Abstract Purpose The knee stiffness acquired following an Anterior Cruciate Ligament (ACL) injury might affect clinical knee tests, i.e., the pivot-shift maneuver. In contrast, the motor effects of spinal anesthesia could favor the identification of rotatory knee deficiencies prior to ACL reconstruction. Hence, we hypothesized that the intra-operative pivot-shift maneuver under spinal anesthesia generates more acceleration in the lateral tibial plateau of patients with an injured ACL than without. Methods Seventy patients with unilateral and acute ACL rupture (62 men and 8 women, IKDC of 55.1 ± 13.8 pts) were assessed using the pivot-shift maneuver before and after receiving spinal anesthesia. A triaxial accelerometer was attached to the skin between Gerdys’ tubercle and the anterior tuberosity to measure the subluxation and reduction phases. Mixed ANOVA and multiple comparisons were performed considering the anesthesia and leg as factors (alpha = 5%). Results We found a higher acceleration in the injured leg measured under anesthesia compared to without anesthesia (5.12 ± 1.56 m.s− 2 vs. 2.73 ± 1.19 m.s− 2, p < 0.001), and compared to the non-injured leg (5.12 ± 1.56 m.s− 2 vs. 3.45 ± 1.35 m.s− 2, p < 0.001). There was a presence of significant interaction between leg and anesthesia conditions (p < 0.001). Conclusions The pivot-shift maneuver performed under anesthesia identifies better rotatory instability than without anesthesia because testing the pivot-shift without anesthesia underestimates the rotatory subluxation of the knee by an increased knee stiffness. Thus, testing under anesthesia provides a unique opportunity to determine the rotational instability prior to ACL reconstruction.

Although the importance of the anterolateral stabilizing structures of the knee in the setting of anterior cruciate ligament (ACL) injuries has been recognized since many years, most of orthopedic surgeons do not take into consideration the anterolateral structures when performing an ACL reconstruction. Anatomic single or double bundle ACL reconstruction will improve knee stability, but a small subset of patients may experience some residual anteroposterior and rotational instability. For this reason, some researchers have turned again towards the anterolateral aspect of the knee and specifically the anterolateral ligament. The goal of this review is to summarize the existing knowledge regarding the anterolateral ligament of the knee, including anatomy, histology, biomechanics and imaging. In addition, the most common anterolateral reconstruction/tenodesis techniques are described together with their respective clinical outcomes.

AbstractThe rate of anterior cruciate ligament (ACL) retear remains high and revision ACL reconstruction has worse outcomes compared with primaries. To make advances in this area, a strong understanding of influential research is necessary. One method for systematically evaluating the literature is by citation analysis. This article aims to establish and evaluate “classic” articles. With consideration of these articles, this article also aims to evaluate gaps in the field and determine where future research should be directed. The general approach for data collection and analysis consisted of planning objectives, employing a defined strategy, reviewing search results using a multistep and multiauthor approach with specific screening criteria, and analyzing data. The collective number of citations for all publications within the list was 5,203 with an average of 104 citations per publication. “Biomechanical Measures during Landing and Postural Stability Predict Second Anterior Cruciate Ligament Injury after Anterior Cruciate Ligament Reconstruction and Return to Sport” by Paterno et al contained both the highest number of total citations and the highest number of citations per year, with 403 total citations and 43.9 citations per year. The most recurring level of evidence were level II (n = 18) and level III (n = 17). “Clinical Outcomes” was the most common article type (n = 20) followed by “Risk Factors” (n = 10). The American Journal of Sports Medicine had the highest recorded Cite Factor with over 50% of the articles (n = 27) published. The most productive authors included R.W. Wright (n = 6), S.D. Barber-Westin (n = 5), F.R. Noyes (n = 5), and K.P. Spindler (n = 5). Historically, influential studies have been published in the realms of clinical outcome and risk factor identification. It has been established that revision ACL reconstruction has worse outcomes and more high-level studies are needed. Additionally, prospective studies that apply the knowledge for current known risk factor mitigation are needed to determine if graft tear rates can be lowered.

