Background: Achilles tendon (AT) ruptures are devastating injuries that are highly prevalent among athletes. Despite our understanding of the effect of AT rupture and in particular its relationship to basketball, no study has examined the effects of AT rupture and repair on performance metrics in collegiate basketball players. Purpose: To evaluate the effect of AT rupture and subsequent surgical repair on performance metrics in National Collegiate Athletic Association (NCAA) Division I basketball players who return to play after injury. Study Design: Descriptive epidemiology study. Methods: NCAA Division I basketball players who sustained an AT rupture and underwent subsequent surgical repair between 2000 and 2019 were identified by systematically evaluating individual injury reports from databases comprising NCAA career statistics and individual school statistics; 65 male and 41 female players were identified. Athletes were included if they participated in at least one-half of the games of 1 collegiate season before tearing the AT and at least 1 season after operative repair. A total of 50 male and 30 female athletes were included. Each injured athlete was matched to a healthy control by conference, position, starter status at time of injury, class year, and number of games played. Matched controls were healthy players and experienced no significant injuries during their NCAA careers. Results: After AT repair, male athletes had significantly more minutes per game, points per game, and compared with before injury. Total blocks significantly decreased after injury. Female athletes scored significantly more points per game but demonstrated a significantly lower 3-point shooting percentage after return to play. Despite undergoing AT rupture and repair, 14% of male players played in the National Basketball Association, and 20% of injured female athletes played in the Women’s National Basketball Association. Conclusion: After returning to play, men demonstrated a significant drop-off in performance only in regard to total blocks. Female athletes after AT repair demonstrated a significant improvement in points per game but had a significant drop-off in 3-point shooting percentage.

A 21-year-old male baseball catcher sustained an injury to the right testicle from a foul ball that ricocheted from the ground to his groin. The athlete was removed from competition when testicular pain began to increase. Ice was applied to the affected area and nonsteroidal anti-inflammatory drugs (NSAIDs) were utilized through the evening. Further diagnosis and treatment were warranted the following morning. Diagnostic ultrasound revealed a fracture to the right testicle, necessitating surgical repair. The athlete followed a return-to-play progression and returned to play 20 days postinjury. This case describes evaluation and treatment of a rare acute condition and the need for early recognition and diagnosis.

Iron deficiency is the most prevalent nutritional deficiency in the United States. This condition has been reported to affect 60% of female athletes. The Centers for Disease Control and Prevention emphasize screening for anemia in women of childbearing age. The purpose of this study was to determine the number of National Collegiate Athletic Association (NCAA) Division I-A schools that implement screening for iron deficiency in female athletes as well as the screening policies for those who do. A link to an online survey was sent to 94 NCAA Division I-A schools to determine current practices concerning screening and treating female athletes for iron deficiency. There was a 58% response rate. Frequencies for each response were computed. Forty-three percent of responding institutions report screening female athletes for iron deficiency. This study suggests that screening for iron deficiency in female athletes at NCAA Division I-A schools is not a routine procedure and, for those who do screen, variability exists in the criteria for diagnosis, as well as in treatment protocols. Standard protocols for assessment and treatment of iron deficiency in female athletes need to be developed and implemented.

Context Conflict is prevalent between sports medicine professionals and coaching staffs regarding return-to-play decisions for athletes after injury in the National Collegiate Athletic Association (NCAA) Division I setting. The firsthand experiences of athletic trainers (ATs) regarding such conflict have not been fully investigated. Objective To better understand the outside pressures ATs face when making medical decisions regarding patient care and return to play after injury in the NCAA Division I Football Bowl Subdivision (FBS) setting. Design Qualitative study. Setting Semistructured one-on-one telephone interviews. Patients or Other Participants Nine ATs (4 men, 5 women; age = 31 ± 8 years [range = 24–48 years]; years certified = 9 ± 8). Data Collection and Analysis Interviews were audio recorded and later transcribed. Thematic analysis was completed phenomenologically. Researcher triangulation, peer review, and member checks were used to establish trustworthiness. Results Two major themes emerged from the qualitative analysis: (1) pressure is an expected component of the Division I FBS AT role, and (2) strategies can be implemented to mitigate the negative effects of pressure. Three subthemes supported the second major theme: (1) ensuring ongoing and frequent communication with stakeholders about an injured athlete's status and anticipated timeline for return to play, (2) providing a rationale to coaches or administrations to foster an understanding of why specific medical decisions are being made, and (3) establishing positive relationships with coaches, athletes, and administrations. Conclusions External pressure regarding medical decisions was an anticipated occurrence for our sample. Such pressure was described as a natural part of the position, not negative but rather a product of the culture and environment of the Division I FBS setting. Athletic trainers who frequently face pressure from coaches and administration should use the aforementioned strategies to improve the workplace dynamic and foster an environment that focuses on patient-centered care.

