To assess the benefit and safety of composite therapy (Tamsulosin and Trospium drugs) in treating benign prostatic enlargement complain, mainly overactive urinary bladder symptoms. OBJECTIVE: The study has been designed to estimate the benefit and safety of Trospium and Tamsulosin in processing of symptoms of excessive bladder and benign prostate hyperplasia (BPH). Patients and Methods: Prospective clinical trial study conducted at Al-Diwaniya teaching hospitals and private clinics from march 2016 to march 2017, to patients BPH and bothersome symptoms. 60 patients were treated with Tamsulocin and 60 patients were treated by Tamsulocin and Trospium. RESULTS: Median scores of IPSS, OABSS, and QOL proved no big difference between two study sets before staring treatment (P = 0.544), (P = 0.287) , (P = 0.668) consecutively. After one month, both treatments led to a big reduction in IPSS, OABSS median score; (P < 0.001), (P < 0.001). Following three months, both treatments resulted in significant reduction in IPSS (P < 0.001), OABSS (P < 0.001), QOL result, (P < 0.001) consecutively. Conclusion: These results suggest which treatment with Trospium and tamsulosin therapy provides benefit for men with natural - Syndrome of the acute urinary tract and benign enlarged prostate.

The evaluation of severity of symptoms, tracking complications, choosing effective and efficient approach to treatment, and prognosis is important in benign prostatic enlargement (BPE) patients. Measurement of intravesical prostatic protrusion (IPP) is an easy, affordable and non-invasive marker for benign prostatic obstruction with a high diagnostic value. This study aims to undertake a descriptive review of current literature for the role of IPP in the evaluation and treatment of patients with BPE. Relevant articles written in English language were retrieved from PubMed and Google Scholar. Keywords used for searching articles included intravesical prostatic Protrusion and Benign Prostatic Enlargement/ hyperplasia/ Obstruction; evaluation; and treatment. Papers published between January 1990 and October 2020 were extracted and evaluated. The analyzed studies showed that IPP can be assessed by various modalities and is important in the evaluation of symptom severity, tracking progression and complications of disease, choosing treatment option and prognostication. It is important that IPP be always assessed at the point of initial evaluation of BPE patients.

Male lower urinary tract symptoms are frequently due to benign prostatic enlargement. Medical therapy is strongly recommended in patients with moderate to severe symptoms. Lower urinary tract symptoms may require a different medical approach using drugs with different mechanisms of action. Alpha-1 blockers, muscarinic receptor antagonists and phosphodiesterase type 5 inhibitors are the most frequently used drugs. 5-Alpha reductase inhibitors are commonly prescribed to reduce prostate volume and to prevent benign prostatic hyperplasia progression. Currently, medical treatment of lower urinary tract symptoms suggestive of benign prostatic enlargement can be tailored according to different symptom characteristics and severity, and to different patient comorbidities and preferences. For this reason, the decision-making process should be based on an accurate patient counselling with detailed clarification of potential benefits and, above all, potential side effects of different drugs. This non-systematic review of the literature presents an update of the current options for medical treatment of lower urinary tract symptoms suggestive of benign prostatic enlargement, helping urologists in the decision-making and counselling process.

Shared decision-making (SDM), a component of patient-centered care, is the process in which the clinician and patient both participate in decision-making about treatment; information is shared between the parties and both agree with the decision. Shared decision-making is appropriate for health care conditions in which there is more than one evidence-based treatment or management option that have different benefits and risks. The patient's involvement ensures that the decisions regarding treatment are sensitive to the patient's values and preferences. Audiologic rehabilitation requires substantial behavior changes on the part of patients and includes benefits to their communication as well as compromises and potential risks. This article identifies the importance of shared decision-making in audiologic rehabilitation and the changes required to implement it effectively.