Abstract Objectives To compare the clinical outcomes of isolated anterior cruciate ligament (ACL) reconstruction with combined reconstruction of the ACL and anterolateral ligament (ALL) of the knee. Methods A search was conducted on the PubMed, Medline, Google Scholar, EMBASE, and Cochrane library databases, in line with the PRISMA protocol. The indexation terms used were “anterior cruciate ligament” OR “acl” AND “anterolateral ligament” AND “reconstruction.” Articles that compared patients submitted to combined ACL and ALL reconstruction with those submitted to isolated reconstruction of the ACL, with levels of evidence I, II, and III, were included. Studies with follow-up of less than 2 years and articles that did not use “anatomical” techniques for ALL reconstruction, such as extraarticular tenodesis, were excluded. A meta-analysis with R software was conducted, with a random effects model, presented as risk ratio (RR) or mean difference (MD), with a 95% confidence level (CI) and statistically significant at p < 0.05. Results Ten articles were selected, with a total of 1495 patients, most of whom were men, of whom 674 submitted to ACL and ALL reconstruction and 821 to isolated ACL reconstruction. Combined ACL and ALL reconstruction exhibited a statistically significant advantage in residual pivot shift (RR 0.34, 95% CI 0.24–0.47, I2 = 0%, p < 0.01), rerupture rate (RR 0.34, 95% CI 0.19–0.62, I2 = 0%, p < 0.01), Lachman test (RR 0.59, 95% CI 0.40–0.86, I2 = 21%, p < 0.01), and postoperative Lysholm score (MD 2.28, CI 95% 0.75–3.81, I2 = 73%, p < 0.01). Conclusions Combined ACL and ALL reconstruction obtained better postoperative clinical outcomes when compared with isolated ACL reconstruction, especially in reducing residual pivot shift and rerupture rate.

<p class="abstract"><strong>Background:</strong> <span lang="EN-IN">Arthroscopic ACL reconstruction has become one of the most commonly performed arthroscopic surgeries. Inspite of extensive research on available autograft options, controversy still persists regarding the ideal graft. Allograft tendons usage in orthopedic operations has increased because of its advantages. This study was conducted to assess the functional outcome in patients undergoing ACL reconstruction with soft tissue allografts.</span></p><p class="abstract"><strong>Methods:</strong> <span lang="EN-IN">15 patients with Anterior Cruciate Ligament injury presenting from 2012-2014 who underwent Arthroscopic ACL reconstruction with soft tissue allograft were the subjects of this Prospective study. Assessment of the involved knee was performed to obtain subjective measures of the clinical outcomes of the ACL reconstruction. All patients were followed up at regular intervals of 3 weeks, 6 weeks, 12 weeks, 6 months and 2 years</span>.<strong></strong></p><p class="abstract"><strong>Results:</strong> <span lang="EN-IN">At the final follow up, </span><span lang="EN-IN">the patients had an average Lysholm knee score of 85.60, Tegner score of 7.24 and IKDC score of 85.28. Knee flexion of &gt;120<sup>0</sup> was achieved in &gt;80% of patients and minimal laxity in 60% patients but no functional disability in any of them. No graft failures were noted. </span></p><p class="abstract"><strong>Conclusions:</strong> <span lang="EN-IN">Contrary to the fear of graft failure and complications associated with allograft construct for primary ACL reconstruction, allografts yield the same clinical outcome as autografts with the advantages of reduced operative time, no donor site morbidity, preservation of native hamstring tendons, faster and comfortable rehabilitation. Allografts are a good alternative to autografts for primary ACL reconstruction.</span></p>

