Purpose To investigate the impact of organizational characteristics and processes of care on hospital mortality and resource use in patients with cancer admitted to intensive care units (ICUs). Patients and Methods We performed a retrospective cohort study of 9,946 patients with cancer (solid, n = 8,956; hematologic, n = 990) admitted to 70 ICUs (51 located in general hospitals and 19 in cancer centers) during 2013. We retrieved patients’ clinical and outcome data from an electronic ICU quality registry. We surveyed ICUs regarding structure, organization, staffing patterns, and processes of care. We used mixed multivariable logistic regression analysis to identify characteristics associated with hospital mortality and efficient resource use in the ICU. Results Median number of patients with cancer per center was 110 (interquartile range, 58 to 154), corresponding to 17.9% of all ICU admissions. ICU and hospital mortality rates were 15.9% and 25.4%, respectively. After adjusting for relevant patient characteristics, presence of clinical pharmacists in the ICU (odds ratio [OR], 0.67; 95% CI, 0.49 to 0.90), number of protocols (OR, 0.92; 95% CI, 0.87 to 0.98), and daily meetings between oncologists and intensivists for care planning (OR, 0.69; 95% CI, 0.52 to 0.91) were associated with lower mortality. Implementation of protocols (OR, 1.52; 95% CI, 1.11 to 2.07) and meetings between oncologists and intensivists (OR, 4.70; 95% CI, 1.15 to 19.22) were also independently associated with more efficient resource use. Neither admission to ICUs in cancer centers compared with general hospitals nor annual case volume had an impact on mortality or resource use. Conclusion Organizational aspects, namely the implementation of protocols and presence of clinical pharmacists in the ICU, and close collaboration between oncologists and ICU teams are targets to improve mortality and resource use in critically ill patients with cancer.

Intercropping provides opportunity to harness available resources. Thus, maize intercropped with soybean or peanut (groundnut) was tested with 1 : 1, 1 : 2, and 1 : 5 row proportions along with sole plantings of each crop to measure resource capture and resource-use efficiency. Results revealed that sole peanut had 60% higher maize-equivalent yield and 55% better production efficiency, followed by the 1 : 5 row proportion of maize–peanut, over sole maize. Intercropping increased land-use efficiency by 17–53% and land-equivalent coefficient by 0.21–0.56. The relative crowding coefficient was 39% higher with 1 : 2 maize–soybean, whereas the monetary advantage index was the highest (US$107) with the 1 : 5 maize–peanut. Nitrogen (N) and potassium (K) uptakes by maize were 42.9% and 38.2%, respectively, higher with 1 : 5 maize–soybean, whereas phosphorus (P) uptake was 64% higher with 1 : 5 maize–peanut. However, the least amounts of N, P and K were mined overall with 1 : 5 of maize–soybean. Soil moisture content was improved by 24% and solar radiation interception by 37.8% with soybean alone over maize alone, and these parameters increased with higher row proportions of soybean. The energy parameters also improved with the 1 : 5 row proportions of maize–soybean or peanut intercropping. This study provides a basis for efficient resource use by maize–soybean (or peanut) intercropping system.

Islamic economic thought became increasingly prevalent with the birth of various schools of thought in Islamic economics. One of them is the critical alternative school that was pioneered by Timur Kuran. This research will discuss from Islamic economics to the urgency of prohibiting usury; an offer of the East Kuran Islamic economy. With the aim of knowing and analyzing the extent to which Eastern Kuran's economic thought made significant contributions to the development of contemporary Islamic economics. This research is a qualitative research with a library research approach and all data obtained were analyzed using descriptive analysis methods. The results of this study indicate that Timur Kuran is one of the pioneers of the Critical alternative school which critically studies various concepts of Islamic economics and banking. Various forms of economic thought such as from the concept of Islamic economics are none other than modern doctrines that offer alternatives to the economic systems of Capitalism and Socialism. The emergence of an Islamic economy was none other than political-cultural, with the largest contribution coming from Pakistanis, zakat is a more effective safety net than a secular redistribution system and the waqf system does not yet have the flexibility needed for efficient resource use, and the Qur'an prohibits the practice of usury which involves compounding the debt of a loan that is unable to make payments according to a predetermined schedule.
