Dental Traumas – Things May Not Be as They Seem: A Case Report

This case report describes a 52-year-old female patient who attended a specialist adult dental trauma clinic with a confusing history about a re-implanted avulsed maxillary left central incisor (UL1). Following examination, further investigations and clinical investigations, it was determined that the tooth had been re-implanted in the sub-periosteal space. The upper central incisor was extracted and re-implanted into the correct position and then managed by following the 2020 guidelines of the International Association for Dental Traumatology (IADT). This case highlights an unusual complication of managing avulsed teeth and draws attention to the challenges posed by using two-dimensional radiographic imaging when assessing dental injuries. A six-month follow-up appointment confirmed functional success and a reasonably satisfactory aesthetic outcome for the patient.

Dental trauma is very common, especially in children and adolescents. Tooth avulsion (exarticulation), which is a result of complete rupture of periodontal ligaments is one of the most severe dental injuries. The treatment of choice in avulsion of permanent teeth is reimplantation. Survival of reimplanted teeth depends greatly on the time and conditions in which teeth were stored outside the oral cavity. This report presents a case of 18-year old male with avulsion of central maxillary incisors. The teeth were reimplanted 14 hours after injury. During 8-year follow up no major complications occurred and the teeth still maintain their function, although signs of mild replacement resorption are visible on the follow-up radiographs.

Transmigrated mandibular canines increase treatment complexity in terms of both anchorage and biomechanical planning, posing challenges for both Orthodontists and Oral Surgeons. This case report presents the orthodontic treatment in a 12-year-old girl patient, with transmigrated and impacted mandibular right canine positioned horizontally below the apices of the mandibular incisors. The mandibular deciduous right canine was extracted followed by the traction of the transmigrated permanent canine. Eruption was properly guided, and the correct position of the tooth was orthodontically guided into its normal position in the arch. The patient’s occlusion was significantly improved with good aesthetic outcome, functional occlusion, and a stable result after 1.5 years of follow-up.

Trauma to teeth is one of the most serious dental injuries, and it requires correct and timely management for the favorable prognosis. Splinting of teeth is considered best practice to maintain the repositioned tooth in the correct position, provide patient comfort and improve function. Amongst several methods, splinting with composite and wire is an economical, maintainable and flexible option for stabilizing traumatized mobile teeth. This case report shows splinting of teeth with composite and wire of traumatized teeth in a 24-year-old-male with two years follow up.

External resorption is sequelae of necrotic periodontal membrane over a large area of root following an injury to the tooth. This usually occurs after severe dental injuries such as intrusion, severe luxations or exarticulation injuries complicated by a prolonged extra oral period. This case report presents a clinical and radiographic follow up (13 months) of treatment of inflammatory external root resorption on maxillary central incisor using Vitapex®. Gradual healing of resorption was observed radiographically with no tenderness or pathological mobility.

Most common form of dental trauma in children are the fractures of crown mainly in anterior teeth. A case of complicated crown fracture treated with Cvek pulpotomy using MTA followed by fragment re-attachment with careful follow-up is presented here. Tooth fragment re-attachment provides feasible conservative alternative approach to restore esthetics and tooth function. Successful outcomes have been shown by the long term follow up of the treatment with the preservation of pulp vitality and continued development of root. Clinical results have also shown good esthetics and functional results of presented technique. This paper describes the clinical course of a pediatric patient developing cervical external root resorption (CERR). An 11-year old male patient had sustained dental trauma and was diagnosed with crown fracture affecting the incisal and middle thirds of the maxillary right permanent central incisor and the maxillary right permanent lateral incisor with pulp exposure and CERR after 24 months. Diagnosis and treatment of CERR are a challenge for dental practitioners. In this case, preservation of natural dentition is shown as a successful treatment in a 6-year follow-up.

AbstractInjury to anterior teeth is a relatively common event. Dentists regularly deal with management of dental trauma and restoration of fractured teeth. Hence the treatment approach and medicaments that assures the biologically acceptable healing and improve long term success rate are of potential value and should be considered.A big cystic lesion, which is unable to heal non-surgically, heals well with use of Mineral Trioxide Aggregate (MTA Angelus) by surgical approach. Results of clinical trials have recommended the use of MTA as the most suitable root end filling material.This article presents the case report of management of big cystic lesion along with the lateral root perforation with maxillary left central incisor. Root canal treatment was followed by the resection of root end of tooth in question at the level of perforation and sealed with MTA. 12 month's follow up radiograph showed completely healed Cystic lesion.

BACKGROUND: Root fractures are uncommon injuries in permanent teeth and account for only 0.5–7% of dental trauma. It occurs more frequently in fully erupted permanent teeth, in which the completely formed root with closed apices is solidly supported in the bone and periodontium. This may lead to complex consequences due to the combined damage to the pulp, dentine, cementum, bone, and periodontium. They are transverse to oblique in direction and result from a horizontal impact. Their incidence is more in the middle third of the root than at the cervical and apical thirds. CASE REPORT: This paper describes a case of complicated horizontal root fracture at the middle third of the maxillary right central incisor. After receiving an endodontic treatment, the fractured root fragments of the maxillary right central incisors were united with the help of a glass fibre post. Eventually, the incisor was restored with a zirconia crown. CONCLUSION: Follow-up after a year revealed a well-stabilized assembly of the root fragments and the post.

Avulsion is one of the most serious dental traumatic injuries with a reserved prognosis. This case report describes multiple trauma lesions in permanent central incisors of an eight-year-old girl and a four-year follow-up. The right upper incisor suffered avulsion, remained 16 h extraorally, and was replanted after extraoral endodontic therapy. The left maxillary central incisor suffered a noncomplicated crown fracture with concomitant subluxation. The present case adds to the literature a rare occurrence of success in a severe case with poor prognosis. For this reason, the International Association for Dental Traumatology (IADT) guidelines should be followed and, even in extreme situations, replantation should always be considered.

Traumatic dental injuries are highly prevalent among children. This article describes a case report of a patient who has experienced dental trauma at 8 years old, which has led to enamel-dentine fracture with pulp exposure in the left central incisor and crown-root fracture with pulp exposure in the right central incisor. Partial pulpotomy was performed with the aim of maintaining the neurovascular bundle, thus allowing normal radicular formation. During follow-up 5 years later, teeth were asymptomatic and with no evidence of radiolucent lesions in radiographic examinations. This report demonstrates that traumatic fractures with pulp exposure can be treated effectively by the described technique.