Abstract Background: Evaluating the treatment results of Anterior Cruciate Ligament (ACL) reconstruction with the all-inside technique using the TightRope system. Patients and methods: It was a longitudinal descriptive study to investigate 56 patients who underwent ACL reconstruction with an all-inside technique using the Tight-Rope system in Viet Duc University Hospital between June 2019 and March 2020. Result: 82% of patients were men, and mainly of working age. The primary cause of ACL injury was a sports injury. Six months after surgery, the outcomes were excellent. The patient's knee joint range of motion and stability (evaluated by the Lachman and Pivot-Shift test) has improved significantly. Conclusions: Arthrosopic ACL reconstruction with all-inside technique using TightRope system leads to significant improvement in range of motion of patient's knee joint. The result of this study helps surgeons choose more options for management of ACL injury. Key word: Knee arthroscopy, anterior cruciate ligament reconstruction, all-inside. Tóm tắt Mục tiêu: Nghiên cứu đánh giá kết quả điều trị của phẫu thuật tái tạo dây chằng chéo trước (DCCT) theo kĩ thuât tái tạo "all inside" (tất cả bên trong) sử dụng Tightrope (TGR) hai đầu. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả tiến cứu trên 56 người bệnh (NB) phẫu thuật nội soi tái tạo DCCT với kỹ thuật tất cả bên trong sử dụng Tightrope hai đầu tại bệnh viện Hữu Nghị Việt Đức từ tháng 6 năm 2019 đến tháng 3 năm 2020. Kết quả: NB chủ yếu là nam giới (82%), trong độ tuổi lao động, nguyên nhân chủ yếu do tai nạn thể thao. Kết quả sau mổ 6 tháng rất tốt, NB cải thiện biên độ vận động và độ vững của khớp gối rõ rệt qua đánh giá bằng dấu hiệu Lachman và nghiệm pháp Pivot-shift. Kết luận: Nội soi tái tạo DCCT bằng kỹ thuật "tất cả bên trong" sử dụng TGR hai đầu đạt kết quả cao về sự phục hồi biên độ vận động khớp gối của NB sau mổ, giúp cho phẫu thuật viên có thêm lựa chọn về phương pháp phẫu thuật trong điều trị tái tạo DCCT. Background: Biomechanical studies on anterior cruciate ligament (ACL) injuries and reconstructions are based on ACL transection instead of realistic injury trauma. Purpose: To replicate an ACL injury in vitro and compare the laxity that occurs with that after an isolated ACL transection injury before and after ACL reconstruction. Study Design: Controlled laboratory study. Methods: Nine paired knees were ACL injured or ACL transected. For ACL injury, knees were mounted in a rig that imposed tibial anterior translation at 1000 mm/min to rupture the ACL at 22.5° of flexion, 5° of internal rotation, and 710 N of joint compressive force, replicating data published on clinical bone bruise locations. In contralateral knees, the ACL was transected arthroscopically at midsubstance. Both groups had ACL reconstruction with bone–patellar tendon–bone graft. Native, ACL-deficient, and reconstructed knee laxities were measured in a kinematics rig from 0° to 100° of flexion with optical tracking: anterior tibial translation (ATT), internal rotation (IR), anterolateral (ATT + IR), and pivot shift (IR + valgus). Results: The ACL ruptured at 26 ± 5 mm of ATT and 1550 ± 620 N of force (mean ± SD) with an audible spring-back tibiofemoral impact with 5o of valgus. ACL injury and transection increased ATT ( P < .001). ACL injury caused greater ATT than ACL transection by 1.4 mm (range, 0.4-2.2 mm; P = .033). IR increased significantly in ACL-injured knees between 0° and 30° of flexion and in ACL transection knees from 0° to 20° of flexion. ATT during the ATT + IR maneuver was increased by ACL injury between 0° and 80° and after ACL transection between 0° and 60°. Residual laxity persisted after ACL reconstruction from 0° to 40° after ACL injury and from 0° to 20° in the ACL transection knees. ACL deficiency increased ATT and IR in the pivot-shift test ( P < .001). The ATT in the pivot-shift increased significantly at 0° to 20° after ACL transection and 0° to 50° after ACL injury, and this persisted across 0° to 20° and 0° to 40° after ACL reconstruction. Conclusion: This study developed an ACL injury model in vitro that replicated clinical ACL injury as evidenced by bone bruise patterns. ACL injury caused larger increases of laxity than ACL transection, likely because of damage to adjacent tissues; these differences often persisted after ACL reconstruction. Clinical Relevance: This in vitro model created more realistic ACL injuries than surgical transection, facilitating future evaluation of ACL reconstruction techniques.

