Retrospective Assessment of Occupational Exposure to Whole-Body Vibration for a Case-Control Study

BACKGROUND: Dumper operators in mines worldwide are subjected to Musculoskeletal Disorders (MSDs) due to whole-body vibration exposure. This study evaluated the working-life-Whole-Body Vibration (WBV)-exposure and their association with various MSDs among dumper operators in mines which remains poorly addressed. METHODS: This case-control study in Indian iron ore mines was conducted to compare randomly selected 65 dumper operators and 65 office workers. Data were collected through face-to-face interviews using the Nordic Musculoskeletal Questionnaire (NMQ) and were analysed using logistic regression models. RESULTS: The study revealed that majority of the dumper operators were exposed to WBV exceeding the ISO-2631 limits. Compared with controls, the dumper operators had a much higher risk of upper back pain (age-overweight-adjusted odds ratio ORao = 5.37, 95%CI = 1.78–16.20), lower back pain (ORao = 2.72, 95%CI = 1.25–5.94), knee and leg pain (ORao = 3.68, 95%CI = 1.22–11.11), and having 2+ MSDs (ORao = 5.05, 95%CI = 1.88–13.51, vs. no MSDs). Working-life-WBV-exposure was higher among dumper operators having upper back pain (mean (SD) = 7.1 (1.91) vs. 5.7 (1.91), p <  0.01) and lower back pain (mean (SD) = 6.63 (2.10) vs. 5.55 (1.71), p <  0.01) compared to those without these MSDs. Older age was associated with higher risk of MSD pains. CONCLUSION: Dumper operators have excess MSDs due to high working-life-WBV-exposure. Their MSDs and working-life-WBV-exposure should be regularly evaluated and reduced.

Abstract BackgroundAlthough a causal relationship between mesothelioma and asbestos exposure is well known, few studies have shown a relationship to non-occupational exposure, including neighborhood exposure, most likely because of the large effect size of occupational exposure. The aim of this study was to quantify the risk of malignant mesothelioma death associated with neighborhood asbestos exposure due to a large-scale asbestos-cement (AC) plant in Amagasaki, Japan, by properly adjusting for occupational exposure. MethodsThis was a nested case-control study in which a fixed population of 143,929 residents who had been living in Amagasaki City between 1975 and 2002 were followed from 2002 to 2015. All 133 cases and 403 matched controls were interviewed about their occupational, domestic, household, and neighborhood asbestos exposures. Odds ratios (ORs) for mesothelioma death associated with neighborhood exposure were estimated by a conditional logistic-regression model that adjusted for other asbestos exposures. We adopted cumulative indices that considered residence-specific asbestos (crocidolite) concentrations and durations during the potential exposure period of 1957-1975 to evaluate individual neighborhood exposures.ResultsThere was an increasing, dose-dependent risk of mesothelioma death associated with neighborhood exposure, demonstrated by ORs in the highest quintile category that were 21.4 (95%CI: 5.8 to 79.2) for all, 23.7 (95% CI: 3.8-147.2) for males and 26.0 (95% CI: 2.8-237.5) for females, compared to the lowest quintile, respectively. These results clearly demonstrated no substantial differences between males and females in relation to the magnitude of risk from neighborhood exposure.Our findings suggest that the risk of mesothelioma death associated with neighborhood exposure persists and will not be diminished for many years, even though it has been decades since the AC plant closed. ConclusionsBy adjusting for occupational and other asbestos exposures, a dose-dependent relationship was demonstrated between mesothelioma death and neighborhood asbestos exposure from a large-scale AC plant.

ObjectivesTo assess the relationship between occupational exposure to metalworking fluids (MWFs) in the steel-producing industry and bladder cancer incidence.MethodsA nested case–control study on bladder cancer was set up in a cohort of workers from six French steel-producing factories. Three controls were randomly selected for each incident bladder cancer case diagnosed from 2006 to 2012. Controls were matched to cases on age at diagnosis and counter-matched on a surrogate measure of exposure to MWFs derived from a job-exposure matrix. Cases (n=84) and controls (n=251) were face-to-face interviewed. Experts assessed occupational exposure to MWFs (straight, soluble and synthetic) using questionnaires and reports from factory visits. Occupational exposures were based on three metrics: duration, frequency-weighted duration and cumulative exposure index. Conditional multiple logistic regressions were used to determine ORs and 95% CIs, taking non-occupational and occupational exposure into account.ResultsIn the 25 years before diagnosis, ORs increased significantly with duration of exposure to straight MWFs (OR=1.13 (1.02–1.25)) and increased with frequency-weighted duration of exposure to straight MWFs (OR=1.44 (0.97–2.14)). These results remained valid after adjusting for duration of smoking, average number of cigarettes smoked per day, time since smoking cessation and exposure to polycyclic aromatic hydrocarbons (PAHs). ORs also increased with soluble MWFs but not significantly. No significant association was found with older exposures to MWFs or with exposure to synthetic MWFs.ConclusionThe increased risk of bladder cancer observed among workers exposed to straight MWFs and to a lesser extent to soluble MWFs may be explained by the presence of carcinogens (such as PAH) in mineral oils component of straight and soluble oils. Prevention therefore remains necessary in sectors using MWFs.
