HER-2/Neu Gene in Primary and Local Metastatic Axillary Lymph Nodes in Human Breast Tumors

In order to verify whether the HER-2/neu gene is involved in the initial phases of neoplastic disease or in its progression, we evaluated the amplification and overexpression of this gene in the primary tumor and in synchronous metastatic axillary lymph nodes of 26 women with operable breast cancer. HER-2/neu was amplified in 35% and overexpressed in 33% of the primary sites; similar percentages were found in lymph nodes. The clear correlation between the two disease sites regarding gene, mRNA and protein levels, supports the hypothesis that this gene is involved in the initial and invasive phases of neoplasia. Its actual role with respect to other biological tumor characteristics during the metastatic process should be investigated further.

The aim of the study was to investigate the relationship between the expression of the HER-2 membrane protein and other clinical-pathological parameters such as: histological size of the tumor, degree of the tumor's differentiation, presence of vascular invasion and presence of metastases in regional lymph nodes, in cases of ductal infiltrative breast cancer. We have investigated 56 cases of ductalinfiltrative breast cancer. In all patients a mastectomy with a dissection of axillary lymph nodes has been performed. All tissue samples, taken by biopsy, were embedded in the paraffin, stained by hematoxylin-eosin technique and screened, and evaluation was performed by using a semiquantitative method of the immunohistochemical expression of the HER-2 protein. A decrease of the protein HER-2 expression was noticed in cases of an increase of the tumor's diameter above 50 mm. Increased expression of the HER-2 protein was noticed in cases of moderate (grade II) and poor (grade III) differentiation of carcinoma, as well as in cases where there was no metastases in the regional lymph nodes. No relationship has been observed between the expression of HER-2 and occurrence of vascular invasion. In cases of ductalinfiltrative breast cancer the expression of HER-2 protein is in correlation with the size and degree of tumor's differentiation, as well as with the presence of metastases in regional lymph nodes.

Purpose Immunohistochemical (IHC) staining for cytokeratins (CK) is common practice in evaluating sentinel lymph nodes (SLNs) in patients with breast carcinoma. IHC positivity typically indicates metastasis. SLN procedures are increasingly common in patients with pure intraductal carcinoma (DCIS). Iatrogenic epithelial cell displacement and benign transport of breast epithelial cells into axillary lymph nodes are recently described phenomena. We report 25 cases in which these factors probably resulted in benign epithelial cells in axillary SLNs (ie, false positivity). Methods We reviewed 25 cases of CK-positive SLNs in which the epithelial cells had histologic and IHC characteristics different from their respective patients’ underlying breast carcinomas. Results In all cases, the cytologic features of the epithelial cells in the SLNs were benign, and 22 matched those of corresponding intraductal papillomas that were involved by or were separate from the DCIS in the original cores or surgical biopsies. Fifteen cases were pure DCIS; most invasive tumors were smaller than 1.0 cm. In six carcinomas (DCIS) showing strong Her-2/neu staining, the corresponding epithelial cells in the SLNs were negative. In 13 tumors that were strongly and uniformly positive for estrogen receptor (ER), the cytokeratin-positive cells in the SLNs were negative for ER. Nineteen cases showed benign epithelial cell displacement at the biopsy site. Conclusion Epithelial cells in SLNs may result from transport of displaced cells, usually originating in intraductal papillomas. Positive immunohistochemical results in SLNs should be interpreted with extreme caution to avoid automatically concluding that such cells represent metastasis. Sentinel lymph nodes in breast carcinoma can be falsely positive.

10644 Background: To evaluate the correlations of clinical pathologic status, expression of tumor markers and prognosis in 5000 Chinese breast cancer. Methods: A series primary operable breast cancers from 2002 to 2005 were studied in Tianjin Cancer Hospital. 55% of them are premenopausal and median age is 45 years old; 45% of them are postmenopausal and median age is 58 years old. All of the cases were reviewed by pathologist. ER, PgR, HER-2, p53, PCNA were measured by imunohistochemistry (IHC). Recurrence or metastasis were seen in 126 cases (median follow-up 30 months). Results: Negative axillary lymph nodes in 49.8%,and nodes positive in 50.2% (1 to 3 nodes positive in 28.3%, 4 to 9 nodes positive in 12.1% and over 10 nodes positive in 9.8%). Hermone receptor were observed in 51.7% ER+, 44.7% PgR+, 36.2% ER+/PgR+, 15.5% ER+/PgR−, 8.5% ER−/PgR+, 39.8% ER−/PgR−. HER-2 were observed in 56.8% her-2(-) ,21.7% her-2(+), 10.4% her-2(++) and 11.1% her-2(+++). p53 were observed in 34.4% (+) and PCNA were observed in 85.9% (+). Conclusions: First, Her-2 expression is shown positive correlation with positive axillary lymph nodes, especially remarkable correlated in the number of positive nodes in premenopausal patients. Second, Her-2 expression is shown negative correlation with ER and PgR. Third, Her-2 expression has no correlation with histologic grades of tumors and in some conditions has positive correlation with p53 and PCNA. Fifth, Her-2 expression has no correlation with recurrence, but positive correlation with distant metastasis. In general, Her-2 expression in breast cancer could be taken as the independent index for predicting prognosis. Status of axillary lymph nodes, expression of ER, PgR are the independent factor for expression of her-2. Expression of p53 and PCNA couldn’t be taken as the independent index for predict prognosis.
