Medical students perceptions of the influence of undergraduate experiences on making a career choice in psychiatry

Purpose Psychiatric recruitment and retention are at an unprecedented low within the UK. The reasons for this shortfall may include public and professional stigma, recent NHS service developments and changes in undergraduate training. The purpose of this study is to explore medical student’s perceptions of the nature and magnitude of these factors on influencing whether or not they would choose a career in psychiatry. Design/methodology/approach This qualitative study was conducted with year four medical students at a single UK University with low levels of recruitment into psychiatry. Two focus groups were asked about their undergraduate experience within the speciality. Thematic analysis of the resulting transcripts enabled the identification of codes and over-arching themes, which formed the focus of this study. Findings Four key themes were identified during analysis and these included: the core subject matter (of psychiatry) viewed as being different; curriculum or course variables; interpersonal factors and; career factors. Placement enjoyment, positive role-modelling and enthusiasm were all important when considering psychiatry as a career. Therapeutic success, career flexibility and pay-banding were also powerful determinants. Practical implications These findings led to the initiation of an apprentice programme for undergraduates on psychiatric placement, designed to enhance the student experience of psychiatry and the perception of the speciality as a career. Originality/value While there are a number of possible solutions to current adverse national trends in psychiatric recruitment, increasing efforts to increase both the variety and quality of undergraduate placements and establishing a clearer sense of team identity is of vital importance if these patterns are to be reversed.

UNSTRUCTURED The UK General Medical Council (GMC) explicitly states doctors have a duty to ‘contribute to teaching and training…by acting as a positive role model’. However, recent studies suggest some are not fulfilling this, which is impacting medical students' experiences and attitudes during their training. As such, doctors have a duty to act as role models and teachers, as specified by the GMC, which it seems are not currently being fulfilled. This would improve the medical students’ learning experiences and demonstrate good professional values for them to emulate. Therefore, these duties should be as important as patient care, since this will influence future generations.


Subject Russia's Foreign Intelligence Service. Significance President Vladimir Putin said yesterday that according to Russian intelligence, the United States was already supplying military equipment to Ukraine. While the overt conflict in Donbas has raged, behind the scenes the covert struggle between Russia and the West has been intensifying. Russia's Foreign Intelligence Service (SVR) is highly active and currently enjoying a resurgence in its political fortunes. However, it is doubtful whether it is playing an especially positive role in framing Kremlin policy. The SVR is Russia's direct equivalent to the CIA and the UK Secret Intelligence Service, and generally operates out of Russian embassies with most officers working under diplomatic cover. It also operates a large network of so-called 'illegals' who have no diplomatic immunity. Impacts The SVR will continue to seek to expand its operations, especially so long as its resources remain protected. It will concentrate especially on Europe, seeking to foment divisions within and between members of NATO and the EU. SVR will continue to struggle for political traction in Moscow, requiring it to reflect the official line and leap on the latest bandwagon.

BackgroundMost patients are cared for in the community by GPs, and workforce planning for the UK NHS suggests that 50% of medical school graduates need to train as GPs. While there is pressure on medical schools to provide more student teaching in general practice, general practice must be prepared for this increase in workload and teachers should have appropriate training.AimThis study examined the influence that teachers as role models can have on the development and career choices of medical students and whether GPs who teach medical students might benefit from assistance in developing positive attributes.MethodA literature search was carried out. Relevant papers were those that examined the influence of a doctor as teacher role model for medical students, both in assisting in professional development and clinical skills, and in influencing career choice.ResultsThe review identified eleven papers. There was evidence of association between a student having an influential role model as teacher and choosing specialty training in that area, although evidence of a causative connection is less convincing. A recurring theme is the recommendation that teachers should be aware of the influence they can have as role models, both positive and negative.ConclusionAs medical student teaching moves into general practice GPs who teach will need to be helped to understand and develop positive role model attributes, to promote general practice as an attractive career to the students. Further research needs to identify the extent of assistance required and whether medical schools are prepared for this extra responsibility.

AbstractBackgroundThere is growing concern over a future shortfall in provision of UK otolaryngology consultants. There is a declining rate of applications to otolaryngology specialty training in the UK.ObjectiveThis study aimed to systematically review the literature to establish what factors influence medical students’ and junior doctors’ decision to pursue a career in otolaryngology.MethodMedline, Embase and PubMed databases were searched in January 2019. Additional manual reference checks of identified literature were performed.ResultsEleven articles were included in the review. Common factors that positively influenced the decision to pursue a career in otolaryngology were exposure to the specialty, positive role models and a good work-life balance. Lack of exposure was a consistent deterrent from pursuing a career in otolaryngology.ConclusionThis review reiterates the need for greater exposure to otolaryngology in the undergraduate curriculum. In addition, mentorship for students with an interest in otolaryngology should be a priority.