Abstract Introduction: A few years ago, arthroscopic became one of the most common treatments for anterior cruciate ligament (ACL) injury. Patients who are older than 50 years of age have problems of degenerative knee, ACL reconstruction indication in this group has many different perspectives. Materials and Methods: Retrospective and prospective descriptive study was conduted in the patients group over 50 years of age having ACL reconstruction in Viet Duc University Hospital from April 2017 to November 2018. Follow-up examination was in one, three and six months. Results: After 1 month, 96,2% of the patients had movement amplitude over 90 degrees, there were no cases of swelling knee, knee joint effusion or infection of the incision. After 3 months of surgery, all patients could flex their knee joints over 120 degrees, 93,2% of patients had negative the Lachmann sign, 91% of them had negative anterior draw test and negative Pivot shift sign ratio was 97,7%. Re-examination after 6 months of surgery showed that 83,9% patients had good or very good the Lysholm score, amplitude of joint movement means was 147 ± 11,8 degree. Conclusion: Our results suggest that individuals 50 years and older who have symptomatic instability from an absent or insufficient ACL can have good outcomes with surgical reconstruction if there are no contraindications. Tóm tắt Đặt vấn đề: Trong những năm gần đây, phẫu thuật nội soi đã trở thành phương pháp thường quy để điều trị đứt dây chằng chéo trước (DCCT) khớp gối cho người bệnh dưới 50 tuổi, có nhu cầu hoạt động thể lực. Tuy nhiên tổn thương dây chằng chéo trước (DCCT) ở lứa tuổi trên 50 thường kèm theo bệnh lý thoái hóa khớp gối, vì vậy chỉ định phẫu thuật nội soi tái tạo DCCT ở lứa tuổi này còn nhiều quan điểm khác nhau. Phương pháp nghiên cứu: Nghiên cứu mô tả hồi cứu kết hợp tiến cứu được tiến hành trên 56 người bệnh trên 50 tuổi, được phẫu thuật nội soi tái tạo DCCT tại Bệnh viện Hữu nghị Việt Đức từ tháng 4/2017 đến tháng 11/2018. 100% người bệnh được tái khám, đánh giá kết quả sau phẫu thuật 1, 3, 6 tháng. Kết quả: Sau 1 tháng phẫu thuật, 96,2% người bệnh có biên độ vận động > 900, không có trường hợp nào nhiễm trùng vết mổ. Sau phẫu thuật 3 tháng, tất cả người bệnh có biên độ gấp gối đạt trên 1200, duỗi hết gối, 93,2% người bệnh có dấu hiệu Lachmann âm tính, 91% có dấu hiệu rút ngăn kéo trước âm tính, tỷ lệ âm tính với dấu hiệu Pivot shift là 97,7%. Đánh giá lại sau 6 tháng điều trị có 83,9% người bệnh có điểm Lysholm ở mức tốt hoặc rất tốt, biên độ vận động khớp gối trung bình là 147 ± 11,8 độ. Kết luận: Kết quả của chúng tôi cho thấy rằng những người bệnh từ 50 tuổi trở lên có triệu chứng lâm sàng do tổn thương ACL có thể cho kết quả tốt với phẫu thuật nội soi tái tạo nếu không có chông chỉ định.
