Management of severe worn dentition in patients with bruxism is challenging as a result of the loss of tooth structure and occlusal vertical dimension, temporomandibular implications, tooth hypersensitivity, and masticatory or aesthetic impairment. This case describes the 4-year follow-up clinical evaluation of a full mouth tooth-supported rehabilitation made on a 66-year-old man with bruxism and tooth wear, with aesthetic complaints and compromised masticatory function. The prosthetic treatment was planned with a digital smile design and a mock-up technique for an aesthetic and minimally invasive approach using lithium disilicate pressed and layered veneers on anterior teeth, posterior CAD/CAM lithium disilicate overlays with facial coverage, and CAD/CAM monolithic zirconia crowns with facial feldspathic ceramic on maxillary and mandibular canines and first molars in order to ensure the occlusal stability at the increased occlusal vertical dimension. After 4 years of function, no complications were registered. The choice of an appropriate material for the rehabilitation of these patients is essential to improve treatment prognosis and should be guided by mechanical and aesthetical properties. The use of four-point occlusal stabilization with CAD/CAM high strength monolithic zirconia crowns combined with ceramic veneers and overlays appears to be a reliable treatment option that enhances aesthetics and minimizes the occurrence of ceramic fractures, ensuring the treatment prognosis and predictability.

Rehabilitation of worn dentition is a challenging task because several predisposing factors may be included in the pathogenesis. The treatment options and process should be carefully considered based on the clinical condition and patients’ requirements. This case report represents the esthetic rehabilitation of a 68-year-old patient who exhibited severely worn teeth caused by attrition and erosion. After clinical evaluation, 3 mm increase in occlusal vertical dimension was planned with partial lithium-disilicate ceramic restorations as laminate veneers for anterior teeth and vonlays (onlays with veneer extentions) for posterior teeth in maxilla. The adaptation of the patient to the increased occlusal vertical dimension was tested for 1 month by provisional direct composite restorations. As the patient did not show any discomfort, definitive restorations were completed. The patient was satisfied with the esthetics and functionality of her new dentition.

Abstract Background: Providing restorations to anterior teeth in children is a challenging task due to the need for high esthetics, strength, and durability. This study was done to compare prefabricated primary zirconia with resin composite strip crowns on primary maxillary central and lateral incisors with regards to gingival health, plaque accumulation, recurrent caries, restoration failure, and opposing teeth wear over a period of 3, 6 and 12 months. Methods: Children attending the King Abdulaziz University, Faculty of Dentistry (KAUFD) clinics who needed restorations were screened for inclusion criteria. A total of 120 teeth were treated; 60 with zirconia and 60 with strip crowns. Randomization was done by simple random allocation using SPSS software version 20.0 (Armonk, NY; IBM Corp.). A simple descriptive statistic was used for analysis by Wilcoxon Signed-Rank test and Chi-square test. Level of significance was set at (α = 0.05) and level of confidence at (95%). The presented research was registered retrospectively at ClinicalTrials.gov in 6th of August 2017, under registration number NCT03184012.Results: Zirconia crowns showed significantly less gingival bleeding at the 3- and 6-months follow up periods (p<0.006, p<0.001; respectively), less plaque accumulation at all follow up visits (p<0.001), no restoration failure (p<0.001), but more wear to opposing teeth (p<0.02). No significant difference was found between the two crowns with regards to recurrent caries (p<0.135).Conclusion: Based on our data we conclude that overtime teeth covered with zirconia crowns show better gingival health and less bleeding, plaque accumulation as well as less loss of material. On the other hand, zirconia can cause more loss of opposing tooth structure.

New monolithic zirconia materials can be used to fabricate full-contour fixed dental prostheses with the computer-aided design/computer-aided manufacturing (CAD/CAM) method. The aim of this study was to examine the fracture strength and precision of fit of screw-retained monolithic zirconia crowns made directly on implants or by cementing on prefabricated titanium (Ti) bases. Monolithic screw-retained implant crowns (n = 6) were produced by CAD/CAM method using partially (PSZ) and fully stabilized (FSZ) zirconia. Industrially produced zirconia crowns were used as a reference. A lateral incisor study model was made onto an implant replica. Crowns were produced either directly on the implant or through cementing on a prefabricated titanium base (PSZ+Ti, FSZ+Ti). The crowns were tightened to implant replicas with a torque of 35 Ncm. The gap between the replica and the abutment or crown was measured from ×400 scanning electron microscope images for precision of fit. Mechanical testing until failure was completed with a universal testing machine with loading angle of 45°. Statistical analysis was performed (analysis of variance). Mean (±SD) failure loads were 259 ± 23 (PSZ), 140 ± 13 (FSZ), 453 ± 25 (PSZ+Ti), 439 ± 41 (FSZ+Ti), and 290 ± 39 (Procera). Mean (±SD) gap values were 2.2 ± 0.2 (PSZ), 2.5 ± 1.0 (FSZ), 7.0 ± 1.0 (PSZ+Ti), 7.7 ± 1.6 (FSZ+Ti), and 6.7 ± 1.7 (Procera). Monolithic zirconia crowns with a Ti base clearly show higher fracture strengths than the crowns fixed directly on the implant surface. Better marginal fit can be achieved with direct zirconia crowns than with crowns on a titanium base or industrially produced zirconia crowns.

