Currently, some progress has been achieved in the treatment of diffuse purulent peritonitis (RP), associated with the use of new antibiotics and their routes of administration (intraperitoneal, endolymphatic, lymphotropic), as well as with the use of hemosorption and plasmapheresis. Nevertheless, mortality in this pathology is still quite high and, according to our observations, is 50-60%, which is largely due to pronounced endogenous intoxication. In turn, the latter leads to inhibition of the absorptive function of the liver and spleen, which may be associated with the functional state of macrophages of these organs and with a deficiency of opsonic factors due to their consumption.

Inclusion of bioflavonoid quercetin in complex therapy of patientswith acute myocardial infarction provided improvement of hemodynamic parameters and liver function contributed in significant reduction of complications of myocardial infarction. The usage of RNA drug was significantly assotiated with improvement of immune status, reduced symptoms of necrosis-resorptive and immuno-inflammatory syndrome in patients with AMI and liver disfunction.

The Objective was to evaluate the effectiveness of original physiotherapeutic methods in the treatment of patients with acute spread purulent peritonitis.Methods and Materials. We done a treatment approach analysis of 72 patients with peritonitis after programmed sanitation of the abdominal cavity. The control group included 42 patients with standardized treatment regimen. The main group consisted of 35 patients. Their treatment included low-intensity laser radiation of the abdominal cavity, electrical stimulation of the duodenum according to the original methods. We evaluated dynamic changes of the blood parameters, endogenous intoxication markers, the functional status of the liver, kidneys, and intestines.Results. The use of original physiotherapeutic methods in the treatment of patients with acute peritonitis leads to a significant improvement in clinical and laboratory data. The functional status of the liver, kidneys and intestine restored rapidly. Mortality rate decreased from 28.7 to 17.1 % (χ2=1.392, p=0.238), hospital stay decreased by 5.7 hospital days (p<0.05).Conclusion. The treatment regimen for severe peritonitis leads to a relatively rapid relief of the inflammatory process in the abdominal cavity, as well as the restoration of the functional status of the liver, kidneys and intestine. It largely determines the correction of homeostatic indicators, including reducing the severity of endogenous intoxication.

The results of diagnostics and complex treatment of dentoalveolar anomalies during the early shift bite in children living in the city of Krasnodar and the territories of the Krasnodar Territory with different levels of socio-economic development and the effectiveness of the health care system were studied. The prevalence and structure of dentoalveolar anomalies is established, the risk factors for their occurrence and progression are specified. Comprehensive rehabilitation and correction of the functional state of the dentoalveolar apparatus led to a noticeable change in the structure of the RFA in children who turned during the period of an early shift bite.

The aim: To evaluate the effectiveness of the appointment of α- lipoic acid for correcting electroneuromyographyс indicators polyneuropathy of the lower extremities in patients with hemoblastosis. Materials and methods: The study included 77 patients from hematology department of Poltava Regional Hospital, who were divided into 3 groups according to the underlying disease: Group 1 - patients with chronic myeloid leukemia (CML) (n = 26 ) groups and 2 - patients with chronic lymphocytic leukemia (CLL) (n = 27 ), group 3 - patients with multiple myeloma (MM) ( n = 24 ). For the examined patients were taken electroneuromyographyc researches of distal lower extremities by measuring the following parameters: the amplitude of the M-response from short extensors nerves of the fingers ( MEDB) and abductor of the big toe ( MAH) and the rate of excitation the spread of short extensors nerve of the fingers ( VEDB), thumb abductor ( VAH), superficial shallow ( VSF) and calf ( VC) nerves on both sides (sin/dex ) before and after administration of α- lipoic acid. Results: ENMG examination of the peripheral nerves of the lower extremities in patients with CML and CLL revealed a slight decrease in M-responses at normal values of the rate of conduction of excitation through sensory and motor fibers. In patients with MM, multiple lesions of peripheral nerve fibers of the lower extremities were detected, which is represented by a decrease in the M-response and the rate of excitation through sensory fibers and indicates deeper lesions, namely the axial cylinder, which requires longer treatment. The difference in the dynamics of the studied indicators indicates a different degree of PNP. Administration of α-lipoic acid increased the M-response rate and the rate of excitation of motor fibers in all groups of patients. The rate of excitation through sensory fibers was improved in patients with CLL and MM. Following the use of α-lipoic acid in the complex treatment of patients with MM ENMG, the signs of peripheral nerve fiber damage in the lower extremities were worse due to the severe initial condition, but reached a medium level. The positive effect of α-lipoic acid can be caused by a decrease in oxidative stress and endothelial dysfunction in nerve vessels, and, as a consequence, an improvement in the trophism and functional state of nerve fibers. Conclusions: ENMG signs of defeat of PNP of the lower extremities are more expressed at MM. The appointment of α-lipoic acid in the complex treatment of PNP in patients with chronic hemoblastosis improves the functional state of the peripheral nerve fibers of the lower extremities and may be a component of pathogenetic therapy.