Representation of Hispanics/Latinas in intercollegiate athletics is lacking. During the 2014-2015 National Collegiate Athletic Association (NCAA) athletic season, only 2.2 percent of all female Division I student-athlete’s identified as Hispanic or Latina (NCAA, 2015). This low percentage of Hispanic/Latina female participants calls into question how these young women become involved in athletics and sustain their involvement within sport. While previous research has examined the socialization processes of youth athletes and parents of youth athlete participants, there is little research aimed at examining these processes for elite-level athlete participants (Dorsch, Smith, & McDonough, 2015; Greendorfer, Blinde, & Pellegrini, 1986;). Thus, the aim of this current study was to examine the potential factors that may have contributed to consistent sport participation for an elite group of Hispanic/Latina female athletes throughout their youth and collegiate careers. Participants for this study identified as current NCAA Division I Hispanic/Latina female student-athletes. Results showed that family, specifically parents and siblings, contributed to socializing Hispanic/Latina athletes into sport, while family and coaches contributed to the persistence of their athletic endeavors. Findings also show a sense of cultural indifference, youth coaches who invested in the participants long-term, and a significant involvement of the patriarch of the family in their athletic success.

Monitoring external training load (eTL) has become popular for team sport for managing fatigue, optimizing performance, and guiding return-to-play protocols. During indoor sports, eTL can be measured via inertial measurement units (IMU) or indoor positioning systems (IPS). Though each device provides unique information, the relationships between devices has not been examined. Therefore, the purpose of this study was to assess the association of eTL between an IMU and IPS used to monitor eTL in team sport. Retrospective analyses were performed on 13 elite male National Collegiate Athletic Association (NCAA) Division I basketball players (age: 20.2 ± 1.2 years, height: 201.1 ± 7.6 cm, mass: 96.8 ± 8.8 kg) from three practices during the off-season training phase. A one-way analysis of variance was used to test differences in eTL across practices. Pearson’s correlation examined the association between the Distance traveled during practice captured by IPS compared to PlayerLoad (PL), PlayerLoad per Minute (PL/Min), 2-Dimensional PlayerLoad (PL2D), 1-Dimensional PlayerLoad Forward (PL1D-FWD), Side (PL1D-SIDE), and Up (PL1D-UP) captured from the IMU. Regression analyses were performed to predict PL from Distance traveled. The eTL characteristics during Practice 1: PL = 420.4 ± 102.9, PL/min = 5.8 ± 1.4, Distance = 1645.9 ± 377.0 m; Practice 2: PL = 472.8 ± 109.5, PL/min = 5.1 ± 1.2, Distance = 1940.0 ± 436.3 m; Practice 3: PL = 295.1 ± 57.8, PL/min = 5.3 ± 1.0, Distance = 1198.2 ± 219.2 m. Significant (p ≤ 0.05) differences were observed in PL, PL2D, PL1D-FWD, PL1D-SIDE, PL1D-UP, and Distance across practices. Significant correlations (p ≤ 0.001) existed between Distance and PL parameters (Practice 1: r = 0.799–0.891; Practice 2: r = 0.819–0.972; and Practice 3: 0.761–0.891). Predictive models using Distance traveled accounted for 73.5–89.7% of the variance in PL. Significant relationships and predictive capacities exists between systems. Nonetheless, each system also appears to capture unique information that may still be useful to performance practitioners regarding the understanding of eTL.

Objectives: The risk of myocardial damage after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection has been controversial. There is an urgent need for data to support the appropriate level of cardiovascular screening for safe return-to-play. The purpose of this study is to report the incidence of abnormal cardiovascular findings in National Collegiate Athletic Association (NCAA) Division I student-athletes with a history of SARS-CoV-2 infection. Methods: This is a case series of student-athletes at a single NCAA Division I institution who tested positive for SARS-CoV-2 by polymerase chain reaction (PCR) or antibody testing (IgG) from 4/15/2020 to 10/31/2020. From 452 athletes who were screened, 5,124 PCR and 452 antibody tests were completed. Student-athletes were followed through 12/31/2020 (median 104 days, range 64-182 days). Cardiac work-up included clinical evaluation, troponin level, electrocardiogram (ECG), and echocardiogram. Additional work-up was ordered as clinically indicated. Results: 55 student-athletes tested positive for SARS-CoV-2. Of these, 38 (69%) had symptoms of Coronavirus Disease (COVID-19), 14 (26%) had a positive IgG test, and 41 (74%) had a positive PCR test. Eight abnormal cardiovascular screening evaluations necessitated further testing including cardiac magnetic resonance imaging (cMRI). Two athletes received new cardiac diagnoses, one probable early cardiomyopathy and one pericarditis, while the remaining six had normal cardiac MRIs. Conclusion: These data support recent publications which recommend the de-escalation of cardiovascular testing for athletes who have recovered from asymptomatic or mildly symptomatic SARS-CoV-2 infection. Continued follow-up of these athletes for sequelae of SARS-CoV-2 is critical.

