Using World Health Organization Disability Assessment Schedule 2.0 in people with schizophrenia: a 4-year follow-up

The World Health Organization <em>Suicide trends in at-risk territories</em> study is a multi-site regional research program operating first in French Polynesia and countries of the Western Pacific, then extended to the world. The aims of the study were to establish a monitoring system for suicidal behaviors and to conduct a randomised control trial intervention for non-fatal suicidal behaviors. The latter part is the purpose of the present article. Over the period 2008-2010, 515 patients were admitted at the Emergency Department of the Centre Hospitalier de Polynésie Française for suicidal behavior. Those then hospitalized in the Psychiatry Emergency Unit were asked to be involved in the study and randomly allocated to either Treatment As Usual (TAU) or TAU plus Brief Intervention and Contact (BIC), which provides a psycho-education session and a follow-up of 9 phone contacts over an 18-months period. One hundred persons were assigned to TAU, while 100 participants were allocated to the BIC group. At the end of the follow-up there were no significant differences between the two groups in terms of number of presentations to the hospital for repeated suicidal behaviors. Although the study could not demonstrate the superiority of a treatment over the other, nevertheless – given its importance – the investigation captured public attention and was able to contribute to the awareness of the need of suicide prevention in French Polynesia. The BIC model of intervention seemed to particularly suit the geographical and health care context of the country.

BackgroundDecline in social functioning occurs in individuals who later develop psychosis.AimsTo investigate whether baseline differences in disability are present in those who do and those who do not make a transition to psychosis in a group clinically at high risk and whether disability is a risk factor for transition.MethodProspective multicentre, naturalistic field study with an 18-month follow-up period on 245 help-seeking individuals clinically at high risk. Disability was assessed with the Disability Assessment Schedule of the World Health Organization (WHODAS–II).ResultsAt baseline, the transition group displayed significantly greater difficulties in making new friends (z =−3.40, P = 0.001), maintaining a friendship (z =−3.00, P = 0.003), dealing with people they do not know (z =−2.28, P = 0.023) and joining community activities (z =−2.0, P = 0.05) compared with the non-transition group. In Cox regression, difficulties in getting along with people significantly contributed to the prediction of transition to psychosis in our sample (β = 0.569, s.e. = 0.184, Wald = 9.548, P = 0.002, hazard ratio (HR) = 1.767, 95% CI 1.238–2.550).ConclusionsCertain domains of social disability might contribute to the prediction of psychosis in a sample clinically at high risk.

Intersectoral approach is essential to develop program for people with disability in Indonesia. Coordination across ministries are necessary to manage this issue. The planning, provision and monitoring of medical and support services as well as program for population groups with disability may require assessment. Purpose of this study is to assess disability in Indonesia. Methods: performing analysis of disability data from 2013 Indonesian Household Health Survey known as Riskesdas. Level of disability obtained from two main population groups: those with and without non communicable disease (NCD). They then divided by age. The World Health Organization Disability Assessment Schedule II (WHODASII) was used to measure disability. Results: contribution of NCD on disability is obvious among older age indicated by higher proportion of disability with NCD. While risk of disability among younger age is unclear, since disability with NCDproportion is lower than disability without NCD. Probably risk of disability among younger age is other than NCD. None of the groups had members with extreme disability on their global WHO-DASII scores. The analysis identifi es target groups for each stakeholder to develop program for people with disability to reach their maximum potential. Abstrak Penanganan penduduk dengan disabilitas membutuhkan keterlibatan lintas kementerian. Diperlukan informasi besar masalah penduduk dengan disabilitas di Indonesia. Analisa bertujuan mengetahui besaran masalah. Metode: analisa deskriptif univariat dan bivariat data disabilitas Riskesdas 2013 untuk memperoleh Informasi disabilitas seluruh penduduk. Selanjutnya dilakukan analisa besaran disabilitas pada penduduk dengan dan tanpa Penyakit Tidak Menular (PTM). Kelompok ini kemudian digolongkan menurut umur. Instrumen The World Health Organization Disability Assessment Schedule II (WHO-DASII) digunakan untuk mengukur disabilitas. Hasil: 18% penduduk Indonesia mengalami disabilitas. Informasi lebih rinci 8,2% mengalami kesulitan ringan, 6,8% kesulitan sedang dan 3% kesulitan berat. Kontribusi PTM terhadap disabilitas terlihat jelas pada kelompok usia 45 tahun atau lebih ditunjukkan dengan lebih tingginya proporsi disabilitas dengan PTM. Sedangkan risiko disabilitas pada kelompok usia sebelum 45 tahun bukan PTM, karena proporsidisabilitas dengan PTM lebih rendah.

