Instructions for Australian and New Zealand trainees in developing skills in formulation: a systematic review of local evidence

Objective: To examine the peer-reviewed literature on psychiatric formulation. Methods: The term (formula*) was used to systematically search Australasian Psychiatry, Australian and New Zealand Journal of Psychiatry, British Journal of Psychiatry, BJPsych Bulletin, American Journal of Psychiatry and Academic Psychiatry. The resulting papers were reviewed. Results: Of the 42 papers located, 22 (52%) were published between 2002 and 2019; 90% papers were published in Australasian Psychiatry (15), Academic Psychiatry (12) or BJPsych Bulletin (10), journals that focus on training and clinical practice. The papers varied in their aims and recommendations and not all justified the need for formulation. Formulation was recommended as a necessity for training, a communication tool and a guide to treatment. No article provided evidence for the superiority of any type of formulation, and the role of consumers in formulation was conspicuously lacking. Conclusion: There are many ways to structure formulation. However, the existing literature does not support any particular approach. More consideration needs to be given to the needs of consumers in conceptualising and practicing formulation.

The use of instant messaging platforms such as WhatsApp for civic and political purposes has been observed and reported to be growing faster than other social media platforms especially in recent years. Using empirical research on WhatsApp studies published from 2009 to 2019 as its corpus of data, this article systematically reviews them to provide more robust conclusions about WhatsApp and its relationship with political and/or civic engagement. This paper seeks to answer three central questions related to WhatsApp and engagement: 1) What are the motivations in using WhatsApp and how do they manifest in the use of WhatsApp as a communication tool? 2) What is the role of WhatsApp in civic and political engagement? 3) How do researchers study the use of WhatsApp in civic and political engagement? The review finds that across empirical studies, while WhatsApp is used by activists and organisational networks for mobilisation and coordinating actions, it is also used by users who draw on the affordances of the medium for informal and ‘de-politicised’ conversations. The findings contribute to the theorising of social media-mediated movements and activism and highlight methodological gaps of ongoing research on WhatsApp.

Despite the importance placed upon the concept and act of formulation across multiple therapeutic approaches, there is a lack of literature from within the profession of counselling psychology directly on the role, use and practice of formulation, with existing literature predominantly emanating from the related yet distinct therapeutic fields of counselling, psychotherapy, clinical psychology or psychiatry. This, in conjunction with the controversies and lack of consensus on the subject, as well as the demands of the professional and regulatory bodies, have led to this paper. Our aim is to shed light on the role that formulation plays within the profession under the lens of counselling psychology’s philosophical underpinnings. More specifically, this systematic review investigates whether formulation may be considered as fact or opinion; whether formulations across therapeutic approaches may be cohesive or divisive within counselling psychology practice; whether formulations should be undertaken inclusively with clients and other professionals or exclusively by the practitioner. Such exploration uncovers key areas of debate and potential considerations for the profession regarding how formulation is approached, utilised in clinical practice and, taught by training institutions.
