Numerous transformations have taken place in the workplace during the past several decades, combining to produce a dramatically different career landscape for individuals, educators, and organizations. Career pathways is a workforce development strategy that can be used to support career development activities and transitions across school and work roles. Adopting a career pathways framework and approach can help guide educational institutions in teaching students competencies that will increase their employability and can also help organizations develop people strategically, build engagement, and improve retention. In this book, a wide variety of critically important career pathway topics are addressed, including the role of career technical education, apprenticeships, and career support in career pathways; proactivity and career crafting; the gig economy and emerging career pathways; the role of data analytics in providing career and workforce insights; and career pathways for late career workers. Background: In-hospital pain services (IPS) are commonplace, but evidence of efficacy is inadequate, and patients’ pain management in any hospital ward remains problematic. This service evaluation aimed to measure the effect of a contemporary IPS, its appropriate use and cost-efficacy. Methods: Records of 249 adults reviewed by the IPS in an inner London Teaching Hospital over an 8-month period were analysed for demographic data, interventions, workload and change in pain intensity measured by numerical rating scale (NRS). Non-parametric tests were used to evaluate differences between initial and final NRS. Spearman’s rank correlation analysis was used to create a correlation matrix to evaluate associations between all identified independent variables with the change in NRS. All strongly correlated variables (ρ > 0.5) were subsequently included in a binary logistic regression analysis to identify predictors of pain resolution greater than 50% NRS and improvement rather than deterioration or no change in NRS. Finally, referral practice and cost of inappropriate referrals were estimated. Referrals were thought to be inappropriate when pain was not optimised by the referring team; they were identified using a set algorithm. Results: Initial median NRS and final median NRS were significantly different when a Wilcoxon signed-rank test was applied to the whole cohort; Z = –5.5 (p = 0.000). Subgroup analysis demonstrated no significant difference in the ‘mild’ pain group; z = –1.1 (p = 0.253). Regression analysis showed that for every unit increase in initial NRS, there was a 62% chance of general and a 33% chance of >50% improvement in final NRS. An estimated annual cost-saving potential of £1546 to £4558 was found in inappropriate referrals and patients experiencing no benefit from the service. Discussion: Results suggest that patients with moderate to severe pain benefit most from IPS input. Also pain management resources are often distributed inefficiently. Future research is required to develop algorithms for easy identification of potential treatment responders.

Chronic nonspecific diarrhea of infancy (CNSD) or irritable bowel syndrome represents one of the most common gastrointestinal problems confronted by practicing pediatricias. In the subspecialty setting of the pediatric gastroenterologist, this entity comprised almost 35% of the outpatient referral practice. CNSD, originally thought to be part of the celiac syndrome, was described as a separate clinical entity by Cohlan in 1956.1 Since that time in the classic paper on this subject, Davidson and Wasserman2 have described consistent diagnostic criteria further characterizing CNSD as a recognizable syndrome. The onset of symptoms occurs classically between 6 and 30 months of age with the development of three to six loose stools with mucous per day, with no associated malabsorption or growth and development abnormalities (to be discussed in detail below). Whereas spontaneous resolution of CNSD is anticipated by 39 months of age, longitudinal observations indicate that these patients have a high incidence of functional bowel complaints during adolescence and beyond suggesting a continuum with "irritable bowel syndrome" of adulthood.3 From the standpoint of the child, this "complaint" all too frequently becomes the "problem" when the frequency and/or consistency of the bowel movements impair training or become intolerable to the parents.

Purpose This paper explores gendered beliefs about the Glass Ceiling (GC) using a new measure, the “Career Pathway Survey” CPS, in an under-researched country, Egypt. Design/methodology/approach Data were collected from 438 employees in Egypt. Participants completed the CPS and other demographic and work measures. The CPS tests four GC beliefs: denial, resilience, acceptance and resignation. Findings Factor analysis validated the configuration of the four factors of the CPS. Descriptive tests showed the female sample with resignation belief, whereas male sample views their females counterparts as resilient. The chi-squared test showed differences in beliefs with different age groups, education level, marital status, number of children, job contracts, job tenure and managerial levels. Moreover, there are evidences of more intensive GC barriers in male-dominant organizations. Originality/value This paper is the first to: re-validate the CPS measure, examine demographic and work factor roles and the CPS, test CPS on a male sample and analyse differences between male and female beliefs according to gender-dominated sectors.

Herein, I intend to capture highlights shared with my academic and research colleagues over the 60 years I devoted initially to my graduate and postdoctoral training and then to academic endeavors starting as an assistant professor in a new medical school at the University of California, San Diego (UCSD). During this period, the Department of Pharmacology emerged from a division within the Department of Medicine to become the first basic science department, solely within the School of Medicine at UCSD in 1979. As part of the school's plans to reorganize and to retain me at UCSD, I was appointed as founding chair. Some years later in 2002, faculty, led largely within the Department of Pharmacology and by practicing pharmacists within UCSD Healthcare, started the independent Skaggs School of Pharmacy and Pharmaceutical Sciences with a doctor of pharmacy (PharmD) program, where I served as the founding dean. My career pathway, from working at my family-owned pharmacy to chairing a department in a school of medicine and then becoming the dean of a school of pharmacy at a research-intensive, student-centered institution, involved some risky decisions. But the academic, curricular, and accreditation challenges posed were met by a cadre of creative faculty colleagues. I offer my experiences to individuals confronted with a multiplicity of real or imagined opportunities in academic health sciences, the related pharmaceutical industry, and government oversight agencies.
