The aim of the research was to study the effect ofoxidative factors impact and modality of renal replacement therapy (BBT) on indices ofoxidative stress (OS) and resistance of erythrocytes membranes in patients with chronic kidney disease stage V(CKD VD) and anemie. Material and methods. The study involved 68 patients with CKD VD: 14 patients were treated by hemodiafiltration (HDF), 25 patients by hemodialysis (HD) and 29 patients by peritoneal dialysis (PD). The severity ofanemia was assessed according to the KDIGO (2012) criteria. The control group consisted of 30 healthy people of the same age and sex. Along with the standard diagnostic methods, we defined the content of malonic dialdehyde in serum (MDAs) and in erythrocytes (MDAe), the content of ceruloplasmin (CPs), transferrin (TBs) and SH-groups in the blood serum, the index of the OS (IOS), catalase activity in serum (CATs), glucose-6-phosphate dehydrogenase (G-6-PDHe) and total peroxidase activity (TPA) in erythrocyte, peroxide resistance (PR) of red blood cells and erythrocyte membrane permeability (EMP). Statistical analysis was performed using the programs of Microsoft Excel 7.0. Results. It has been stated that in the CKD VD patients in compatison with control group the MDAs content increased by 3.3 times and MDAe - 1.2 times, TBs content reduced by 34%, SH-groups - by 31%, TPAe - by 41% and G- 6-FDGe - by 58%, PB-by 60%; 4.6 times increased CATs activity and OSI; 2 times grew peroxide hemolysis (PH) and 1.3 times - EMP. The analysis (depending on the BBT modality) showed that the patients treated by HDF had typical MDAs increase by 3.9 times on a background of CPs by 24%, TBs - 33%, SH-groups - 25%, TPAe - 51%, G-6-PDHe - 42%; the increase in serum OSI - 5.4 times and 2.6 times in erythrocytes, PB - by 3.6 times and CATs activity by 3.5 times; HD group was characterized by the highest value of MDAe, OSI, PH and CATs, along with more expressed decrease of TBs indices, SH-groups, TPA and G-6-FDHe activity compared with rates in patients with HDF. The patients treated with PD had the lowest content of MDAs and the highest values on the background ofTPAe, the significant increase of CPs by 1.7 times and lowest TBs and G-6-PDHe. The patients with PD showed twice lower OS activity by OSI. Conclusion. Thus, in patients with CKD VD, who had HD, HDF or PD an anemie was associated with high OS activity and the increased degree of hemolysis. These changes are stipulated by BBT methods: for patients receiving HDF were typical the lowest rates of hemolysis and the highest degree ofprotection for erythrocytes, and for patients treated with HD - the highest OS.

Material and methods. The study involved 47 patients with CKD VD: 14patients were treated by hemodiafiltration (HDF), 14 patients by hemodialysis (HD) and 19 patients by peritoneal dialysis (PD). The patients treated with PD had the lowest content of MDAs and the highest values on the background ofTPAe, the significant increase of CPs by 1.7 times and lowest TRs and G - 6 - PDHe. The patients with PD showed twice lower OS activity by OSI. Conclusion.Thus, in patients with CKD VD, who had HD, HDF or PD an anemic syndrome was associated with high OS activity and the increased degree of hemolysis. Patients with chronic kidney disease stage 5 belong to the group of patients with the highest risk of complicated course of COVID-19. The purpose of the study was to study the prevalence and clinical and laboratory features of the COVID-19 infection among patients with end-stage of chronic kidney disease who receive renal replacement therapy. Materials and methods. The study included 172 patients with chronic kidney disease stage 5, who were receiving renal replacement therapy and who were diagnosed with COVID-19 for the period from April 2020 to April 2021. According to the type of renal replacement therapy all patients were divided into 3 groups: group 1 – 98 (56.9%) patients who received hemodialysis, group 2 – 60 (34.8%) patients who were on peritoneal dialysis, group 3 – 14 (8.1%) patients with kidney transplantation. The main clinical and laboratory parameters that characterize the course of the COVID-19 infection such as fever, diarrhea, anosmia, the percentage of oxygen saturation, the percentage of lung damage, levels of total protein, albumin, C-reactive protein, urea, creatinine, D-dimer, hemoglobin, leukocytes, platelets were evaluated. Results and discussion. The COVID-19 infection was diagnosed in 172 (38.2%) patients, the vast majority of whom – 110 (63.9%) patients were women with a mean age of 47.4±4.6 years. The analysis of clinical and laboratory parameters revealed the most severe course of the disease in patients with a kidney transplant and in patients who were on hemodialysis. In the group of renal transplant recipients there was a significant increase in the level of D-dimer, as well as a higher percentage of lung damage, compared with the average values of these indicators among patients on hemodialysis and peritoneal dialysis, but mortality in this group was 13.3% and was significantly lower than in the group of hemodialysis patients – 27.5%. It should be noted that in patients on renal replacement therapy by the method of hemodialysis, the adequacy of dialysis therapy was insufficient, which led to a more pronounced overall inflammatory response, which was characterized by the highest level of C-reactive protein (64.4±5.1 mg/l), compared with patients in the other study groups. The course of the disease in patients on hemodialysis was also complicated by anemia. In patients on renal replacement therapy by peritoneal dialysis, the COVID-19 infection was characterized by a mild course, with the lowest percentage of lung damage (22.0±2.8%) and fatalities (11.6%). Conclusion. According to our study, the incidence of COVID-19 among patients receiving renal replacement therapy is 38.2%. A more severe course of the disease was registered in the group of patients with a kidney transplant, due to the nature of comorbid pathology, as well as immunosuppressive therapy. The data obtained allow us to consider patients with a transplanted kidney as a risk group for severe COVID-19 infection

