Background: A hamstring strain injury (HSI) has become the most common noncontact injury in soccer. Isokinetic muscle strength deficits are considered a risk factor for HSIs. However, underpowered studies with small sample sizes unable to determine small associations have led to inconclusive results regarding the role of isokinetic strength and strength testing in HSIs. Purpose: To examine whether differences in isokinetic strength measures of knee flexion and extension represent risk factors for hamstring injuries in a large cohort of professional soccer players in an adequately powered study design. Study Design: Cohort study; Level of evidence, 2. Methods: A total of 614 professional soccer players from 14 teams underwent isokinetic strength testing during preseason screening. Testing consisted of concentric knee flexion and extension at 60 deg/s and 300 deg/s and eccentric knee extension at 60 deg/s. A clustered multiple logistic regression analysis was used to identify variables associated with the risk of HSIs. Receiver operating characteristic (ROC) curves were calculated to determine sensitivity and specificity. Results: Of the 614 players, 190 suffered an HSI during the 4 seasons. Quadriceps concentric strength at 60 deg/s (odds ratio [OR], 1.41; 95% CI, 1.03-1.92; P = .03) and hamstring eccentric strength at 60 deg/s (OR, 1.37; 95% CI, 1.01-1.85; P = .04) adjusted for bodyweight were independently associated with the risk of injuries. The absolute differences between the injured and uninjured players were 6.9 N·m and 9.1 N·m, with small effect sizes ( d < 0.2). The ROC analyses showed an area under the curve of 0.54 and 0.56 for quadriceps concentric strength and hamstring eccentric strength, respectively, indicating a failed combined sensitivity and specificity of the 2 strength variables identified in the logistic regression models. Conclusion: This study identified small absolute strength differences and a wide overlap of the absolute strength measurements at the group level. The small associations between lower hamstring eccentric strength and lower quadriceps concentric strength with HSIs can only be considered as weak risk factors. The identification of these risk factors still does not allow the identification of individual players at risk. The use of isokinetic testing to determine the association between strength differences and HSIs is not supported.

To investigate the effect of the years of training on the hamstring-quadriceps isokinetic strength and establish normative data for the cardiorespiratory fitness of first and second division Greek male professional soccer players ( M = 25.1, SD = 5.1 yr.) a representative sample of 72 soccer players was chosen randomly from six teams of these divisions. Hamstring-quadriceps ratios of the preferred and nonpreferred leg correlated with years of training weekly ( r = .51) in the preferred leg. In conclusion, data indicated that the training background of the players (years of training) might have a small effect on the strength balance of quadriceps and hamstrings of the preferred leg.

AbstractStrength imbalances between the hamstrings and quadriceps are an essential predictor for hamstring strain in soccer. The study aimed to investigate and compare the muscle strength imbalances of professional soccer players of different performance levels. One hundred and fifty nine senior male professional soccer players from first (n = 75) and second league (n = 84) Portuguese clubs participated in this study. Muscle strength was evaluated with a REV9000 isokinetic dynamometer. Maximal peak torque data were used to calculate quadriceps and hamstrings strength during concentric and eccentric actions, bilateral asymmetry, conventional strength ratios and dynamic control ratios. Second league athletes produced slightly lower conventional strength ratios in the right and left legs (ES = 0.22, p = 0.17 and ES = 0.36, p = 0.023, respectively) compared to the first league athletes. No significant differences were found in dynamic control ratios or in bilateral asymmetry among first and second league athletes. These findings do not show a clear link between the competitive level and injury risk in soccer players. However, some of the differences found, particularly in conventional strength ratios, highlight the importance of performing off-season and pre-season strength assessments to prescribe and adjust individual strength training programs among professional soccer players.

Isokinetic Strength Responses to Season-long Training and Competition in Turkish Elite Soccer PlayersThere are not enough studies that describe the isokinetic strength of professional soccer players at high angular velocities. The purpose of this study was to evaluate the seasonal changes in isokinetic strength of Turkish professional soccer players (n=14) over the course of a 24-week soccer season. The isokinetic strength of players who underwent usual soccer training and weekly competition throughout the soccer season was assessed by means of the Biodex System 3 dynamometer with the knee attachment. The peak torque of knee extensor and flexor muscles were measured at angular velocities of 60°/s, 300°/s and 500°/s. Players were tested at the beginning and end of the competitive season. While the first- and second-test measurements did not show significant changes at 60°/s and 300°/s angular velocities, at the end of the training period, players' knee strength changed significantly at 500°/s angular velocities. In addition, the H/Q ratio improved significantly for the dominant as well as non-dominant leg at 500°/s. Significant bilateral strength improvements for knee flexors were also observed at 500°/s. The findings of this study suggest that usual daily soccer training (technical, tactical, power, strength, endurance, flexibility, etc.) and weekly competition might produce changes in knee strength at high angular velocities.

Context: Although there are multiple, validated return-to-play programs following hamstring strain injuries, no studies have evaluated their changes in match performance parameters. Objectives: The aim of this study was twofold as follows: (1) to determine the changes in match-based physical performance parameters in professional soccer players before and after sustaining a hamstring strain injury and undergoing a soccer-specific rehabilitation program and (2) to observe the progress of these performance parameters 6 to 10 weeks after the player returned from injury. Design: Prospective, quasi-experimental longitudinal study. Setting: Soccer playing and training grounds. Participants: Nineteen players suffering a hamstring strain injury from 2 male professional teams playing in the Spanish professional football league (La Liga) were followed during the 2015–2016, 2016–2017, and 2017–2018 seasons. Intervention: Participation in on-field training program following a hamstring injury. Main Outcome Measures: Match global positioning system data were collected in the following stages: prior to injury (PRE), after return to play (RTP), program, and 6 to 10 weeks following RTP (C2). Peak velocities and distances ran at sprint velocities showed most likely improvements in C2 versus PRE, and very likely improvements in RTP versus PRE. Results: The distances ran at high and very high intensities, the average velocity, and work-to-rest ratio showed very likely improvements in C2 versus RTP and likely improvements in RTP versus PRE. Likely improvements were observed for all variables in C2 versus RTP. The authors’ results showed an improvement of physical performance during competitive match after RTP, compared with PRE. There was a steady progression in the progress, and in 8 months following RTP, there was no injury reported in the players. Conclusions: The current findings may indicate that the hamstring muscle complex not only recovered completely from the injury but could also withstand a greater training and match load reducing the risk of reinjury.

The aim was to determine if players with a prior hamstring strain injury (HSI) exhibit bilateral deficits in knee flexor eccentric strength and hamstring muscle volume and differences in sprinting performance compared with players without a history of HSIs. Forty-six male professional soccer players participated in this study. Eccentric knee flexor strength, hamstring muscle volume (MRI), and a 20-m running sprint test (5- and 10-m split time) were assessed at the start of the preseason. Eccentric knee strength of the previously injured limbs of injured players was greater (ES: 1.18–1.36) than the uninjured limbs in uninjured players. Previously injured limbs showed possibly larger biceps femoris short heads (BFSh) and likely semitendinosus (ST) muscle volumes than the contralateral uninjured limbs among the injured players (ES: 0.36) and the limbs of the uninjured players (ES: 0.56), respectively. Players who had experienced a previous HSI were possibly slower in the 5-m (small ES: 0.46), while unclear differences were found in both the 10-m and 20-m times. Players with a prior HSI displayed greater eccentric knee flexor strength, possibly relatively hypertrophied ST and BFSh muscles, and possibly reduced 5-m sprinting performances than previously uninjured players. This can have implication for the design of secondary hamstring muscle injury prevention strategies.
