Aims. To study the factors associated with the DSH in the elderly group of 60 years and above and to recommend changes to be implemented in order to improve the management in this specific group. Materials and Methods. Five-year retrospective study was undertaken from July 2005 to July 2010 in the Plastic Surgery Department of the Royal Preston Hospital, NHS Trust. A Performa was designed to collect data about the inpatient admission and included certain areas of key information. The case notes for all patients were extensively analysed in order to gather adequate information for the devised Performa. Results. DSH is getting more common in the elderly group, and males are more affected than females. 60% of the patients had a previous history of DSH. A large number (80%) of patients had a previous history of mental illness. 60% of those DSH patients were living with family. Almost all patients (90%) were reviewed by the Psychiatry Liaison Team. The timing of patients being assessed was highly variable. Conclusions. Marriage is not a protective factor in the prevention of the DSH in the elderly group. A mental health team referral in the early phases of the management would be of huge benefit and a likely step to prevent possible future admissions. The Department would benefit from the creation of a protocol for the management of these patients. There should be a joint effort of the professionals in the management of DSH in the elderly, and GPs play a very important role in the prevention of DSH in the later life.

A case of sub-acute intestinal obstruction due to spontaneous complete transmural migration of a retained surgical mop into the small intestinal lumen has been treated surgically in surgery department of Rajshahi Medical College Hospital. The patient presented with colicky abdominal pain, nausea, occasional abdominal distention and vomiting. She had history of abdominal operation twice previously. Abdominal examination revealed a mobile lump in the right side of abdomen. Diagnosis could be made only at laparotomy.   doi: 10.3329/taj.v19i1.3167 TAJ 2006; 19(1): 34-37

A 60 years old postmenopausal lady was admitted in the surgery department of Islami Bank Medical College Hospital with complaints of a lump in anal region and occasional per rectal bleeding for six months. Examination revealed a small well defined globular mass in anal canal which extended from the anal verge up to lower rectum internally. Internal part was ulcerated from where foul smelling discharge was coming out. She had no history of exposure to any radiation or any surgery over the anal canal. On radiogram of the chest a cannon ball shadow was found. Incisional biopsy of the lesion revealed malignant melanoma. Abdomino-perineal resection of rectum and anal canal was done for the lesion with an uneventful postoperative period. Adjuvant therapy could not be instituted because the patient could not afford it. Background: Palatal fractures are often associated with maxillofacial fractures and Le Fort fractures. The diagnosis and management of palatal fractures in the midface area is a challenge for a plastic surgeon in restoring function and aesthetics. With the results of this study, it is expected to be a database of maxillofacial fractures treated at SMF Reconstructive Plastic Surgery and Aesthetic Dr. Soetomo, Surabaya and gave the ability to make a fast and precise diagnosis for time and technical maxillofacial fractures.Methods : This study uses medical record data for all patients diagnosed with palatal fractures in Dr. Soetomo General Hospital, Surabaya during January 2012 to December 2017. The variables studied were demographic data including sex, age, mechanism of occurrence of accidents, types of fractures, management, complications that occur and length of treatment.Results : There were 82 patients with palatal fractures, with traffic accidents being the most common cause of palate fracture (n = 61) followed by workplace accidents and households in second place (12 and 9%). Most sufferers were men (68%), women (14%) with the highest age range of men aged 19-30 years who were followed by ages 31-45. The most were parasagittal fractures (56%), then Sagittal (15%), paraalveolar (9%), alveolar (1%), comminutive (1%). no fractures with anterior and posterolateral alveolar types, posterolateral type or transverse type fractures. Hospitalization period with plating (12 days), transmolar wiring (10.6 days), conservative (13.8 days).Conclusions: In this study assessed the experience in the reconstruction and aesthetic plastic surgery department of Dr. Soetomo General Hospital regarding palatal fractures and accompanying demographic data. The type of fracture that occurs is also related to the management performed. Incomplete medical records caused problems in this study.

Son of Ram Das Joshi, Prof Keshav Das Joshi was born at Kathmandu. He completed MBBS on 1968 from Trivandrum Kerela, residency (1970 to 1974) in MS (General Surgery) on 1974 from AIIMS, New Delhi. He served as a registrar on 1975 at Maulana Azad Medical College and training in Clinical Oncology in Tokyo at the National Cancer Hospital) on 1983. He had his training in Plastic Surgery in U.K. 1987 and various periods of training and Observation in Burn & Plastic Surgery in Canada, Belgium, France, Singapore and Switzerland. Since 2032 he has worked as a general surgeon in Butwal Hospital, Lumbini Anchal, Bheri Anchal Hospital followed by six years in Gandaki Zonal Hospital a total of 10 years. The history of Burns and Plastic surgery services in Nepal was started in Bir hospital. The separate Burns and Plastic surgery unit was established in the year 1998 AD. Dr Keshav Das Joshi, senior consultant plastic surgeon, proposed it. In his endeavor the first organized Burn Unit was established in 2000 A.D. For his contribution to the society he has been felicitated by the government by ‘Gorkha Dakshin Bahu’ Third, ‘Trishaktipatta’ Third, ‘Birendra Aishworya Sewa Padak’ etc to name a few. He served as the president of Society of Surgeon of Nepal for the period 1999 – 2002. He served as the editor of souvenir of the third International Surgical Conference of SSN on 1994. He had a major role in the establishment of the Journal of Society of Surgeons of Nepal (JSSN). He served as the chief editor for the period 1998, 1999.

Background: The term "ovarian cancer" includes several different types of cancer that  arise from cells of the ovary, most commonly, tumors arise from the epithelium or lining cells of the ovary.  Ovarian cancer risk is positively associated with higher consumption of dietary cholesterol and eggs, and inversely associated with a higher intake of vegetables. High consumption of fats may increase circulating estrogen levels, thus increasing the possibility of cell damage and proliferation that is responsible for cancerous growth. Material & Methods: The present study was conducted at Geetanjali Medical College and Hospital, Udaipur (Rajasthan). Total  100 cases (females) attending the obstetrics and gynecology department for some gynecological and other problem  were selected for this study between the age of 40-60 years, who were attending cancer centre at GEETANJALI MEDICAL COLLEGE AND  HOSPITAL, Udaipur (Rajasthan).                GROUP I: - It consisted of healthy females control subjects (n=50) .By routine examination and tests, we ensured that all the subjects were healthy and there were no signs and symptoms or history of ovarian tumor and diseases GROUP II: - It consisted of ovarian cancer females subjects (n=50) with a history of ovarian tumor. Results:   Higher level of cholesterol, LDL, VLDL and low level of HDL are found in ovarian cancer patients. Conclusion: The present study we highlights the importance and role of serum lipid profile in diagnosis, prognosis and recurrence of the disease. The study shows that serum level of cholesterol, LDL, VLDL was elevated in  patients of ovarian cancer while low level of HDL are found in ovarian cancer patients.