Background and objectives: Optimal timing of arteriovenous fistula placement in chronic kidney disease remains difficult and contributes to high central venous catheter use at initial hemodialysis. We tested whether a prediction model for progression to renal replacement therapy developed at Kaiser Permanente Northwest may help guide decisions about timing of referral for arteriovenous fistula placement. Design, setting, participants, and measurements: A total of 205 chronic kidney disease stage 4 patients followed by nephrology referred for arteriovenous fistula placement were followed for up to 2 years. Patients were censored if they died or discontinued Kaiser Permanente Northwest coverage. Survival analyses were performed for overall progression to renal replacement therapy divided by quartiles based on 2-year risk for renal replacement therapy and estimated glomerular filtrate rate at time of referral. Results: By 2 years, 60% progressed to renal replacement therapy and 11% had died. 80% in the highest risk versus 36% in the lowest risk quartile progressed to renal replacement therapy (predicted risk 84% vs 17%). 75% in the lowest estimated glomerular filtrate rate versus 56% in the highest estimated glomerular filtrate rate quartile progressed to renal replacement therapy (mean estimated glomerular filtrate rate 13 mL/min vs 21 mL/min). The hazard ratio was significantly higher for each consecutive higher renal replacement therapy quartile risk while for estimated glomerular filtrate rate, the hazard ratio was only significantly higher for the lowest compared to the highest quartile. The extreme quartile risk ratio was higher for 2-year risk for renal replacement therapy compared to estimated glomerular filtrate rate (4.0 vs 2.4). Conclusion: In patients with chronic kidney disease stage 4 referred for arteriovenous fistula placement, 2-year renal replacement therapy risk better discriminated progression to renal replacement therapy compared to estimated glomerular filtrate rate at time of referral.

this clinical case demonstrates renal amyloidosis – the most severe complication of familial Mediterranean fever (FMF). This clinical case gives an example of rapid evolution of renal failure in the lack of treatment. 62 years old man, Armenian, consults  a physician with such complaints as evening fever up to 38° for a month, frequent urination and dark urine. Biochemical blood test revealed an increase in the following indices: creatinine-489 μmol / l, urea 28.3 μmol / l, uric acid 619 μmol / l. GFR-6ml / min. No amyloid deposits were detected by biopsy of the buccal mucosa. The diagnosis of FMF, chronic kidney disease (CKD) stage 5. Initiation of treatment. Tenkhoff catheter installation. Treatment includes сontinuous ambulatory peritoneal dialysis (CAPD) in mode 4 exchanges with a glucose solution of 1.36% 2.0 liters. The dynamics of treatment is positive in  presence of constant replacement therapy. A feature of this clinical case is the insidious development of secondary (AA) amyloidosis in uncontrolled patient, which leads to the development of end-stage chronic kidney disease (CKD). This case demonstrates the need of constant examination and treatment of the patients with end-stage CKD by renal replacement therapy, which significantly reduces the quality of patient`s life.
