Remote monitoring system of vital signs for triage and detection of anomalous patient states in the emergency room

This paper presents a real-time monitoring system with a novel approach to assess the human health status without the need for using a body sensor. The project mainly targets improving the quality of life for those living independently but still require close monitoring. Skin fluctuation of the human face is monitored real time with a high-speed camera to determine vital signs including the heart rate and blood pressure. A few image processing algorithms have been utilized to determine the image fluctuations and extract the related features and acquire vital signals. An algorithm assesses and evaluates the risks involved in irregular behaviors and takes follow up actions where required. The application has been implemented on two platforms and interfaced with a high-speed camera to evaluate the performance of the remote monitoring system in indoor situations.

BACKGROUND Febrile neutropenia represents one of the most common oncologic emergencies and is associated with significant, preventable morbidity and mortality. The vast majority of patients suffering a febrile neutropenia episode are hospitalized, resulting in significant economic cost. OBJECTIVE This exploratory study implemented a remote monitoring platform including a digital infrared thermometer and a pulse oximeter with the capability to notify providers in real-time of vital signs abnormalities that could suggest early clinical deterioration, and thereby improve upon clinical outcomes. METHODS The remote monitoring system was implemented versus standard of care vital signs monitoring in hospitalized patients with underlying hematologic malignancies complicated by a febrile neutropenia episode in order to assess both feasibility and validity of the system. RESULTS Intraclass correlation coefficient analysis (ICC), confirmed the high repeatability and accuracy of heart rate assessment (ICC= 0.856), acting as a supplement to sole, remote temperature assessment. While the sensitivity and specificity for capturing tachycardia above a rate of 100 was excellent (88% and 97% respectively), the sensitivity of the remote monitoring system capturing temperature greater than 100 degrees Fahrenheit and oxygen saturation less than 92% was 45% and 50% respectively. CONCLUSIONS Overall, this novel approach including temperature, heart rate and oxygen saturation assessment successfully provides real-time, clinically valuable feedback to providers. While the temperature and oxygen saturation lags in terms of sensitivity when compared to a standard in-hospital system, the heart rate data helps overcome some of this deficit, and as a whole, the system provides additional information that can be applied to a clinically vulnerable population. By transitioning its application to the high-risk patients in the outpatient setting, the novel system can help prevent additional healthcare utilization through early provider intervention and potentially improve outcomes.

To improve the efficiency of the remote monitoring system for logistics transportation, we proposed a remote monitoring system based on wireless sensor network and GPRS communication. The system can collect information from the wireless sensor network and transmit the information to the ZigBee interpreter. The monitoring system mainly includes the following parts: Car terminal, GPRS transmission network and monitoring center. Car terminal mainly consists by the Zigbee microcontroller and peripherals, wireless sensor nodes, RFID reader, GPRS wireless communication module composed of a micro-wireless monitoring network. The information collected by the sensor communicates through the GPRS and the monitoring center on the network coordinator, sends the collected information to the monitoring center, and the monitoring center realizes the information of the logistics vehicle in real time. The system has high applicability, meets the design requirements in the real-time acquisition and information transmission of the information of the logistics transport vehicles and goods, and realizes the function of remote monitoring.
