Objective: To assess the validity of the Finnish translation of the 12-item World Health Organization Disability Assessment Schedule (WHODAS 2.0). Design: Cross-sectional cohort survey study. Setting: Physical and Rehabilitation Medicine outpatient university clinic. Subjects: The 501 consecutive patients with chronic musculoskeletal pain. Main measures: Exploratory factor analysis and a graded response model using item response theory analysis were used to assess the constructs and discrimination ability of WHODAS 2.0. Results: The exploratory factor analysis revealed two retained factors with eigenvalues 5.15 and 1.04. Discrimination ability of all items was high or perfect, varying from 1.2 to 2.5. The difficulty levels of seven out of 12 items were shifted towards the elevated disability level. As a result, the entire test characteristic curve showed a shift towards higher levels of disability, placing it at the point of disability level of +1 (where 0 indicates the average level of disability within the sample). Conclusions: The present data indicate that the Finnish translation of the 12-item WHODAS 2.0 is a valid instrument for measuring restrictions of activity and participation among patients with chronic musculoskeletal pain.

Objective: To investigate the factor structure of the 12 item World Health Organization Disability Assessment Schedule (WHODAS) 2.0. Design: Cross-sectional cohort survey study. Setting: Physical and Rehabilitation Medicine outpatient university clinic. Subjects: The 408 consecutive patients with chronic musculoskeletal pain. Main measures: Exploratory and confirmatory factor analysis. Results: A two-factor model most accurately fit the observed data of musculoskeletal pain patients (root mean square error of approximation 0.049, relative Chi square value 1.99). Twelve WHODAS 2.0 items were distributed between two factors with covariance between them of 0.8. The first factor contained domains related mostly to physical functioning, while another was associated mostly with social and cognitive functioning. There were a few differences between single items in their importance in defining the variance within these two factors. Of the six International Classification of Functioning, Disability and Health domains belonging to the first construct, the ability to carry out household responsibilities explained most, 84% of the total variance in this construct. For the second factor, the ability to participate in community activities seemed to be the most important, explaining 85% of the total variance in this construct. Conclusions: In this study, the two-factor structure model of the 12-item WODAS 2.0 demonstrated the most accurate fit within patients with musculoskeletal pain conditions.

Intersectoral approach is essential to develop program for people with disability in Indonesia. Coordination across ministries are necessary to manage this issue. The planning, provision and monitoring of medical and support services as well as program for population groups with disability may require assessment. Purpose of this study is to assess disability in Indonesia. Methods: performing analysis of disability data from 2013 Indonesian Household Health Survey known as Riskesdas. Level of disability obtained from two main population groups: those with and without non communicable disease (NCD). They then divided by age. The World Health Organization Disability Assessment Schedule II (WHODASII) was used to measure disability. Results: contribution of NCD on disability is obvious among older age indicated by higher proportion of disability with NCD. While risk of disability among younger age is unclear, since disability with NCDproportion is lower than disability without NCD. Probably risk of disability among younger age is other than NCD. None of the groups had members with extreme disability on their global WHO-DASII scores. The analysis identifi es target groups for each stakeholder to develop program for people with disability to reach their maximum potential. Abstrak Penanganan penduduk dengan disabilitas membutuhkan keterlibatan lintas kementerian. Diperlukan informasi besar masalah penduduk dengan disabilitas di Indonesia. Analisa bertujuan mengetahui besaran masalah. Metode: analisa deskriptif univariat dan bivariat data disabilitas Riskesdas 2013 untuk memperoleh Informasi disabilitas seluruh penduduk. Selanjutnya dilakukan analisa besaran disabilitas pada penduduk dengan dan tanpa Penyakit Tidak Menular (PTM). Kelompok ini kemudian digolongkan menurut umur. Instrumen The World Health Organization Disability Assessment Schedule II (WHO-DASII) digunakan untuk mengukur disabilitas. Hasil: 18% penduduk Indonesia mengalami disabilitas. Informasi lebih rinci 8,2% mengalami kesulitan ringan, 6,8% kesulitan sedang dan 3% kesulitan berat. Kontribusi PTM terhadap disabilitas terlihat jelas pada kelompok usia 45 tahun atau lebih ditunjukkan dengan lebih tingginya proporsi disabilitas dengan PTM. Sedangkan risiko disabilitas pada kelompok usia sebelum 45 tahun bukan PTM, karena proporsidisabilitas dengan PTM lebih rendah.

Abstract Background: Survival and freedom from disability are arguably the most important patient-centered outcomes after surgery, but it is unclear how postoperative disability should be measured. The authors thus evaluated the World Health Organization Disability Assessment Schedule 2.0 in a surgical population. Methods: The authors examined the psychometric properties of World Health Organization Disability Assessment Schedule 2.0 in a diverse cohort of 510 surgical patients. The authors assessed clinical acceptability, validity, reliability, and responsiveness up to 12 months after surgery. Results: Criterion and convergent validity of World Health Organization Disability Assessment Schedule 2.0 were supported by good correlation with the 40-item quality of recovery scale at 30 days after surgery (r = −0.70) and at 3, 6, and 12 months after surgery with physical functioning (The Katz index of independence in Activities of Daily Living; r = −0.70, r = −0.60, and rho = −0.47); quality of life (EQ-5D; r = −0.57, −0.60, and −0.52); and pain interference scores (modified Brief Pain Inventory Short Form; r = 0.72, 0.74, and 0.81) (all P < 0.0005). Construct validity was supported by increased hospital stay (6.9 vs. 5.3 days, P = 0.008) and increased day 30 complications (20% vs. 11%, P = 0.042) in patients with new disability. There was excellent internal consistency with Cronbach’s α and split-half coefficients greater than 0.90 at all time points (all P < 0.0005). Responsiveness was excellent with effect sizes of 3.4, 3.0, and 1.0 at 3, 6, and 12 months after surgery, respectively. Conclusions: World Health Organization Disability Assessment Schedule 2.0 is a clinically acceptable, valid, reliable, and responsive instrument for measuring postoperative disability in a diverse surgical population. Its use as an endpoint in future perioperative studies can provide outcome data that are meaningful to clinicians and patients alike.