Liver damage in diabetes mellitus is of particular interest, since this factor significantly affects the course of the disease, the level of compensation and prognosis of the underlying disease. The aim. To study the effect of complex treatment using ursosan on the functional state of the hepatobiliary system in patients with diabetes mellitus. Materials and methods. The study included 30 patients with type 1 diabetes mellitus and 48 patients with type 2 diabetes. According to the duration of diabetes, patients were divided into three subgroups: a) up to five years; b) 5–10 years; c) more than 10 years. The number of patients was: 1a group – 13, 1b group – 10, 1c group – 7 people; Group 2a – 23, 2b group – 13, 2c group – 10 people. The control group consisted of 23 apparently healthy people. Ursosan was prescribed at a dose of 10-12 mg per 1 kg of body per day for 6 months. Clinical laboratory and instrumental research methods were used to study the functional state of the liver and gallbladder. Research results. After the course of treatment with Ursosan, patients with diabetes mellitus showed normalization of protein, pigment, enzymatic metabolism and, to a lesser extent, lipid metabolism in the liver, which led to an improvement in cellular metabolism and redox processes, providing a stable course of diabetes. Conclusions. A 6-month course of treatment with Ursosan in diabetic patients promotes long-term diabetes compensation. Patients during treatment have a decrease in cytolysis syndrome indicators (alanine aminotransferase (AlAT), aspartate aminotransferase (AsAT), lactate dehydrogenesis (LDH)) by about 1.5 times in all 3 indicators compared to the indicators before treatment. The same trend was observed in terms of gamma-glutamine transferase (GGT) and alkaline phosphatase (ALP)

Objective. To study the lipid peroxidation processes and endogenous intoxication indicators in patients with widespread peritonitis before surgery and during the treatment. Materials and methods. The study included 97 patients (43 men and 54 women) aged from 17 to 69 years with purulent peritonitis. According to treatment scheme, patients were divided into two groups. Patients in group 1 received standard treatment, and in patients in group 2 standard treatment was supplemented by immunoreactions. The concentration of diene conjugates and malonic dialdehyde, reduced glutathione, catalase activity and the concentration of medium molecular weight peptides were determined in the blood of patients and almost healthy people. Results. The concentration of diene conjugates, malonic dialdehyde and medium molecular weight peptides in the blood of patients with advanced peritonitis increased significantly depending on the clinical stage of the disease, while the activity of the enzyme catalase and the concentration of reduced glutathione were decreased. Conclusions. Immunoreactions in combination with standard treatment leads to normalization of lipid peroxidation and reduction of endogenous intoxication on the 5th and 10th days of treatment.

Summary. The problem of treatment of acute purulent paraproctitis is one of the most urgent problems of modern coloproctology. Study objective. To evaluate the effectiveness of the use of Dalmaxin rectal suppositories in the complex treatment of acute paraproctitis. Materials and methods. The effectiveness of use of Dalmaxin rectal suppositories was conducted in 62 patients with acute subcutaneous paraproctitis, assigned to two groups, a main group of 30 patients and a comparison group of 32 patients. All patients underwent radical surgery. Antimicrobial, detoxification and symptomatic therapy was prescribed postoperatively. Local treatment of purulent wounds was carried out using hydrophilic ointments depending on the phase of the wound process; the patients of the main group were additionally administered Dalmaxin rectal suppositories twice a day. The effectiveness of the treatment was evaluated by the severity of the swelling, the nature of the discharge from the wound, the level of microbial contamination, the wound cleansing period, the appearance of granulation tissue and epithelialization, and by cytological examination. Furthermore, general laboratory and biochemical studies were performed. Endogenous intoxication (EI) and its changes in the course of treatment were evaluated by the level of average-mass molecules, leukocyte intoxication index and hematological intoxication index. Statistical processing and comparison of the obtained data was performed using the STATISTICA® 5.5 integrated system (STATSOFT® Inc., USA), license A XX 910A374605FA. Results. The study showed that in patients with acute paraproctitis, the severity of the postoperative period is due to the severe intoxication and the development of a systemic inflammatory response syndrome that were observed in patients at the time of hospitalization. After surgical intervention, clinical and laboratory parameters did not normalize in the first five days of the postoperative period, and only starting from the fifth day, there was a gradual decrease in the level of EI and laboratory indicators, reaching the normal ones only at the time of discharge from the hospital. EI complicates the course of the postoperative period, primarily by weakening the level of humoral immunity and inhibiting the processes of reparative regeneration. When comparing the results of treatment of acute paraproctitis in patients using Dalmaxin rectal suppositories with traditional methods. The hospitalization period of the patients of the main group was also shorter than in the comparison group, and was (9.2 ± 0.5) and (11.9 ± 0.9) bed days, respectively. Conclusions. A comparative evaluation of the use of Dalmaxin in the complex treatment of acute purulent paraproctitis showed that the drug, due to its properties, significantly improves the course of the postoperative period, as evidenced by faster reduction of endogenous intoxication, reduction of microbial contamination and wound cleansing from necrotic tissues, faster granulation and epithelialization, as well as wound healing, thus helping to reduce the time of hospitalization.

The objective: was to increase the effectiveness of treatment of patients with a hyperactive bladder and hypokinetic tone of detrusor (muscle – urine ejector) and combined neurogenic pathology of the distal colon. Materials and methods. Complex diagnostics and conservative treatment of patients with hyperactive bladder and hypokinetic tone of detrusor and combined pathology of the distal colon were performed in 36 (53%) women and 32 (47%) men. The average age of women was 43.4±2.1 years, men – 39.8± .9 years. The duration of the disease in women was 19.7±1.5 months, in men – 22.4±1.8 months. Efficacy was assessed by electromyography. Results. It has been established that electromyography reflects the functional state of the urinary tract and distal parts of the colon at their combined pathology. Conservative treatment of patients of this category with the help of stimulation by electro stimulation was effective. Conclusion. Complex conservative treatment of patients with a hyperactive bladder and hypotonic tonus detrusor and combined pathology of the distal colon is effective, which is confirmed by electromyographic control according to the results of treatment.
