Endogenous intoxication is a complicated multicomponent process caused by pathological biological activity of endogenous products or dysfunction of natural detoxification systems. The article presents the results of the experiment on ischemic-reperfusion syndrome as an aggravating factor in the development of endogenous intoxication in the model of combined injury of the abdominal cavity in the background of hypovolemic shock.The aim of the study – to learn the effect of ischemic-reperfusion syndrome on the changes of indicators of endogenous intoxication in the model of combined injury of the abdominal cavity in the background of hypovolemic shock.Materials and Methods. The experiment was conducted on 80 nonlinear male rats weighing 190–220 grams. In experimental groups ischemic-reperfusion syndrome was simulated in combination with trauma of the abdominal cavity in the background of hypovolemic shock and contents of endogenous intoxication markers (molecules of average mass fractions determined at wavelengths of 238, 254, 260 and 280) and the erythrocytes index of intoxication were analyzed. The probability of differences between the control group and the study group was evaluated using non-parametric Mann-Whitney U-test.Results and Discussion. The applications of haemostatic tourniquets on two lower limbs caused a significant reliable increase of EII indicator in all study groups. The maximum increase of EII indicator was in the group where the applications of the tourniquets were combined with the trauma of the abdominal cavity and hypovolemic shock. These data proved the potentiating effect of ischemicreperfusion syndrome on the development of systemic changes in combined trauma. It has also been shown that the manifestation of endogenous intoxication within the first 24 hours after the applications of haemostatic tourniquets is similar to that arising from the combined injury of the abdominal cavity in the background of massive external bleeding.Conclusions. The applications of haemostatic tourniquets to the two lower limbs for two hours enhance the development of endogenous intoxication in the model of combined injury of the abdominal cavity in the background of hypovolemic shock, that was confirmed by significant increase of EII indicator and the median mass molecules seven days after reperfusion.

The use of hemostatic tourniquet is a proved means of primary care. However, systemic disorders, as well as ultrastructural, in the area of compression can significantly worsen the condition of the injured organism. The aim. Estimation of catalase level in rats’ liver on the background of modifications of ischemic-reperfusion syndrome to know the severest pathogenic combination for organism. Materials and methods. 260 white adult male rats were divided into 5 groups: control (KG), EG1 – simulation of isolated ischemia-reperfusion syndrome (IRS) of the limb, EG2 – simulation of isolated volumetric blood loss, EG3 – combination of IRS of the limb with blood loss, EG4 – simulation of isolated mechanical injury of the thigh, EG5 – combination of IRS of the limb and mechanical injury. The variability of catalase level in liver was analyzed. Results. It was found that each of the experimental interventions has led to changes of catalase activity in the liver. The most expressed pathological expressions were observed on the 3rd after interventions, when the studied index in EG3 was lower than in EG1 and EG2 in 6,2 times and by 33,1 %. On the 7th day catalase activity in EG3 was in 9,4 times and by 44,5 % times lower than in EG1 and in EG2 data concordantly. The combination of limb ischemia-reperfusion with blood loss in EG3 led to exhausting of liver antioxydant enzyme catalase in the most critical posttraumatic period (day 3). The same, but less significant effect was registered in the group of combination of mechanical trauma with ischemia-reperfusion in EG5. This proved the role of the tourniquet as a factor that complicated the course of traumatic disease due to ischemic reperfusion. Conclusions. In this experiment, founded risk factors of combination of ischemia-reperfusion with heavy blood loss emphasized the importance and particular attention on such widespread method of bleeding tratment, as the imposition of a tourniquet, as in our experiment it triggered risk factors of ischemia-reperfusion. It was shown katalase activity depression respectively to the periods of increasing of lipid peroxydation. There was peculiarity, that on the base of isolated IRS catalase activity was increased in 2,5 times comparely to control group, whereas the hardest depression of it was found on the background of IRS, combined with blood loss – catalase activity was lower, comparely to KG – in 2,5 times. The importance of understanding the suppression of hepatocytes’ antyoxydants is great, as it might help in prevention the development of liver failure or hepatorenal syndrome on the background of limb ischemia-reperfusion.

