Agent Technology is an emerging and promising research area in software technology, which increasingly contributes to the development of value-added information systems for large healthcare organizations. Through the MediMAS prototype, resulting from a case study conducted at a local Swiss hospital, this paper aims at presenting the advantages of reinforcing such a complex E-health man-machine information organization with software agents. The latter will work on behalf of human agents, taking care of routine tasks, and thus increasing the speed, the systematic, and ultimately the reliability of the information exchanges. We further claim that the modeling of the software agent layer can be methodically derived from the actual “classical” laboratory organization and practices, as well as seamlessly integrated with the existing information system.

Even though information systems is a maturing research area, information systems case study reports generally lack extensive method descriptions, validity defense, and are rarely conducted within a multicase research project. This reduces the ability to build theory in information systems research using case study reports. In this chapter we offer guidelines, examples, and improvements for multicase studies. If information system researchers stick to these guidelines, case study reports and papers will get published more often, improving the rapidly maturing research area of information systems.

Summary Objectives: The aim of this paper is to build a theoretical framework for analysis of when decision-makers should use end-user participation as a form of insurance for unforeseen consequences of implementing information systems in healthcare organizations. Method: Data were collected in a case study of an information system development project in a small clinical setting. During the initial phase, the future end-users of the system were allowed to actively influence the system design and test every new tool that was considered for implementation. Results: The results of the case study suggest that when time and effort are invested in allowing health-care staff to participate in information system development processes, the benefits can well exceed the costs throughout the life cycle of the project. Risk-averse decision-makers fearing negative secondary consequences of a HIS, with regard to clinical work flow, will always adopt measures to prevent future failures, if they can find a possibility of shifting these risks. Therefore, they calculate the present discounted value of the effects accrued over time to the unit and predict the amount of resources they are willing to pay to acquire an insurance (such as design participation) that will protect the organization from future losses. Conclusions: End-user participation in the design process can be the key positive influence on the quality of the service and, thereby, organizational effectiveness. Investments in broad design participation can, consequently, be a productive activity that transforms potential current income into future benefits.

The process of designing a university curriculum in the information systems discipline needs to follow many of the same processes that professional systems analysts use. Of concern are the product, the stakeholders, the drivers, and the methods; indeed, an information systems curriculum is an information system. This chapter presents a case study of one small regional university’s efforts to create an updated information systems curriculum addressing the challenges of curriculum development using the framework of the very systems analysis and design course content that the students are expected to learn. The chapter identifies each component of the information system curriculum and details the processes supporting each development step along the way, from problem identification to system operation and support. This case study presents a cohesive approach to dealing with the many pressures associated with information systems curriculum development and might be instructive for curriculum development in other disciplines as well.

Some health care information systems (HCIS) do succeed, but the majority are likely to fail in some way. To explain why this happens, and how failure rates may be reduced, the chapter describes the “ITPOSMO” model of conception-reality gaps. This argues that the greater the change gap between current realities and the design conceptions (i.e., requirements and assumptions) of a new healthcare information system, the greater the risk of failure. Three archetypal large design-reality gaps affect the HCIS domain and are associated with an increased risk of failure: • Rationality—reality gaps: that arise from the formal, rational way in which many HCIS are conceived, which mismatches the behavioral realities of some healthcare organizations. • Private—public sector gaps: that arise from application in public sector contexts of HCIS developed for the private sector. • Country gaps: that arise from application in one country of HCIS developed in a different country.

The rapid increase in investments for computerised systems is a major concern for all health organisations. Questions about these investments arise as information technologyis only one of the areas that are competing for a finite amount of resources. There is also some concern that some of the failures of information technology would have been detected if proper evaluation of information systems were conducted. The state of the art of evaluating information systems shows changes from a very positivist approach tomore comprehensive approaches that would incorporate multiple methods. This paperpresents an assessment of the techniques and methods for information systems evaluation, followed by an application to a case study in community health to illustrate the value of the contextualist approach to evaluation. The paper argues for the use of longitudinal, contextualist approaches to information systems evaluation if decision-makers seek to improve the situation of information systems in the health industry.

The health care system in Libya in recent years does not change significantly, less developed and modernized. This problem is further exacerbated by the revolution that occurred in Libya. The revolution that occurred in 2011 has caused the health care system into collapse. In this study conducted a review for the development of health information systems in Libya by using OpenMRS Framework.At the stage of gathering information using questionnaires concluded that the majority of respondents (about 95.35%) wanted the development of a new system that can resolve the main problems that occurred in Libya, the health information system ineffective and inefficient in which this system has led to wastage of time and increased operating costs. Based on analysis of system vulnerabilities using PIECES method could also be concluded that the current system is not used effectively and efficiently. In this study is also presented on how to implement OpenMRS.Based on the results of the discussion it can be concluded that OpenMRS is a solution that can be taken for the development of a health information system that is fast, low cost, and is an open source application that only requires a little setting in the system of patient management and care. OpenMRS can be implemented to support the health services of a small clinic to the health services with enterprise scale. Therefore, OpenMRS could be the answer to the development of health information systems in various countries around the world, including Libya.
