Identifying opportunities for health promotion and intervention in the ED

BackgroundED staff assess patients with modifiable risk factors for acute and chronic illness. Health promotion interventions delivered in the ED have been advocated for these patients. The engagement of staff is essential to provide effective screening and brief interventions for patients. This survey aimed to assess if staff support the ED as an environment for health promotion.MethodsA multicentre, structured survey was conducted in four EDs in Scotland from 2017 to 2018. Physician and nursing staff at two teaching and two district general hospitals (n=423) were study eligible and offered a multicomponent survey. Outcomes measured included perceived barriers to practice and risk factor specific ED interventions.ResultsOf the 283 respondents, 116 (41%) were physicians and 167 (59%) were nurses. More physicians (86.1%) than nurses (49.7%) reported offering health promotion interventions. Time constraints and a lack of health promotion infrastructure in the ED were cited as challenges to intervention delivery. Staff believed that alcohol (n=170/283, 60.1%) and drug misuse (n=173/283, 61.1%) were more appropriately managed in the ED than primary care. ED staff believed same day brief interventions were more appropriate when alcohol/drug misuse and smoking were directly related to ED presentations.Discussion and conclusionsStaff support the concept of the ED as a potential environment for offering health promotion interventions. ED physicians and nurses have different perspectives on the delivery of health promotion. The role of the ED in health promotion is likely to be multimodal and dependant on the reason for ED attendance.

With respect to harm reduction health promotion interventions, consideration of prior plans is critical in evaluating behavior change. This study addresses prior plans on participation in a singular binge drinking occurrence where some students attempt to drink a fifth (750 ml) of liquor as an unsanctioned component of an annual university event. A pledge campaign with incentives was implemented to reduce participation. A link to an anonymous survey was emailed after the event. The campaign was effective as 90.8% of pledgers did not participate in the event. However, those with prior plans to participate were less likely to engage in the campaign. The association of plan commitment, pledge status and participation status was significant (female: X2(1)=7.95, p=0.005 and males: X2(1)=39.79, p<0.001). Reducing participation for committed students through a pledge campaign alone appears unlikely to change planned behavior. Future research should examine the role of plans and related factors.

AbstractEvidence on the effectiveness of health promotion interventions is mixed, especially in terms of the magnitude of their impact and long-term adherence. This paper proposes a comprehensive approach informed by behavioural economics of developing behavioural change programmes, which is designed to educate, activate, engage and empower people by taking into consideration individual and social mechanisms. Three applied pilots and their results are presented in order to illustrate the approach using cognitive and social mechanisms to lead to better health outcomes, individually and community-wide. More research is needed to explore levers and barriers for the systemic adoption of this framework in implementing health promotion interventions.

Available data indicate that cancer has emerged as an important cause of morbidity and mortality in Ghana. Globally, one of the interventions aimed at disease prevention is through health promotion. To our knowledge, there are no published reports examining the practices of community pharmacists towards cancer health promotion in Ghana. This study was set to examine the perception and perceived barriers of community pharmacists in the provision of cancer health promotion services in Ghana. Methods A cross-sectional study was conducted using electronic questionnaire to assess the perception and perceived barriers of Ghanaian community pharmacists towards provision of cancer health promotion. Key findings The majority of community pharmacists (77.30%) believe that cancer health promotion is an important part of their daily practice. The survey participants were more likely to have a positive perception of the role of the pharmacist if they were older, male, Christian, or had completed the PharmD program ( p < 0.05 for all parameters). Lack of cancer educational materials (69%) was the major perceived barrier in providing cancer health promotion services. Conclusion Ghanaian community pharmacists recognise to play an important role in the provision of cancer health promotion service.

Abstract Background African American adults suffer disproportionately from obesity-related chronic diseases, particularly at younger ages. In order to close the gap in these health disparities, efforts to develop and test culturally appropriate interventions are critical. Methods A PRISMA-guided systematic review was conducted to identify and critically evaluate health promotion interventions for African Americans delivered in barbershops and hair salons. Subject headings and keywords used to search for synonyms of ‘barbershops,’ ‘hair salons,’ and ‘African Americans’ identified all relevant articles (from inception onwards) from six databases: Academic Search Ultimate, Cumulative Index of Nursing and Allied Health Literature (CINAHL), Embase, PsycINFO, PubMed, Web of Science (Science Citation Index and Social Sciences Citation Index). Experimental and quasi-experimental studies for adult (> 18 years) African Americans delivered in barbershops and hair salons that evaluated interventions focused on risk reduction/management of obesity-related chronic disease: cardiovascular disease, cancer, and type 2 diabetes were included. Analyses were conducted in 2020. Results Fourteen studies met criteria for inclusion. Ten studies hosted interventions in a barbershop setting while four took place in hair salons. There was substantial variability among interventions and outcomes with cancer the most commonly studied disease state (n = 7; 50%), followed by hypertension (n = 5; 35.7%). Most reported outcomes were focused on behavior change (n = 10) with only four studies reporting clinical outcomes. Conclusions Health promotion interventions delivered in barbershops/hair salons show promise for meeting cancer screening recommendations and managing hypertension in African Americans. More studies are needed that focus on diabetes and obesity and utilize the hair salon as a site for intervention delivery. Trial registration PROSPERO CRD42020159050.