Endogenous intoxication, which accompanies the complicated course of acute appendicitis in children, is one of the serious conditions, which are the main cause of complications in the postoperative period. Diverse clinical observations done by clinicians suggest that despite the use of modern technologies in operating equipment, anesthetic support, the number of complications in the postoperative period has no tendency to decrease, and the main cause of lethal outcomes with appendicular peritonitis is lightning development of severe degree of endotoxicosis, which develops as a result of resorbing toxic substances formed of many components, including pathogenic microflora and destruction metabolism. Purpose. Creation of a mathematical model for prediction of complicated acute appendicitis in children using the functions of regression analysis of EXCEL spreadsheet by approximating experimental data. Materials and methods. In the work, there is information about 59 patients with acute appendicitis treated at the clinic of pediatric surgery at the National Pirogov Memorial Medical University. The data on patients were divided into two groups: patients with not complicated course of pathology – 24 patients, and patients with complicated course of acute appendicitis – 35 children. The control group served the data of 37 healthy children. In each of the three groups, the following parameters were studied: gender, age, bed day, duration of the disease, presence of symptoms of peritoneal irritation, the nature of exudate in the abdominal cavity, the number of red blood cells, hemoglobin, leukocytosis, quantitative characteristics of white blood formula, ESR, cellogenic endogenous intoxication: leukocyte index of intoxication and hematological index of intoxication. All the data were determined directly at the time of hospitalization of a patient to the hospital. Results. Discussion. Based on multiple correlation x = f (x1 , x2 ) a two-factor mathematical model of probability of the complicated course of acute appendicitis in children was created. According to the obtained results of the study of the system of three equations with three unknowns, it was established that if the indicator of the form of the disease is y = 2.67 and above, when, in numerical terms, the result falls into the zone of complicated flow of acute appendicitis and completely coincides with data analysis of the three groups of patients. Conclusions. The components that determine the uncomplicated course of acute appendicitis in children, according to the developed two-factor model, include LII and GPI, which exceed the values intoxication of certain intervals studied at the time of hospitalization. The analyzed indices were determined by the degree of severity of the pathological process, duration of disease, virulence of microflora and age of patients against the background of the formed endogenous intoxication syndrome.

The evaluation of renal measurements such as length, width and thickness, volume is important in the diagnosis and treatment of many renal disorders, since there is a close relationship between the sizes of the kidneys and its function. The purpose of the study was to establish and compare during the postoperative period changes in the mass and sizes of the kidney left after nephrectomy in the sexually mature male rats. An experimental study was carried out on 84 sexually mature white male rats weighing 178-194 grams. Animals were divided into two groups: control (42 rats) and experimental (42 rats). In the control group, the animals under ketamine anesthesia were followed by an abdominal cavity dilution, after which the abdominal wall was applied in a row. All animals of the experimental group performed surgical intervention – nephrectomy of the left kidney. The animals were withdrawn from the trial by intra-pleural administration of thiopental-sodium 50 mg/kg after 7, 14, 21, 30, 90 days after nephrectomy. Macroscopic evaluation and description of the kidneys of animals was performed after their removal. Their weight was determined on the laboratory scale of HLR-200 up to 0.1 mg, and the length, width and thickness of the organ were measured with the help of a caliper to an accuracy of 0.05 mm. Calculated the volume of the kidney. The statistical analysis of the obtained results was carried out using the program STATISTICA 5.5 using parametric methods for evaluating the results. It was established that the mass, length, width, thickness and volume of the kidney of animals in the experimental group, as compared with the control group, were statistically significantly higher in all terms of observation. The fraction of the growth of the width and thickness of the single kidney of animals in the experimental group compared with the control animals in the animals was statistically significantly greater than the proportion of kidney growth during the postoperative period. It was found that the largest increase in the mass and thickness of the kidney, as compared with the control group, was observed after 30 days of the postoperative period, and the greatest increase in the length and width of the kidney, as compared with the control group, was observed after 14 days of the postoperative period.

