Serum CA 19–9 Level is Correlated to the Clinical Characteristics and Chronic Complications of Patients Newly Diagnosed with Type 2 Diabetes Mellitus

Abstract Aim This study investigated the relation of serum carbohydrate antigen 199 (CA 19–9) levels to the clinical characteristics and chronic complications of patients newly diagnosed with type 2 diabetes mellitus (T2DM). Methods A total of 371 patients newly diagnosed with T2DM and 133 healthy people with consecutively matched age were compared. The 371 patients with T2DM were divided into four groups by quartiles based on their serum CA 19–9 levels, in which clinical characteristics and chronic complications, such as diabetic retinopathy (DR), diabetic nephropathy, and macrovascular complications were compared. Logistic regression analysis was used to evaluate the risk factors of DR. Results Among the 371 patients newly diagnosed with T2DM, 60 had elevated CA 19–9 levels (16.17%). The frequencies of elevated serum CA 19–9 were 24.39% (30 of 123) for females and 12.10% (30 of 248) for males, in which the values for females were higher than those for males (P<0.01).Differences were observed among the serum CA 19–9 levels, hemoglobin A1c (HbA1c), and DR (P<0.05). Logistic regression analysis showed that serum CA 19–9 levels, fasting blood glucose (FBG) and fasting C-peptide (FC-P) were risk factors for DR (P<0.05). Conclusions Serum CA 19–9 levels were correlated with HbA1c and DR in patients newly diagnosed with T2DM. The elevated serum CA 19–9 levels, high FC-P, and FBG levels were important risk factors for DR in patients newly diagnosed with T2DM.

Abstract Purpose To analyze the clinical features and related risk factors in diabetic retinopathy (DR) and subclinical atherosclerosis, the micro- and macro-vascular diseases in newly diagnosed type 2 diabetes mellitus (T2DM). Methods A retrospective study of 435 cases of inpatients with newly diagnosed T2DM from 2013–2017, and compare the 2 types of T2DM related vascular complications. Results The macro- and microvascular complications are not rare at this stage. Subclinical atherosclerosis was found in 251 subjects (57.7%), which was higher than that of DR (13.1%). In addition, some cases of subclinical atherosclerosis co-existed with DR, suggesting that DR was related with subclinical atherosclerosis (r=0.098, P=0.041). Older age showed a significant association with both subclinical atherosclerosis and DR. Single factor analysis indicated that dyslipidemia was the common risk factor in DR and subclinical atherosclerosis. Conclusions It should be paid attention to the screening of both DR and subclinical atherosclerosis in each age group of newly diagnosed T2DM. Except for the control of blood glucose, the control of the dyslipidemia is important in the prevention and treatment of the micro- and macro-vascular diseases.

Dyslipidemia is considered a significant modifiable risk factor for type-2 diabetes mellitus (T2DM) and has become one of the emerging health problems throughout the world. In Bangladesh, data on dyslipidemia among newly diagnosed T2DM patients are comparatively inadequate. This study aimed to evaluate the prevalence of dyslipidemia and its associated risk factors in newly diagnosed T2DM patients. This cross-sectional study was conducted by a well-structured questionnaire from 132 newly diagnosed type-2 diabetic patients attending the Mujibur Rahman Memorial Diabetic Hospital in Kushtia, Bangladesh. Data regarding socio-demographic, anthropometric, fasting blood glucose, total cholesterol (TC), triglyceride (TG), high-density lipoprotein (HDL), and low-density lipoprotein (LDL) were collected from all the respondents. The association between dyslipidemia and its associated factors was analyzed using the multivariate logit regression model. The findings suggest that the prevalence rate of dyslipidemia was 75.7% in female and 72.6% in male T2DM patients. The odds of having dyslipidemia were 1.74 (95% Cl: 1.58–1.87) times significantly higher in female (p<0.001). The other factors associated with dyslipidemia encompassed age between 30–39 years (OR: 2.32, 95% CI: 1.97–2.69), obesity (OR: 2.63, 95% CI: 2.27–2.90), waist circumferences of male ≥90 and female ≥80 (OR: 1.65, 95% CI: 1.59–1.89), hypertensive patients (OR: 1.51, 95% CI: 1.45–1.74), physically inactive (OR: 3.25, 95% CI: 1.84–4.68), and current smoker or tobacco user (OR: 1.93, 95% CI: 1.85–2.13). This study concluded that the high prevalence of dyslipidemia was found among newly diagnosed type-2 diabetes patients and associated with gender, age, BMI, waist circumference, poor physical activity, and smoking, or tobacco use. This result will support increase awareness of dyslipidemia and its associated risk factors among type-2 diabetes patients.

AbstractNesfatin-1, which is derived from nucleobindin2 (NUCB2), has been recently identified as a novel satiety regulator. However, its pathophysiological role in humans remains unknown. The aim of the present study was to investigate plasma nesfatin-1 levels and the association between plasma nesfatin-1 levels and various metabolic parameters in humans.74 subjects with newly diagnosed type 2 diabetes mellitus (nT2DM), 73 subjects with impaired glucose tolerance (IGT) and 73 subjects with normal glucose tolerance (NGT) were enrolled in this study. Plasma nesfatin-1 levels were measured by a commercially available enzyme- linked immunosorbent assay.Plasma nesfatin-1 levels were elevated in subjects with both nT2DM and IGT compared to controls (1.91±0.79 and 1.80±0.80 vs. 1.41±0.58 μ g/L, P<0.05 or P<0.01 ). Simple regression analysis showed that in subjects with IGT and nT2DM, plasma nesfatin-1 correlated positively with body mass index (BMI), hemoglobin A1c (HbA1c), fasting blood glucose (FBG), 2 h blood glucose after a glucose load (2hPBG), fasting plasma insulin (FINS) and the homeostasis model assessment of insulin resistance (HOMA-IR). Multivariate logistic regression analysis revealed that plasma nesfatin-1 was significantly associated with IGT and nT2DM, even after controlling for differences in BMI.Plasma nesfatin-1 concentrations were found to be elevated in subjects with both IGT and nT2DM and to be related with several clinical parameters known to be associated with insulin resistance.