BACKGROUND Role modelling exponentially affects the attitude and behaviours of medical students and act as a key factor in developing professional attributes in them. Medical students observe role models and learn from them more than through formal teaching. This study investigates the determining elements that make a teacher a role model, so that role modelling can be used as an effective educational tool. METHODS A qualitative exploratory study was conducted on fifth year medical students of three medical colleges of Lahore, Pakistan. Fifteen, telephonic, semi structured interviews were included in the study. Thematic analysis was done by manual coding of transcribed interviews. RESULTS A total of 374 codes were generated in the first cycle of coding, that were further merged to 42 in the second cycle. These codes led to 5 subthemes that finally emerged as two themes. The first theme was “Finding ways around their blaring blunders - are we blind?” highlighting the attributes of negative role models which include humiliation of students and juniors, poor teaching and communication skills, lack of empathy for patients and unethical relationship with pharmaceutical industry. The second theme was “Walking the Line” focusing on the components of positive role modelling like formal workplace attire, candid approach in teaching, strategic dealing with students, empathetic attitude towards patients and prioritising fair treatments. CONCLUSIONS Medical education is not only about acquisition of new knowledge and skills but is also about acquiring physician’s character and identity. The study findings suggest that the medical students pay attention to both positive and negative attributes of their teachers and their influence on promoting professionalism and character development. It is suggested that both teachers and administrators should understand the significance of role models and advocate excellence in role modelling. Purpose – The purpose of this paper is to review Advertising in a Free Society – a defence of the advertising industry – by Ralph Harris and Arthur Seldon, and to evaluate its status as a justifiable forgotten classic of the marketing literature. Design/Methodology/Approach – Advertising in a Free Society is placed in historical context (the Cold War), summarised and reviewed. Findings – During the 1950s, as the UK experienced a period of affluence and growing consumerism, the advertising industry was again subject to the criticisms that had been levelled at it by influential scholars in the late nineteenth and early twentieth centuries. Against this context, Advertising in a Free Society deserves to be remembered as one of the earliest defences of advertising and remains highly relevant. Harris and Seldon were leading figures in the Institute of Economic Affairs (IEA), joining shortly after its inception, which became an influential group both in the UK and abroad, influencing policy on free markets. Originality/Value – Although Advertising in a Free Society attracted few citations (going out of print between its publication in 1959 and 2014 when it was republished by the IEA), and largely forgotten by marketing scholars, it provides a significant source for marketing historians interested in advertising criticism, the growth of the British advertising industry and the role of advertising in democratic societies. A reanalysis of the text situated in its historical context – the height of the Cold War – reveals that the text can be viewed as an artefact of the conflict, deploying the rhetoric of the period in defending the advertising industry and highlighting the positive role that advertising could make in free societies.

Purpose – The purpose of this paper is to examine the experience of the UK office market in embracing green buildings. The empirical analysis considers the spatial pattern and growth of green buildings in cities since 1990. It examines the perceived industry wisdom that the establishment of a green premium for occupation is the key to greening the office stock. Design/methodology/approach – The paper begins by looking at the concept of a green office and then examines the evolving attitudes towards these offices and the issues for local market dynamics. The empirical analysis examines the current spatial pattern of green office buildings in the UK and then their impact on city office markets, where there is a major concentration. The latter part of the paper examines the growth of green offices since 1990. It begins with national trends and then examines the evolution of green development in individual cities. Findings – The initial adoption of green offices was slow. There has been a dramatic rise in green offices at the peak of the past decade’s development boom and in the immediate years that followed. Market acceptance of the importance of greenness appears still to be in the melting pot with limited market transactions since 2008. Green offices represent only 2.7 per cent of office buildings and 12 per cent of total space in the market. Most green offices are in the principal cities with the largest concentration in London. London represents the only potential locality where a green market could have been established so far. Practical implications – The paper provides an empirical assessment of the growth of green offices in the UK. Originality/value – This is the first paper to consider the development and scale of green offices in the context of local markets. It challenges the perceived wisdom that a green premium is central to the green transformation to date.

Aims and MethodThere is a shortage of doctors in the UK, particularly in psychiatry and pathology. Little is known about prospective medical students' career intentions or attitudes. This study aimed to report on the career intentions and attitudes to psychiatry of 819 attenders at a sixth-form conference for prospective medical students.ResultsA much higher proportion of students expressed favourable attitudes to psychiatry as a career than might have been expected. The most popular career was paediatrics and the least popular was genitourinary medicine.IMPLICATIONSMedical schools need to be proactive in providing information, career advice and positive role models. This may counteract negative propaganda and encourage career choice in tune with students' earlier feelings and patients' needs.

Abstract Background Training for a career in podiatry is reported to provide graduates with excellent employability, alongside professional autonomy and suitable renumeration. Yet, there has been an ongoing decline in the number of those applying to study the subject. There is limited literature associated with this topic and we sought to explore the factors that attract ‘generation Z’ (those born 1995–2010) to a potential career in podiatry. Method A qualitative design framework underpinned by phenomenological principles used four focus groups over a two-year period to generate data from participants at University and in Further Education. Focus group conversations were led by external facilitator, recorded, independently transcribed verbatim and anonymised prior to thematic analysis. This was followed by external, independent verification of themes. Results Four main themes were determined from the analysis i) a lack of awareness of podiatry; ii) podiatry: accessible course, accessible career; iii) career status; iv) breadth/opportunity of the scope of practice. Both positive and negative experiences were reported and highlighted key gaps in how the attractiveness of a career in podiatry is portrayed. Conclusion The chronic lack of awareness of podiatry as a career clearly needs to be addressed, ideally with more positive role modelling in mainstream and popular media. The career status offered together with the breadth of, and opportunity associated with, the scope of practice should continue to be celebrated.