The aim of the study was to study the possibility of using a hemostatic biological haemostatic to stop bleeding from the parenchymal organs of the abdominal cavity using the surgical method of Damage control. The experiment was performed on laboratory rats that were divided into three groups: a control group, a group where, after injury, the liver wound was sutured with standard sutures and a group where the wound was plastered with a biological hemostatic agent. In order to determine the operational stress, we studied the enzymatic link of antioxidant protection - the level of catalase and superoxide dismutase. To study the indices in all experimental animals, the following tissues were taken: the lower lobe of the right lung, the proportion of the liver where the wounds were applied, the gastrocnemius muscle below the harness and the right kidney. The control points of the study were 1, 3 and 7 days after injury. During working with experimental animals, they adhered to all ethical norms established by international rules. The effect of operational stress on the enzymatic component of antioxidant protection in the tissues of the kidneys, liver, lungs and muscles was studied in the study of the combined trauma of the abdominal cavity complicated by massive bleeding and ischemic-reperfusion syndrome of the extremity. The changes in the indices of superoxide dismutase and catalase were studied. The following result was obtained: tamponing the liver wound with chitosan was a quick and effective means of temporary bleeding stop. At the same time, the duration of the operation was reduced from 19.8 to 12.5 minutes (by 36.8%) (p <0.05). Also, the level of catalase and superoxide dismutase was significantly lower in the group where the chitosan of the liver injury was used in comparison with the study group where standard stitches were applied. The deviations, which are determined in lung, liver, kidney and muscle tissues in RG-1 are evidence of the depth of their lesion and dysfunction, is a favorable factor for the further development of the triggering mechanisms of the systemic response of the body to inflammation and multiple organ failure. In contrast to the RG-2 rapid development of the systemic response was avoided. We can assume that ischemic-reperfusion syndrome is a stimulating factor in the development of systemic changes, although this statement requires further proof. It is also important to further prospective study of this topic in terms of the following questions: what effect has chitosan on liver tissue with prolonged exposure (more than 2 days) and what effect does additional injection of infusion solutions have as an agent for the treatment of hypovolemic shock.

The Objective was to evaluate the effectiveness of original physiotherapeutic methods in the treatment of patients with acute spread purulent peritonitis.Methods and Materials. We done a treatment approach analysis of 72 patients with peritonitis after programmed sanitation of the abdominal cavity. The control group included 42 patients with standardized treatment regimen. The main group consisted of 35 patients. Their treatment included low-intensity laser radiation of the abdominal cavity, electrical stimulation of the duodenum according to the original methods. We evaluated dynamic changes of the blood parameters, endogenous intoxication markers, the functional status of the liver, kidneys, and intestines.Results. The use of original physiotherapeutic methods in the treatment of patients with acute peritonitis leads to a significant improvement in clinical and laboratory data. The functional status of the liver, kidneys and intestine restored rapidly. Mortality rate decreased from 28.7 to 17.1 % (χ2=1.392, p=0.238), hospital stay decreased by 5.7 hospital days (p<0.05).Conclusion. The treatment regimen for severe peritonitis leads to a relatively rapid relief of the inflammatory process in the abdominal cavity, as well as the restoration of the functional status of the liver, kidneys and intestine. It largely determines the correction of homeostatic indicators, including reducing the severity of endogenous intoxication.

Background: Ki-67 is a nuclear protein expressed in all proliferating cells of vertebrates during mitotic cycle phases S, G1, G2, and M, except for G0. Studying this marker is widely used to diagnose the proliferative activity of tumors. However, studying Ki-67 in non-neoplastic diseases attracts much less attention among the researchers. The aim of this study was to assess the possibility of using staining for Ki-67 to identify the proliferative potential of fibroblasts during the formation of adhesions in the abdominal cavity (AC). Methods and Results: Experiments were carried out on male Wistar rats. The adhesion process in AC was simulated in the control group (n=25), and in the experimental group (n=25) with the administration of Seroguard®. Animals were sacrificed on Days 1–30, and the severity of the adhesive process in AC was assessed. Histological sections were prepared and stained for Ki-67. It was found that the animals of the control group had increased severity of the adhesive process in AC during the observation. Maximum increase in severity was registered on Day 30 – 12[9-13] points in the control group and 4[4-4] points in the experimental group (P=0.0079). High proliferative activity of fibroblasts in the control group was detected on Days 3, 7, 14 and 30, which may indicate an active division of fibroblasts and the formation of adhesions in the damaged area. In the experimental group, single Ki-67 positive cells were noted during the entire observation period, which may point to a reduced potential for the formation of adhesions. Conclusion: Our study showed the prospects of using Ki-67 staining to determine the severity of the developing adhesive process in AC, and also revealed one of the possible mechanisms that inhibit the formation of the adhesive process when using Seroguard® – a decrease in the mitotic activity of fibroblasts in the area of peritoneal injury.

