Validation of a new prognostic body composition parameter in cancer patients.

11597 Background: In cancer patients protein-calorie imbalances are responsible for decreased lean body mass and, in turn, for worse clinical outcome. We evaluated the prognostic value of a new body composition parameter (creatinine height index [CHI]) obtained from bioimpedance vectorial analysis-derived body cell mass and its association with nutritional and functional status. Methods: Data from Italian and German cancer patients based on information from previous prospective cohort studies were used. Multivariable models (adjusted for age, gender, hydration status, performance status, and disease stage) were built in both cohorts to assess the association between body composition outcome parameters (low fat-free mass [FFM], <15 [females] and <17 [males] kg/m2; low standardized phase angle [SPA], <-1.65; low CHI, <510 [females] and <660 [males] mg/24h/m) and 1-year all-cause mortality, low body mass index (BMI; <20 [<70 years] and <22 [>/=70 years] kg/m2), clinically significant weight loss (WL; >/=10% in 6 months) and low handgrip strength (HG; <20 [females] and <30 [males] kg). Results: Overall, 1084 cancer patients were included (Italians, N=454; Germans, N=630). Low CHI was independently associated with mortality in both Italian and German cohorts (Table). Low FFMI and low SPA did not predict survival in the German cohort. In patients with low CHI, worse nutritional and functional status were observed in both study populations. Performance of models addressing the study endpoints showed substantial consistency with both cohorts, particularly of those including low CHI. Conclusions: We validated a new prognostic body composition parameter, which is easier to interpret than standard nutritional parameters and may be useful for identifying cancer patients at nutritional risk, requiring early nutritional support. Background: To what extent pre-season training camp may impact body composition and metabolism in collegiate football players is unknown. Objective: The purpose of this study was to assess changes in body composition, dietary habits and metabolism following pre-season training in Division III American football players. Methods: Seventeen Division III football players (Ht: 1.80±0.6 m; BM: 99.1±60.1 kg; FFM: 79.7±8.6 kg; BF%: 19.3±8.6%) had their body composition and resting energy expenditure (REE) assessed in a fasted state (>12 hr.) before and upon completion of pre-season training. Pre-season training consisted of 14 days of intense training. Results: Linemen had a higher body mass, fat-free mass (FFM), and fat mass likely contributing to the higher REE (p < 0.01). A main effect for time was observed regarding changes in FFM (p<0.001) and body fat % (p = 0.024). A significant interaction was observed for FFM with linemen experiencing a greater reduction in FFM (-1.73±0.37 vs. -0.43±0.74 kg; p<0.001). Linemen (L) experienced a greater reduction in REE compared to non-linemen (NL) (L: -223.0±308.4 vs. NL: 3.27±200.1 kcals; p=0.085) albeit not statistically significant. Non-linemen consumed a higher amount of daily calories (p=0.036), carbohydrates (p=0.046), and protein (p=0.024) when expressed relative to body mass. Conclusion: The greater size in linemen prior to pre-season likely contributed to their higher REE. However, the multiple training bouts appeared to reduce REE in linemen, which may have been driven by the observed losses in FFM and low protein intake. Further, pre-season training increased body fat % in all players.

Using a crossover design, we explored the effects of both short- and long-term time-restricted feeding (TRF) vs. regular diet on Wingate (WnT) performance and body composition in well-trained young men. Twelve healthy male physical education students were included (age: 22.4 ± 2.8 years, height: 174.0 ± 7.1 cm, body mass: 73.6 ± 9.5 kg, body mass index: 24.2 ± 2.0 kg/m2). The order of dieting was randomized and counterbalanced, and all participants served as their own controls. TRF was limited to an 8-h eating window and non-TRF involved a customary meal pattern. Participants performed WnT tests and body composition scans at baseline, post-one and post-four weeks of the assigned diet. Before testing, participants were asked to fill out a dietary record over four consecutive days and were instructed to continue their habitual training throughout the study. Energy intake and macronutrient distribution were similar at baseline in both conditions. WnT mean power and total work output increased post-four weeks of TRF. Both conditions were similarly effective in increasing fat-free mass after four weeks of intervention. However, there was no correlation between change in fat-free mass and WnT mean power after TRF. TRF did not elicit any changes in WnT performance or body composition one week post-intervention. Thus, long-term TRF can be used in combination with regular training to improve supramaximal exercise performance in well-trained men.

