This chapter advocates the use of a value-sensitive design (VSD) approach toward deriving patient intelligence by illustrating that the insights provided by the healthcare data that captures patients' concerns, needs, and desires—known as values—provide more sustainable care. Authors examine three cases extracted from top information systems (IS) peer-reviewed journals in which medical data is collected and analyzed and in which intelligence is derived through a VSD framework. VSD is a three-part methodology that comprises conceptual, empirical, and technical investigations. This chapter investigates the value sensitivity of the following key activities and tasks that result in intelligence from data: data collection, data analysis, and data reporting.

ZusammenfassungDie Behandlung ausgedehnter Knochen-defekte nach Traumata oder durch Tumoren stellt nach wie vor eine signifikante Heraus-forderung im klinischen Alltag dar. Aufgrund der bestehenden Limitationen aktueller Therapiestandards haben Knochen-Tissue-Engineering (TE)-Verfahren zunehmend an Bedeutung gewonnen. Die Entwicklung von Additive-Manufacturing (AM)-Verfahren hat dabei eine grundlegende Innovation ausgelöst: Durch AM lassen sich dreidimensionale Gerüstträger in einem computergestützten Schichtfür-Schicht-Verfahren aus digitalen 3D-Vorlagen erstellen. Wurden mittels AM zunächst nur Modelle zur haptischen Darstellung knöcherner Pathologika und zur Planung von Operationen hergestellt, so ist es mit der Entwicklung nun möglich, detaillierte Scaffoldstrukturen zur Tissue-Engineering-Anwendung im Knochen zu fabrizieren. Die umfassende Kontrolle der internen Scaffoldstruktur und der äußeren Scaffoldmaße erlaubt eine Custom-made-Anwendung mit auf den individuellen Knochendefekt und die entsprechenden (mechanischen etc.) Anforderungen abgestimmten Konstrukten. Ein zukünftiges Feld ist das automatisierte ultrastrukturelle Design von TE-Konstrukten aus Scaffold-Biomaterialien in Kombination mit lebenden Zellen und biologisch aktiven Wachstumsfaktoren zur Nachbildung natürlicher (knöcherner) Organstrukturen.

A pilot medical information system is being implemented and currently is providing services for limited categories of patient data. In one year, physicians’ diagnoses for 500,000 office visits, 300,000 drug prescriptions for outpatients, one million clinical laboratory tests, and 60,000 multiphasic screening examinations are being stored in and retrieved from integrated, direct access, patient computer medical records.This medical information system is a part of a long-term research and development program. Its major objective is the development of a multifacility computer-based system which will support eventually the medical data requirements of a population of one million persons and one thousand physicians. The strategy employed provides for modular development. The central system, the computer-stored medical records which are therein maintained, and a satellite pilot medical data system in one medical facility are described.

Data reporting the experience with an optical mark page reader is presented (IBM 1231Ν1). Information from 52,000 persons was gathered in seven countries, decentrally coded and centrally processed. Reader performance rates (i.e. sheets read per hour, sheet rejection rates, reading error rates) and costs (coding, verification, reading, etc.) are given.