The processes of compensatory hypertrophy of the kidney to the period of puberty are insufficiently studied. Changes in renal parameters after contralateral removal are of scientific interest. The purpose of the study was to establish and compare changes in organometric parameters of the kidney that remained after nephrectomy in non-sexually adult male rats and in sexually mature male rats. Experimental study was performed on 84 non-adult white male rats weighing 53-71 grams, which were kept on a standard vivarium diet. All animals were divided into two groups (42 in each): the first - control, the second - the experimental group. In the control group, the abdominal cavity was opened under ketamine anesthesia, after wich the abdominal wall was sutured in layers. All animals of the experimental group performed surgical intervention - nephrectomy of the left kidney. Animals were withdrawn from the trial by intra-pleural administration of thiopental-sodium 50 mg/kg after 7, 14, 21, 30, 90 days after nephrectomy. Macroscopic evaluation and description of the kidneys of animals was performed after their removal. The statistical analysis of the obtained results was carried out using the “STATISTICA 5.5” program, using parametric and non-parametric methods for evaluating the obtained results. It was found that in the immature rats, the weight of animals in the experimental group (after nephrectomy) was statistically significantly lower in all terms of the observation compared to the control group, and the weight of the kidney of animals in the experimental group, on the contrary, was statistically significantly higher in all terms of observation. The length, width, thickness, volume of kidneys of animals of the experimental group, as well as the magnitude of increment of these parameters with each term of observation are determined. The greatest increase in organometric parameters of the kidney was observed after 30 days of the postoperative period (p<0.05). Subsequently, on the 60th and 90th day, the magnitude of the increase in organometric parameters of the kidney significantly decreased. The index of hypertrophy of the kidney remained unpaired by nephrectomy, which varied from 52.24% to 63.21%. The highest rates of hypertrophy were observed between the 21st and 30th days.

AbstractIschemic-reperfusion injury of the small intestine is an acute clinical condition with high mortality rate. This study describes the changes in levels of phenylalanine and tryptophan metabolites in intestinal homogenates and urine samples of Wistar male rats after 60 minutes of mesenteric ischemia and different reperfusion periods (1, 24, 30 hours) in comparison with a control group without the ischemia. The ischemic-reperfusion injury was quantified by the histopathological injury index. The levels of serotonin, epinephrine, and norepinephrine were determined in the intestinal homogenate and epinephrine, vanillylmandelic acid, and the 5-hydroxyindoleacetic acid was analyzed in urine using the HPLC method. The statistically significant increased level of serotonin, epinephrine and norepinephrine were detected in the intestinal samples after 24 hours of reperfusion (p<0.01); even the most elevated serotonin level was observed after one hour of reperfusion (p<0.001). A statistically significant decreased level of vanillylmandelic acid was observed after one hour of reperfusion, but it significantly increased after 24 hours (p<0.05) in urine. The elevated level of the 5-hydroxyindoleacetic acid after one hour and 24 hours after reperfusion (p<0.05) were determined in the urine. The most significant elevated epinephrine level was observed after 24 hours of reperfusion (p<0.001) in urine. Results showed a potential role of serotonin as an early biomarker (after one hour of reperfusion) of small intestinal damaged homogenate, while the best predictor of intestinal injury in urine was epinephrine, which was elevated after 24 hours.