AbstractPrevious studies on the association between thyroid function and body composition are conflicting and showed strong differences across age groups. Our aim was to clarify age-specific associations of serum thyroid-stimulating hormone (TSH) levels with markers of body composition including body mass index (BMI), waist circumference, fat mass (FM), fat-free mass (FFM) and body cell mass (BCM). We used data from two independent population-based cohorts within the framework of the Study of Health in Pomerania. The study population included 5656 individuals aged 20 to 90 years. Markers of body composition were measured by bioelectrical impedance analysis. Serum TSH levels were significantly positively associated with BMI (β = 0.16; 95% confidence interval [CI]: 0.06 to 0.27), waist circumference (β = 0.35; 95% CI: 0.08 to 0.62) and FM (β = 0.32; 95% CI: 0.12 to 0.52), but not with FFM and BCM. Interaction analysis revealed positive associations of serum TSH levels with BMI, waist circumference, FM, FFM and BCM in individuals older than 60 years, while no such associations were observed in younger individuals. We demonstrated that lower serum TSH levels were accompanied with lower values of BMI, waist circumference, FM, FFM, and BCM in the elderly, while no such associations were observed in younger individuals.

Abstract Background To achieve ideal strength/power to mass ratio, athletes may attempt to lower body mass through reductions in fat mass (FM), while maintaining or increasing fat-free mass (FFM) by manipulating their training regimens and diets. Emerging evidence suggests that consumption of high-fat, ketogenic diets (KD) may be advantageous for reducing body mass and FM, while retaining FFM. Methods A systematic review of the literature was conducted using PubMed and Cochrane Library databases to compare the effects of KD versus control diets (CON) on body mass and composition in physically active populations. Randomized and non-randomized studies were included if participants were healthy (free of chronic disease), physically active men or women age ≥ 18 years consuming KD (< 50 g carbohydrate/d or serum or whole blood β-hydroxybutyrate (βhb) > 0.5 mmol/L) for ≥14 days. Results Thirteen studies (9 parallel and 4 crossover/longitudinal) that met the inclusion criteria were identified. Aggregated results from the 13 identified studies show body mass decreased 2.7 kg in KD and increased 0.3 kg in CON. FM decreased by 2.3 kg in KD and 0.3 kg in CON. FFM decreased by 0.3 kg in KD and increased 0.7 kg in CON. Estimated energy balance based on changes in body composition was − 339 kcal/d in KD and 5 kcal/d in CON. Risk of bias identified some concern of bias primarily due to studies which allowed participants to self-select diet intervention groups, as well as inability to blind participants to the study intervention, and/or longitudinal study design. Conclusion KD can promote mobilization of fat stores to reduce FM while retaining FFM. However, there is variance in results of FFM across studies and some risk-of-bias in the current literature that is discussed in this systematic review.

The purpose of this study was to assess the body composition of male and female basketball athletes (n = 323) across season, year, and sport-position using air displacement plethysmography. An independent sample t-test assessed sport-position differences. An analysis of variance was used to assess within-subjects across season (pre-season, in-season, and off-season), and academic year (freshman, sophomore, and junior). For both men and women basketball (MBB, WBB) athletes, guards had the lowest body fat, fat mass, fat free mass, and body mass. No seasonal differences were observed in MBB, but following in-season play for WBB, a reduction of (p = 0.03) in fat free mass (FFM) was observed. Across years, MBB showed an increase in FFM from freshman to sophomore year, yet remained unchanged through junior year. For WBB across years, no differences occurred for body mass (BM), body fat (BF%), and fat mass (FM), yet FFM increased from sophomore to junior year (p = 0.009). Sport-position differences exist in MBB and WBB: Guards were found to be smaller and leaner than forwards. Due to the importance of body composition (BC) on athletic performance, along with seasonal and longitudinal shifts in BC, strength and conditioning practitioners should periodically assess athletes BC to ensure preservation of FFM. Training and nutrition programming can then be adjusted in response to changes in BC.