This study was carried on to assess the clinical performance of a novel restorative concept consisting in single monolithic lithium disilicate full-contour crowns bonded on computer-aided design/computer-aided manufacturing (CAD/CAM) zirconia complete-arch implant bridges, to overcome the drawbacks related to the chipping of porcelain fused to zirconia restorations. Sixteen patients received 18 implant-supported hybrid screw-cement-retained complete-arch restorations, consisting of single monolithic lithium disilicate full-contour crowns bonded on CAD/CAM zirconia frameworks. The restorations were supported by 4–8 implants. All patients were followed up for at least 3 years on function (range 36 to 60 months, mean 49.3 months). Clinical controls were scheduled every 4 months. The outcomes were implant and prosthetic survival and success rates, any complications, patient satisfaction, and soft tissue parameters. No dropouts occurred. The overall implant and prosthesis survival rates were 100%. One of 18 restorations (1 of 236 dental units) showed a chip-off fracture of the veneering ceramic that was polished intraorally without any additional treatment, scoring a cumulative prosthetic success rate of 100%, according to the California Dental Association index. All patients were functionally and esthetically highly satisfied with their restorations. Successful soft tissue parameters were found around all implants. Single monolithic lithium disilicate full-contour crowns, bonded on CAD/CAM screw-retained complete-arch zirconia frameworks, showed favorable preliminary outcomes with medium-term follow-up. However, randomized controlled studies of this technique are required for further conclusive recommendations.

Abstract The present study was a prospective, controlled, randomized, clinical short-term trial aiming to evaluate the clinical performance of adhesively luted, lithium disilicate and feldspathic glass-ceramics onlays over a period up to 2 years. A total of 11 patients (7 female, 4 male; age range: 18-60 years, mean age: 39 years) were selected for this study. Each patient received a maximum of two restorations per group in a split-mouth-design. LD: Eleven onlays, performed with lithium disilicate-based ceramic (IPS e.max CAD, Ivoclar Vivadent, Schaan, Liechtenstein), and FP: Eleven onlays, performed with feldspathic ceramic (Vita Mark II, Vita Zanhfabrik, Bad Säckingen). Recalls were performed at 2 weeks (baseline = R1), 1 year (R2) and 2 years (R3) after the cementation by three calibrated blinded independent investigators using mirrors, magnifying eyeglasses, probes and bitewing radiographs. The postoperative sensitivity, secondary caries, marginal integrity, marginal discoloration, color match, surface roughness, tooth integrity, and restoration integrity were evaluated. The Friedman test was used to determine if there was a statistically significant difference in time-to-time comparison of the parameters in the ceramics restorations. A total of 95.4% of the restorations were clinically acceptable at the 2-year recall, without a difference for any evaluation parameter for both ceramic materials. Based on the 2-year data, the CAD-CAM onlays manufactured with feldspathic and lithium-disilicate based ceramics showed similar clinical performance.

Objectives: Marginal and internal fit of restorations are two important clinical factors for assessing the quality and durability of computer-aided design/computer-aided manufacturing (CAD/CAM)-fabricated monolithic zirconia restorations. The purpose of this study was to evaluate the marginal and internal fit of CAD/CAM zirconia crowns with two different scanners (i3D scanner and 3Shape D700). Materials and Methods: Twelve extracted sound human posterior teeth were prepared for full zirconia crowns. Two different extraoral scanners namely i3D scanner and 3Shape D700 were used to digitize type IV gypsum casts poured from impressions. The crowns were milled from presintered monolithic zirconia blocks by a 5-axis milling machine. The replica technique and MIP4 microscopic image analysis software were utilized to measure the marginal and internal fit by a stereomicroscope at ×40 magnification. The collected data were analyzed by paired t-test. Results: The mean marginal gap was 203.62 μm with 3Shape D700 scanner and 241.07 μm with i3D scanner. The mean internal gap was 192.30 μm with 3Shape D700 scanner and 196.06 μm with i3D scanner. The results of paired t-test indicated that there was a statistically significant difference between the two scanners in marginal fit (P=0.04); while, there was no statistically significant difference in internal fit (P=0.761). Conclusion: Within the limitations of this study, the results showed that type of extraoral scanner affected the marginal fit of CAD/CAM fabricated crowns; however, it did not have a significant effect on their internal fit.
