ObjectivesReports from national surveys in South Korea and Japan have indicated that the prevalence of periodontal disease is lower in Korea than in Japan. However, these national surveys have not evaluated factors related to periodontal health condition, including diabetes and metabolic syndrome. This study compared periodontal conditions between Korean and Japanese adults, in the context of general health status.DesignCross-sectional study.SettingNational survey in South Korea (Korean National Health and Nutrition Examination Survey, ‘KNHANES’) and a population-based study in Japan (Hisayama study); both were conducted in 2012.ParticipantsThis study included 3574 Korean and 2205 Japanese adults aged 40–79 years.Outcome measuresPeriodontal condition was assessed by using the Community Periodontal Index (CPI). Examiners in Japan underwent clinical calibration training for periodontal examination with a gold-standard examiner from KNHANES, prior to the Hisayama study.ResultsThe age-adjusted prevalences of periodontal disease, defined as CPI score ≥3, were 31.4% and 42.1% in South Korea and Japan, respectively (p<0.001). The age-adjusted prevalences of diabetes (p=0.018) and metabolic syndrome (p=0.001) were higher in Korea than in Japan. The numbers of present and filled teeth and percentages of participants who visited a dental clinic in the last 12 months were higher in Japan than in Korea (all p<0.001). Logistic regression analysis showed that the Japanese participants were more likely to have periodontal disease than were the Korean participants, after adjusting for age, sex, occupation, oral health status, oral health behaviour, diabetes and metabolic syndrome.ConclusionsA higher prevalence of periodontal disease was found in Japanese participants than in Korean participants. Further studies are needed to more clearly elucidate factors underlying the difference in periodontal conditions between the two populations, including those related to the dental healthcare system and dietary intake.

Abstract Background Oral health is a frequently ignored aspect of global health in sub-Saharan patients. Periodontitis, a very frequent oral disease has been proven to be associated to development of the metabolic syndrome. This study aims to evaluate the relationship between periodontal disease and metabolic syndrome components in a sub-Saharan population. Methods We performed a cross sectional study in 3 Yaounde hospitals. Consenting adults aged 21 years and above were recruited. Participants who presented with a tooth loss of at least 50% or any condition which could alter values of biological and periodontal parameters (tobacco smoking, pregnancy, chronic kidney disease, cancer) were excluded. Metabolic syndrome elements (glycaemia, arterial pressure, HDL cholesterol, abdominal circumference, triglycerides) and periodontal variables were recorded (plaque and gingival index of Silness and Loe, periodontal pocket depth and clinical attachment loss). These variables were compared using Fisher’s exact Test and odds ratio calculated with 95% confidence intervals. Results The prevalence of periodontitis and metabolic syndrome were 43.4% and 10.8% respectively. Age (37.75 ± 13.25, P < 0.001) and poor accessory brushing methods were associated risk factors for development of periodontal disease. Sub-Saharan sindividuals with periodontitis had increased odds of having obesity (OR 11.1 [95% CI 3.97–31.03], P < 0.001) and low HDL (OR 4.58 [95% CI 1.79–11.70], P = 0.001) Conclusion Our findings suggest an association between periodontal disease and metabolic syndrome in Sub-Saharan subjects. Increasing age and poor accessory brushing methods are associated risk indicators.

This hospital-based cross-sectional study aimed at determining frequency and risk indicators/predictors of periodontitis in a sample of Egyptian adult population and to develop a prediction equation for classifying periodontal diseases. Seven hundred and fifty subjects were consecutively recruited from outpatient Diagnostic Center, Faculty of Dentistry, Cairo University. Validated oral health questionnaire for adults and oral health impact profile-14 (OHIP-14) questionnaire were filled by all patients. Diagnosis was made based on measurements of clinical periodontal parameters including plaque index, bleeding on probing, pocket depth, clinical attachment level and gingival recession. Radiographic examination was performed using digital periapical radiographs. Ordinal logistic regression analysis was used to determine significant predictors of periodontal diseases and discriminant analysis was performed to predict periodontal disease classification. Gingivitis was the most frequent periodontal disease (39.6%) followed by periodontitis stage I (38%), stage II (20.4%), stage III (1.6%) and stage IV (0.4%). The lowest OHIP-14 scores were in patients with periodontitis stages III and IV. Multivariate analysis showed that education (p < 0.001), OHIP-14 score (p = 0.003), non-smoking (p = 0.001) and non-alcohol drinking (p = 0.021) were significant negative predictors, while never to clean the teeth (p < 0.001) were significant positive predictors of periodontal disease. Periodontitis stages III and IV were the least frequent on a sample of Egyptian adult patients. Education, frequency of teeth cleaning, smoking, alcohol drinking and OHIP-14 scores were significant predictors of periodontal disease. Through discriminant analysis this study could classify patients into different periodontal diseases with an overall correct prediction of 99.2%.