Abstract Purpose To investigate whether the World Health Organization Safety Surgical Checklist (SSC) is an effective tool to reduce complications in spinal surgery. Methods We retrospectively evaluated the clinical and radiological charts prospectively collected from patients who underwent a spinal surgery procedure from January 2010 to December 2012. The aim of this study was to compare the incidence of complications between two periods, from January to December 2010 (without checklist) and from January 2011 and December 2012 (with checklist), in order to assess the checklist’s effectiveness. Results The sample size was 917 patients with an average of 30-month follow-up. The mean age was 52.88 years. The majority of procedures were performed for oncological diseases (54.4%) and degenerative diseases (39.8%). In total, 159 complications were detected (17.3%). The overall incidence of complications for trauma, infectious pathology, oncology, and degenerative disease was 22.2%, 19.2%, 18.4%, and 15.3%, respectively. No correlation was observed between the type of pathology and the complication incidence. We observed a reduction in the overall incidence of complications following the introduction of the SSC: In 2010 without checklist, the incidence of complications was 24.2%, while in 2011 and 2012, following the checklist introduction, the incidence of complications was 16.7% and 11.7%, respectively (mean 14.2%). Conclusions The SSC seems to be an effective tool to reduce complications in spinal surgery. We propose to extend the use of checklist system also to the preoperative and postoperative phases in order to further reduce the incidence of complications. Abstract Background: Survival and freedom from disability are arguably the most important patient-centered outcomes after surgery, but it is unclear how postoperative disability should be measured. The authors thus evaluated the World Health Organization Disability Assessment Schedule 2.0 in a surgical population. Methods: The authors examined the psychometric properties of World Health Organization Disability Assessment Schedule 2.0 in a diverse cohort of 510 surgical patients. The authors assessed clinical acceptability, validity, reliability, and responsiveness up to 12 months after surgery. Results: Criterion and convergent validity of World Health Organization Disability Assessment Schedule 2.0 were supported by good correlation with the 40-item quality of recovery scale at 30 days after surgery (r = −0.70) and at 3, 6, and 12 months after surgery with physical functioning (The Katz index of independence in Activities of Daily Living; r = −0.70, r = −0.60, and rho = −0.47); quality of life (EQ-5D; r = −0.57, −0.60, and −0.52); and pain interference scores (modified Brief Pain Inventory Short Form; r = 0.72, 0.74, and 0.81) (all P &lt; 0.0005). Construct validity was supported by increased hospital stay (6.9 vs. 5.3 days, P = 0.008) and increased day 30 complications (20% vs. 11%, P = 0.042) in patients with new disability. There was excellent internal consistency with Cronbach’s α and split-half coefficients greater than 0.90 at all time points (all P &lt; 0.0005). Responsiveness was excellent with effect sizes of 3.4, 3.0, and 1.0 at 3, 6, and 12 months after surgery, respectively. Conclusions: World Health Organization Disability Assessment Schedule 2.0 is a clinically acceptable, valid, reliable, and responsive instrument for measuring postoperative disability in a diverse surgical population. Its use as an endpoint in future perioperative studies can provide outcome data that are meaningful to clinicians and patients alike.
