ABSTRAK Rumah sakit adalah institusi penyedia jasa layanan kesehatan, Rumah Sakit Umum Daerah Dr. R. Sosodoro Djatikoesoemo Bojonegoro sebagai salah satu Rumah Sakit milik pemerintah kabupaten Bojonegoro. Sebagai pusat rujukandari beberapa rumah sakit di daerah Bojonegoro dan sekitarnya. Dalam penelitian ini menggunkan suatu pen dekatan dengan analisis SWOT yang merupakan langkah awal dari suatu perencanaan strategi pengembangan yang dimulai dengan identifikasi masalah, tujuan organisasi sampai pada menimbang kekuatan dan kelemahan sendiri serta peluang dan ancaman dari luar dan juga melakukan beberapa langkah penting yang menunjang pemasaran atau pengembangan. Jenis penelitian ini adalah penelitian studi kasus dengan menggunakan rancangan penelitian deskriptif kualitatif dan Populasi dalam penelitian ini populasinya adalah pasien rawatinap dan pasien rawat jalan sebanyak 200 orang responden Sampel merupakan sebagian atau wakil populasi yang diteliti. Dalam penelitian ini mengunakan sampling pertimbangan (Judgement Sampling) Dari hasil pendekatan dengan analisis SWOT perlunya pelaksanaan atau realisasi dari struktur organisasi yang menempatkan farmasis dalam farmasi klinik, peningkatan kualitas dan kuantitas sumber daya manusia, serta perlunya penambahan fasilitas berhubungan dengan IPTEK untuk kegiatan pelayanan farmasi baik secara manajerial maupun ke arah farmasi klinik. Kata Kunci : Farmasi, Analisis SWOT ABSTRACT The hospital is an institution health care providers, Regional General Hospital Dr. R. Sosodoro Djatikoesoemo Bojonegoro as one of the government-owned hospital Bojonegoro. As the center rujukandari several hospitals in Bojonegoro and the surrounding area. In this study using the approach with a pen SWOT analysis is the first step of a development strategy planning which starts with the identification of the problem, the purpose of the organization came to weigh their own strengths and weaknesses, opportunities and threats from the outside and also did some important steps to support the marketing or development. This research is a case study using qualitative descriptive study design and population in this study population was rawatinap patients and outpatients as many as 200 people respondent sample is partially or representative of the population studied. In this study, using sampling considerations (Judgement Sampling) From the SWOT analysis approach with the need for the implementation or realization of the organizational structure that puts pharmacists in clinical pharmacy, improving the quality and quantity of human resources, as well as the need for additional facilities related to science and technology for good pharmaceutical service activities managerially and in the direction of clinical pharmacy. Background:: Clinical competence of pharmacy students is better evaluated at their practice sites. compared to the classroom. A clinical pharmacy competency evaluation rubric like that of the American College of Clinical Pharmacy (ACCP)is an effective assessment tool for clinical skills and can be used to show item reliability. The preceptors should be trained on how to use the rubrics as many inherent factors could influence inter-rater reliability. Objective:: To evaluate inter-rater reliability among preceptors on evaluating clinical competence of pharmacy students, before and after a group discussion intervention. Methods:: In this quasi experimental study in a United Arab Emirates teaching hospital, Seven clinical pharmacy preceptors rated clinical pharmacy competencies of ten recent PharmD graduates referring to their portfolios and preceptorship. Clinical pharmacy competencies were adopted from ACCP and mildly modified to be relevant for the local settings. Results:: Inter-rater reliability (Cronbach's Alpha) among preceptors was reasonable being practitioners at a single site for 2-4 years. At domain level, inter-rater reliability ranged from 0.79 - 0.93 before intervention and 0.94 - 0.99 after intervention. No inter-rater reliability was observed in relation to certain competency elements ranging from 0.31 – 0.61 before intervention, but improved to 0.79 – 0.97 after intervention. Intra-class correlation coefficient improved among all individual preceptors being reliable with each other after group discussion though some had no reliability with each other before group discussion. Conclusion:: Group discussion among preceptors at the training site was found to be effective in improving inter-rater reliability on all elements of the clinical pharmacy competency evaluation. Removing a preceptor from analysis did not affect inter-rater reliability after group discussion.

This study aims to evaluate the impact of implementing a specialized clinical pharmacy program in patients with allogeneic hematopoietic stem cell transplant (HSCT) on their adherence to the immunosuppression treatment after discharge. A prospective open interventional design using a retrospective control group was used. The intervention was based on pharmaceutical consultations: the first was performed the day before discharge of HSCT unit and the next consultations during day-care follow-up (weeks 2 and 4 after discharge). Proactive medication reconciliation was implemented with a complete list of medications before the discharge prescription. The discharge prescription summarized on a personalized drug schedule was explained to the patient. The importance of optimal adherence and the potential problems related to self-medication were explained to the patient. Immunosuppression drug adherence was assessed by a direct method using serum levels of calcineurin inhibitors. The potential impact on acute GvHD, and infection was investigated. Twenty-six patients were included in the specialized clinical pharmacy program and 35 patients were in the control group. Seventy-nine pharmaceutical consultations were conducted in the intervention group, lasting a mean 25 min and 16 min for the first and following consultations, respectively. Serum levels in the therapeutic target range were higher in the intervention group (61.5% versus 53.0%, p = 0.07), with greater intra-individual variation (p = 0.005). There was no significant intergroup difference in acute GvHD (53.8% versus 50.3%, p = 0.85) or infection (26.9 versus 22.8%, p = 0.72). The implementation of a specialized clinical pharmacy program for patients who have received allogeneic HSCT seems to be beneficial for immunosuppression drug adherence; this now needs to be confirmed in a multicenter study involving a larger number of patients.

AbstractBackground Although clinical pharmacy is a crucial part of hospital pharmacist’s day-to-day activity, its performance is not usually subject to a holistic assessment. Objective To define a set of relevant and measurable clinical pharmacy and support activities key performance indicators (cpKPI and saKPI, respectively). Setting Portuguese Hospital Pharmacies. Method After a comprehensive literature review focusing on the metrics already in use in other countries, several meetings with directors of hospital pharmacies were conducted to obtain their perspectives on hospital pharmacy practices and existing metrics. Finally, five rounds with a panel of 8 experts were performed to define the final set of KPIs, where experts were asked to score each indicator’ relevance and measurability, and encouraged to suggest new metrics. Main outcome measure The first Portuguese list of KPIs to assess pharmacists’ clinical and support activities performance and quality in hospital pharmacies. Results A total of 136 KPIs were assessed during this study, of which 57 were included in the original list and 79 were later added by the expert panel. By the end of the study, a total of 85 indicators were included in the final list, of which 40 are considered to be saKPI, 39 cpKPI and 6 neither. Conclusion A set of measurable KPIs was established to allow for benchmarking within and between Portuguese hospital Pharmacies and to elevate professional accountability and transparency. Future perspectives include the use of both cpKPIs and saKPIs on a national scale to identify the most efficient performances and areas of possible improvement.
