AbstractThis paper is an update on evolving ideas about brain herniations. Following observations on cerebellar pressure coning that raised doubts about its reputed lethal connotations, herniation at the tentorium was re-examined for its role in critically damaging the brain stem. Combining clinical, pathologic, computed tomography and magnetic resonance imaging data, it is concluded that temporal lobe herniation is not the means by which the midbrain sustains irreversible damage in acute cases, but rather lateral displacement of the brain at the tentorium is the prime mover and herniation a harmless accompaniment. Transtentorial herniation has been investigated with computed tomography using the three calcification relationship and descent through the tentorial opening could not be documented. Bilateral brain stem compression in acute bilateral cases must be distinguished from herniation. Upward cerebellar herniation is only the sign of an overfull posterior fossa. Subfalcial herniation is tolerated unless lateral displacement is excessive.

Changes in the nervous system and musculature of normal 34-month-old rats are described. Wallerian degeneration as well as segmental demyelination were observed in the peripheral nervous system, with changes more severe in the sciatic than in the brachial nerves. Signs of nerve-fibre degeneration were also seen in the cord and lower brain stem. The degenerative changes were usually mild, but in a number of animals there was severe degeneration of the gracile tract and lateral columns. Other changes in the nervous system included lipochrome pigment in nerve cells and other cellular elements throughout brain and cord, and eosinophilic bodies in the lower brain stem and cord. In some animals the ventricular system in the brain was dilated. Changes in the skeletal musculature were believed to represent neurogenic muscular atrophy secondary to changes in the nervous system.

Air computed tomography of the cerebellopontine cistern was performed in 15 consecutive patients. The series comprised 7 patients with a normal cistern and 8 patients with tumor, including one with bilateral tumors. Two tumors were of small size (<10 mm i diameter), 3 were medium sized (10–20 mm in diameter), and 4 were large (>20 mm in diameter). In the normal group, the jugular foramen nerve bundle (the 9th, 10th and 11th nerves), the trigeminal nerve, or the brain stem margin were clearly defined by air. In the tumor group, the possible extension of the tumor to these structures could be defined. Differentiation between a compression or attachment of the trigeminal nerve to the tumor was not always possible. The importance of using reconstruction views is stressed.

A 6-year-old Labrador retriever-cross was evaluated for an abnormal gait and head carriage 6 weeks after suffering trauma. The dog was presented with an ambulatory tetraparesis and was reluctant to move his head. Myelography and computed tomography demonstrated a subluxation of the atlanto-occipital joint with compression of the spinomedullary junction and the brain stem by the occipital bone. Removal of the compressive part of the occipital bone resulted in improvement of the clinical signs within 6 weeks, and resolution of clinical signs occurred 8 months after surgery.
