Understanding the factors generating patterns of genetic diversity is critical to implementing robust conservation and management strategies for exploited marine species. Yet, often too little is known about population structure to properly tailor management schemes. Here we report evidence of substantial population structure in white hake ( Urophycis tenuis ) in the Northwest Atlantic, perhaps among the highest levels of population structure exhibited by a highly exploited, widely dispersed, long-lived marine fish. We show that depth plays a role in this extensive and temporally stable structure, which does not conform to previously established fisheries management units. Three genetically distinguishable populations were identified, where all straddle several management divisions and two (Southern Gulf of St. Lawrence and Scotian Shelf) overlap in their range, coexisting within a single division. The most highly exploited population in the Southern Gulf of St. Lawrence was also the most isolated and likely the smallest (genetically effective). This work shows that conservation and management priorities must include population structure and stability in establishing effective species recovery strategies.

Hydrocele can be found as a collection of fluid within the testicular tunica vaginalis. According to the etiology and pathophysiology of the disease, it can be classified into primary and secondary. Furthermore, primary hydrocele might include the closed or non-communicating, the communicating type, the congenital and or neonatal type. Many management approaches have been proposed for both the communicating and non-communicating hydrocele with different post-operative and prognostic outcomes. In this literature review, we have discussed the current management approaches and prognosis of communicating and non-community hydrocele. Adequate diagnosis of the condition is the first step to achieve favorable management outcomes. Although the reported management outcomes are reported to be effective in the literature, the surgical approaches seem to be superior. However, many side effects might be associated with these operations. Estimates show that following varicocelectomy procedures, ipselateral (left) hemi-scrotal varicocele is the most common condition to occur, which might even develop following the procedure by several months and years (in some cases). Further investigations are still needed because the current evidence is largely based on case reports and small case series investigations. Therefore, larger studies are needed to help draw effective management protocols and enhance the outcomes and prognosis.

Pleasurable sexual activity is an essential component of many human relationships, providing a sense of physical, psychological, and social well-being. Epidemiological and clinical studies show that depressive symptoms and depressive illness are associated with impairments in sexual function and satisfaction, both in untreated and treated patients. The findings of randomized placebo-controlled trials demonstrate that most of the currently available antidepressant drugs are associated with the development or worsening of sexual dysfunction, in a substantial proportion of patients. Sexual difficulties during antidepressant treatment often resolve as depression lifts but can endure over long periods and may reduce self-esteem and affect mood and relationships adversely. Sexual dysfunction during antidepressant treatment is typically associated with many possible causes, but the risk and type of dysfunction vary with differing compounds and should be considered when making decisions about the relative merits and drawbacks of differing antidepressants. A range of interventions can be considered when managing patients with sexual dysfunction associated with antidepressants, including the prescription of phosphodiesterase-5 inhibitors, but none of these approaches can be considered “ideal.” As treatment-emergent sexual dysfunction is less frequent with certain drugs, presumably related to differences in their pharmacological properties, and because current management approaches are less than ideal, a reduced burden of treatment-emergent sexual dysfunction represents a tolerability target in the development of novel antidepressants.

PurposeThis study aims to redefine the urban heritage value of trade streets in Hanoi's Ancient Quarter (AQ) and propose an expanded notion of the “historicity” found through intangible cultural heritage (ICH).Design/methodology/approachA longitudinal analysis was conducted to examine the historical transitions of trade streets, thereby providing an alternative value perspective for considering the area's conservation management. The trade streets were specifically analysed from the pre-colonial era to 2017 using past survey data concerning the distribution and concentration of trade types, statistical documents from the colonial era and recent fieldwork data from investigations into 79 trade streets, thus identifying seven patterns of change.FindingsIndividual trade streets contribute to the unique identities of their respective streets while collectively providing experiential value through the overall variety and density of trade types. Further, the value of modernised trade streets can be found in their support of the area's systemic and experiential values.Practical implicationsCurrent management approaches should shift to include non-traditional trade streets that have experienced gradual changes or retained specific businesses for long periods of time.Originality/valueThis was the first study to conduct a longitudinal analysis of AQ trading with partial support from statistical data. It explored an expanded way of interpreting historicity from the viewpoint of dynamic ICH along the two axes of pace and intensity by tracing changes in commercial activities over time.

Electronic health record (EHR) management systems require the adoption of effective technologies when health information is being exchanged. Current management approaches often face risks that may expose medical record storage solutions to common security attack vectors. However, healthcare-oriented blockchain solutions can provide a decentralized, anonymous and secure EHR handling approach. This paper presents PREHEALTH, a privacy-preserving EHR management solution that uses distributed ledger technology and an Identity Mixer (Idemix). The paper describes a proof-of-concept implementation that uses the Hyperledger Fabric’s permissioned blockchain framework. The proposed solution is able to store patient records effectively whilst providing anonymity and unlinkability. Experimental performance evaluation results demonstrate the scheme’s efficiency and feasibility for real-world scale deployment.