The Federal Bureau of Investigation (“FBI”) and U.S. Attorney’s Office’s investigation into National Collegiate Athletic Association (“NCAA”) Division I men’s basketball revealed allegations of NCAA Division I men’s basketball coaches accepting money from sport agents to persuade NCAA Division I men’s basketball players to become clients of said sport agents. This investigation highlights the pervasiveness of violations of preexisting laws governing sport agents, namely, the Uniform Athlete Agents Act (the “UAAA”) and Sport Agent Responsibility and Trust Act (“SPARTA”). Despite the believed routine violation of the UAAA and SPARTA in the recruitment of NCAA student-athletes as clients by sport agents, the laws are rarely used to prosecute sport agents. Thus, the investigation into NCAA Division I men’s basketball highlights the need for new mechanisms to safeguard the rights and interests of NCAA student-athletes related to sport agents. This paper analyzes the efficacy of existing legal and NCAA mechanisms regulating sport agents and presents a thematic analysis of NCAA Division I Football Bowl Subdivision (“FBS”) member institutions’ sport agent policies to highlight the need for and present a model NCAA Division I FBS member institution sport agent policy and education model.

The purpose of this longitudinal, descriptive study was to observe changes in maximal strength measured via isometric clean grip mid-thigh pull and home runs (total and home runs per game) across three years of training and three competitive seasons for four National Collegiate Athletic Association (NCAA) Division 1 baseball players. A one-way repeated measures analysis of variance (ANOVA) was performed, revealing significant univariate effects of time for peak force (PF) (p = 0.003) and peak force allometrically scaled (PFa) (p = 0.002). Increases in PF were noted from season 1 to season 2 (p = 0.031) and season 3 (p = 0.004), but season 2 was not significantly different than season 3 (p = 0.232). Additionally, increases in PFa were noted from season 1 to season 2 (p = 0.010) and season 3 (p < 0.001), but season 2 was not significantly different than season 3 (p = 0.052). Home runs per game rose from the 2009 (0.32) to 2010 season (1.35) and dropped during the 2011 season (1.07). A unique aspect of the study involves 2010 being the season in which ball-bat coefficient of restitution (BBCOR) bats were introduced to the NCAA competition.

Context:  Female athletic trainers (ATs) can face barriers to employment within the profession. Although there is evidence for an increasing percentage of women in athletic training, the portion providing medical care to male sport teams within the professional sport and collegiate settings continues to be small. Objective:  To investigate the experiences of female ATs when seeking employment with male sport teams within the Division I setting. Design:  Qualitative study. Setting:  National Collegiate Athletic Association (NCAA) Division I. Patients or Other Participants:  A total of 15 NCAA Division I female ATs providing medical care to a male sport team participated in our study. Their mean age was 33 ± 9 years, and they had a mean of 11 ± 9 years of overall clinical experience. Data Collection and Analysis:  All participants completed one-on-one phone interviews, which were recorded and transcribed. Analysis of the data followed thematic analysis using a phenomenologic approach. Credibility was established through credibility checks, peer review, and researcher triangulation. Results:  Factors that played a role in women gaining employment with male sport teams were (1) preexisting professional relationships, (2) prior experience with a male sport, and (3) perseverance. Participants in our study were most attracted to their current positions because of (1) the environment of the collegiate setting and (2) the location of the university. Conclusions:  Job access for female ATs in this study was not viewed as a challenge. Familiarity through previous connections with the university and staff and commitment to career goals helped these women obtain the positions they held. The desire to work in male sports was not a primary contributing factor to the decision-making process. Progress continues for women in athletic training, as evidenced by the reported ease of job access with male sport teams.

The National Collegiate Athletic Association (NCAA) clearly states there are many favorable qualities derived from participation that benefit those “who go pro in something other than sports.” However, the ability of collegiate athletics to deliver on the promise of attributable long-term vocational value is rarely questioned. Instead, student-athletes are encouraged to think of their participation as a personal investment with enduring rewards for the investor. In this study involving former NCAA Division I student-athletes, the authors examined whether participation can be regarded as an investment and how student-athletes perceive the returns thus derived. Extending Becker’s (1962) theory of human capital investment to sport participation, the authors probed participants’ experiences for evidence of investment thinking and lasting benefits in corporeal, economic, social, and cultural varieties. The findings support the notion that participation in collegiate athletics can be broadly defined as an investment, but not in accordance with the long-term utility maximizing rationale described by neoclassical economists. Furthermore, the high cost of participation, inherent uncertainty, and unreliable information confound the athlete’s decision-making and blur the distinction between consumption in the present and investing for the future.
