Pathogenic potential of Parabacteroides distasonis revealed in a splenic abscess case: a truth unfolded

Splenic abscess is a rare entity, however if unrecognised or left untreated, it is invariably fatal. We herein report a case of splenic abscess in a 40-year-old man presenting with fever, left-sided abdominal pain, altered sensorium and vomiting. On clinical examination, hepatosplenomegaly was noted and the ultrasound of the abdomen showed multiple hypoechoic regions in the upper pole of spleen, and the diagnosis of splenic abscess was made. The patient received antimicrobial therapy and underwent an open splenectomy with full recovery. Pus aspirated from the splenic abscess grew an unusual organism named Parabacteroides distasonis. In the literature, there are only a few recorded cases of P. distasonis causing splenic abscess. Through this case report, we would like to emphasise the pathogenic role of P. distasonis in causing clinical disease, as this organism is typically known to constitute a part of the normal flora.

Human infectious fungal diseases are increasing, despite improved hygienic conditions. We present a case of gastrointestinal basidiobolomycosis (GIB) in a 20-year-old male with a history of progressively worsening abdominal pain. The causative agent was identified as a novel Basidiobolus species. Validation of its novelty was established by analysis of the partial ribosomal operon of two isolates from different organs. Phylogeny of ITS and LSU rRNA showed that these isolates belonged to the genus Basidiobolus, positioned closely to B. heterosporus and B. minor. Morphological and physiological data supported the identity of the species, which was named Basidiobolus omanensis, with CBS 146281 as the holotype. The strains showed high minimum inhibitory concentrations (MICs) to fluconazole (>64 µg/mL), itraconazole and voriconazole (>16 µg/mL), anidulafungin and micafungin (>16 µg/mL), but had a low MIC to amphotericin B (1 µg/mL). The pathogenic role of B. omanensis in gastrointestinal disease is discussed. We highlight the crucial role of molecular identification of these rarely encountered opportunistic fungi.

Embryonal tumor with abundant neuropil and true rosettes (ETANTR) is rarely seen in the brainstem, and there are few case reports of brainstem ETANTR in the literature. Accordingly, the characteristics and the role of surgical treatment of this rare entity remain unclear. The authors present a case of brainstem ETANTR involving a 33-month-old boy along with a review of the literature and discuss the role of surgical removal in the treatment of this entity. In the authors’ case, the tumor was surgically treated with subtotal resection, which resulted in improvement of the patient’s preoperative symptoms. Chemotherapy was initiated but did not appear to be effective, radiotherapy was declined, and the boy died 6 months after the operation. Based on their analysis of 10 previously reported cases and their own case, the authors conclude that, with respect to survival, surgery may be beneficial even in cases of ETANTR in the brainstem. They note, however, that further studies with a large number of cases are needed to validate the role of surgical treatment in brainstem ETANTR.

Burkholderia pseudomallei, a facultative intracellular Gram-negative bacterium casuses melioidosis, a tropical disease which is more commonly reported these days. Melioidosis can present as visceral abscess, pneumonia, localized ulcers, musculoskeletal and neurological infections, even as full blown sepsis. Melioidosis presenting as parotid abscess is commonly reported among pediatric population, however in adults it is a rare entity with only very few cases being reported. There are very limited studies that have described melioidosis with abscess in unusual sites apart from liver and splenic abscess. Here we present a case of a diabetic male with left sided parotid abscess and on microbiological evaluation found to have melioidosis which was successfully treated with combined surgical and medical treatment.

Pulmonary mucoepidermoid carcinoma (MEC) is a rare malignant neoplasm with the clinical picture mimicking infectious aetiologies in most of the patients. Hence, this rare entity poses a great challenge to the pathologist in terms of diagnosis and to the oncologist in terms of treatment. This case report aims to look at the clinicopathological features of pulmonary MEC, the role of immunohistochemical analysis in diagnosis and choice of chemotherapeutic agent. The objective of reporting this case on MEC is not only the rare frequency of this carcinoma but also to highlight the importance of adequate immunohistochemical analysis in establishing the diagnosis.Key words: Lung cancer, mucoepidermoid carcinoma, chemotherapy

A 23-year-old young man with a transfusion-dependent thalassemia major presented to internal medicine outpatient clinic of our institution. His complaints included fatigue and abdominal pain for 1-2 days. He needed blood transfusions in every two weeks. On admission, he had a fever that not responded to empiric antibiotics. General condition of the patient deteriorated rapidly and he died in intensive care unit of our institution. In conclusion, physicians should aware of serious infectious caused by such microorganisms in patients with iron overload and proper antimicrobial therapy should be initiate at once.

Actinomyces are a part of the normal flora of the cervicofacial region, gastrointestinal tract, and urogenital tract, but can cause infections when the normal mucosal barrier is lost. Herein, we report a rare case of actinomycosis of the gallbladder in a 60-years-old-female. The patient presented with right hypochondrium pain since three months; ultrasonography showed cholelithiasis with thick oedematous wall. An open cholecystectomy was carried out. Histological examination revealed an inflamed gallbladder with colonies of radiating filamentous structures having numerous sulphur granules which on gram staining showed filamentous gram-positive rods. The diagnosis of Actinomycosis of gallbladder was made. After cholecystectomy, prolonged antimicrobial therapy is recommended for patient with actinomycosis to prevent recurrence and even mortality.

Mesenteric tumors are rare and consist of a heterogeneous group of lesions. Among them undifferentiatedcarcinoma of the mesentery is very rare entity. Here we present a case of undifferentiatedcarcinoma of the mesentery in a 51 year old male presented with rapidly increasing hugeabdominal lump,central abdominal pain and weight loss. Ultrasound of the whole abdomen andCT scan of whole abdomen reports were suggestive of retroperitoneal mass. The tumor wasresected and histopathology report was suggestive of malignant mesothelioma whereas immunohistochemistryreport revealed undifferentiated carcinoma of the mesentery.
