The results of the treatment of patients with exogenous and endogenous intoxication by the hemosorption method are presented. The effectiveness and safety of this method of purifying the blood from toxic substances make it possible to expand the indications for its use.

Currently, some progress has been achieved in the treatment of diffuse purulent peritonitis (RP), associated with the use of new antibiotics and their routes of administration (intraperitoneal, endolymphatic, lymphotropic), as well as with the use of hemosorption and plasmapheresis. Nevertheless, mortality in this pathology is still quite high and, according to our observations, is 50-60%, which is largely due to pronounced endogenous intoxication. In turn, the latter leads to inhibition of the absorptive function of the liver and spleen, which may be associated with the functional state of macrophages of these organs and with a deficiency of opsonic factors due to their consumption.

It is defined the differences between the state of non-specific organism’s resistance and the level of endogenous intoxication in patients with generalized periodontitis (GP) and concomitant chlamydia infection. A thorough laboratory examination of 164 patients with GP of I and II stages of chronic course was conducted. To study the protective properties of periodontal tissue, nature of inflammatory response, level of phagocytosis we used the Yasinovskiy’s test (calculated migration of leukocytes in the oral cavity), we determined the concentration of lysozyme and secretory immunoglobulin A levels in oral fluid. In order to determine the overall redox activity of neutrophils we determined the rate of NBT test in peripheral blood and leukocytic intoxication index have been calculated from the results of complete blood count with the help of special formula. Taking into account the results of laboratory examination of patients, we found that the presence of chronic chlamydial infection damages the periodontal tissues greatly. More expressed lesion is confirmed by the increasing number of leukocytes emigration in 1.67 times, desquamation of epithelial cells - 1.25 times), decreased levels of lysozyme in 1.19 times and sIgA - to 1.22 times, the increasing of the level of endogenous intoxication, increasing of the total redox activity of neutrophils compared with those patients without comorbidity.

The preparations of polyhydric alcohols, namely sorbitol-based ones, and their combination with adjuvant therapy, namely L-arginine are sufficiently promising for treatment of purulent-septic complications. The systemic nature of their action makes it appropriate to carry out studies of specific effect on the circulatory system, since their wide homeostatic and pharmacodynamic spectrum is beyond doubt for other systems and organs within the “efficiency-safety” algorithm.The objective of the research was to study the effect of combined use of sorbilact and L-arginine on the cardiovascular system of patients with endogenous intoxication syndrome during the period of developing early secondary autoaggression.Materials and methods. The study included 117 patients who were divided into the following groups: Group I consisted of 31 patients with systemic inflammatory response syndrome; Group II included 27 patients with endogenous intoxication syndrome treated according to 2016 Surviving Sepsis Campaign; Group ІІI comprised 29 patients with endogenous intoxication syndrome who received sorbilact in addition to standard therapy; Group IV included 30 patients with systemic inflammatory response syndrome who received standard therapy as well as sorbilact and L-arginine.Results. Co-administration of sorbilact and L-arginine was accompanied by an increase in stroke volume and cardiac output, activation of left ventricular minute work alongside with a moderate decrease in mean arterial pressure and systemic vascular resistance.Conclusions. The application of sorbilact and L-arginine in patients with endogenous intoxication syndrome provides hemodynamic stability.

Aim. To study the characteristic features of long-term facial phlegmon morbidity in Perm Krai with assessment of the efficiency of infusion therapy using indices of endogenous intoxication expression. Materials and methods. Manifestations of epidemic process of facial phlegmons were studied on the basis of a detailed retrospective analysis of sickness rate among the inhabitants of Perm Krai for the period of 2011–2016. In group 1, all patients besides standard therapy, underwent infusion therapy, and in group 2 – standard therapy alone. Prior to and after the therapy, peripheral blood cellular composition was assessed, and leukocytic indices of intoxication were calculated. Results. A long-term dynamics of facial phlegmon morbidity was established to show the irregularity of process expression according to years. Growth of the quantity of patients is provided by increase in the number of cases with complicated forms. Besides, males play a significant role in formation of sickness rate: the chances for the development of phlegmon among them are 1.49 times higher than in females. In the age structure of morbidity, prevail persons aged 21–30 and 31–50. A half of patients have endogenous intoxication. The course of infusion therapy essentially reduces the expression of endogenous intoxication. Conclusions. There is observed a tendency to growth of facial phlegmon morbidity that is probably connected with changes in etiological structure of pathogenic agents as well as with formation of pathosymbiosis of some opportunistic pathogenic microorganisms. As a rule, phlegmon is being developed against the background of endogenous intoxication, conditioned by both microbial metabolites and tissue destruction products. The efficiency of correction of endogenous intoxication using infusion therapy is shown.

Endogenous intoxication, which accompanies the complicated course of acute appendicitis in children, is one of the serious conditions, which are the main cause of complications in the postoperative period. Diverse clinical observations done by clinicians suggest that despite the use of modern technologies in operating equipment, anesthetic support, the number of complications in the postoperative period has no tendency to decrease, and the main cause of lethal outcomes with appendicular peritonitis is lightning development of severe degree of endotoxicosis, which develops as a result of resorbing toxic substances formed of many components, including pathogenic microflora and destruction metabolism. Purpose. Creation of a mathematical model for prediction of complicated acute appendicitis in children using the functions of regression analysis of EXCEL spreadsheet by approximating experimental data. Materials and methods. In the work, there is information about 59 patients with acute appendicitis treated at the clinic of pediatric surgery at the National Pirogov Memorial Medical University. The data on patients were divided into two groups: patients with not complicated course of pathology – 24 patients, and patients with complicated course of acute appendicitis – 35 children. The control group served the data of 37 healthy children. In each of the three groups, the following parameters were studied: gender, age, bed day, duration of the disease, presence of symptoms of peritoneal irritation, the nature of exudate in the abdominal cavity, the number of red blood cells, hemoglobin, leukocytosis, quantitative characteristics of white blood formula, ESR, cellogenic endogenous intoxication: leukocyte index of intoxication and hematological index of intoxication. All the data were determined directly at the time of hospitalization of a patient to the hospital. Results. Discussion. Based on multiple correlation x = f (x1 , x2 ) a two-factor mathematical model of probability of the complicated course of acute appendicitis in children was created. According to the obtained results of the study of the system of three equations with three unknowns, it was established that if the indicator of the form of the disease is y = 2.67 and above, when, in numerical terms, the result falls into the zone of complicated flow of acute appendicitis and completely coincides with data analysis of the three groups of patients. Conclusions. The components that determine the uncomplicated course of acute appendicitis in children, according to the developed two-factor model, include LII and GPI, which exceed the values intoxication of certain intervals studied at the time of hospitalization. The analyzed indices were determined by the degree of severity of the pathological process, duration of disease, virulence of microflora and age of patients against the background of the formed endogenous intoxication syndrome.
