung und bei Frauen mit fehlendem Migrationshintergrund. Diskussion Die Berücksichtigung von Migrationsstatus, Einkommen und Bildung könnte zur zielgenauen Ausrichtung der HBV-Prävention beitragen.

Patients with occult hepatitis B infection (OBI) have undetectable hepatitis B surface antigen (HBsAg) by conventional assays but detectable hepatitis B virus (HBV) DNA in blood/liver. We evaluated the key performance characteristics of a sensitive HBsAg assay (ARCHITECT HBsAg Next Qualitative Assay, referred as NEXT) with respect to HBsAg detection. Assay precision, sample carryover and seroconversion sensitivity of NEXT were evaluated. HBsAg was measured by NEXT in 1,138 individuals, including 1,038 patients who attended liver clinics in a tertiary hospital (200 HBV DNA-positive blood donors whose HBsAg was undetectable by conventional assays, and 38 patients receiving immunosuppressive therapy, 800 chronic hepatitis B patients with HBsAg seroclearance) and 100 HBsAg-negative subjects recruited from a community project. The within-run and within-laboratory coefficients of variation were <6% for the positive sample pools. In 9 seroconversion panels tested, NEXT allowed an earlier HBsAg detection than conventional assays. NEXT detected HBsAg in 10/200 (5%) HBsAg-negative blood donors, 1/20 (5%) and 0/18 HBsAg-negative patients with and without HBV reactivation respectively, and 59/800 (7.3%) patients with HBsAg seroclearance. HBsAg was detectable by NEXT in 27.8%, 8.2%, 6.9%, 3.8% and 1.9% samples at <3, 3–5, >5–8, >8–11, and >11 years after HBsAg seroclearance, respectively. Seven out of 100 HBsAg-negative community identified subjects was tested positive by NEXT. Comparing with conventional HBsAg assays, NEXT demonstrated a higher sensitivity and conferred an increment of 5–7% detection rate in patients with OBI, thereby helping in identifying HBV carriers and prevention of OBI-associated HBV transmission and reactivation.
