AbstractObjectiveObesity is associated with chronic pain, but the contribution of body mass index (BMI) trajectories over the life course to the onset of pain problems remains unclear. We retrospectively analyzed how BMI trajectories during the transition to adulthood were associated with a measure of pain interference obtained at age 29 in a longitudinal birth cohort study.MethodsData from the National Longitudinal Survey of Youth, 1997 Cohort (follow-up from 1997 to 2015), were used to determine BMI trajectories from age 14 to 29 via group trajectory modeling. At age 29, respondents described whether pain interfered with their work inside and outside the home over the past four weeks (not at all, a little, or a lot). Multivariable ordinal logistic regression was used to evaluate pain interference according to BMI trajectory and study covariates.ResultsAmong 7,875 respondents, 11% reported “a little” and 4% reported “a lot” of pain interference at age 29. Four BMI trajectory groups were identified, varying in starting BMI and rate of weight gain. The “obese” group (8% of respondents) had a starting BMI of 30 kg/m2 and gained an average of 0.7 kg/m2/y. On multivariable analysis, this group was the most likely to have greater pain interference, compared with “high normal weight” (odds ratio [OR] = 1.47, 95% confidence interval [CI] = 1.14–1.88), “low normal weight” (OR = 1.45, 95% CI = 1.13–1.87), and “overweight” trajectories (OR = 1.33, 95% CI = 1.02–1.73).ConclusionsObesity and rapid weight gain during the transition to adulthood were associated with higher risk of pain interference among young adults.

Background: Both anaemia and high haemoglobin in pregnancy are associated with adverse pregnancy outcomes including foetal growth restriction. The objective of this study was to investigate the associations between maternal haemoglobin in pregnancy and trajectories of length/height and weight from birth through childhood. Methods: Data from 7,597 singleton pregnancies in the Avon Longitudinal Study of Parents and Children (ALSPAC), an ongoing, prospective, UK population-based pregnancy cohort study were used. We examined associations between maternal haemoglobin (overall pregnancy and trimester specific) and offspring length and weight at birth, as well as trajectories of height and weight gain from birth to age 10 years derived from multilevel models. Results: Mean pregnancy haemoglobin was 11.61 g/dL (SD 1.12). For each 1g/dL higher mean overall pregnancy haemoglobin, offspring were on average -0.30 cm shorter (95%CI: -0.35, -0.24, p <0.001), and -97.7 g lighter (95%CI: -110.42, -84.93, p <0.001) at birth when adjusting for potential confounders. Trimester specific inverse associations with birth length and weight were strongest for third trimester haemoglobin. There was evidence of a positive association between maternal haemoglobin levels and offspring height gain up to the age of one year and no strong evidence of associations between pregnancy haemoglobin and childhood weight gain. Conclusions: In high income countries, higher maternal haemoglobin in pregnancy may be a concern, as well as anaemia. Further studies are needed to define ‘high’ haemoglobin in pregnancy and whether monitoring of women with high pregnancy haemoglobin is warranted.

Understanding the determinants of early introduction of sugar sweetened beverages (SSBs) may assist in designing effective public health interventions to prevent childhood weight related conditions (obesity). This study explores the relationship between family/infant characteristics and the early introduction of SSBs among infants in Sydney, Australia. Mothers (n = 934) from an ongoing birth cohort study were interviewed at 8, 17, 34, and 52 weeks postpartum. Multivariable logistic regression analysis was used to identify family/infant factors independently associated with the likelihood of early introduction of SSBs (<52 weeks of age). Of the 934 mothers interviewed, 42.7% (n = 399) of infants were introduced to SSBs before 52 weeks. Mothers who were born in Vietnam (adjusted Odds Ratio (AOR) = 2.14; 95% confidence interval (CI) 1.33, 3.47), other Asian countries (AOR = 1.62; 95% CI 1.02, 2.58) as well as single mothers (AOR = 3.72; 95% CI 2.46, 5.62) had higher odds of introducing SSBs early to their infants. Mothers from highly advantaged socioeconomic background (AOR = 0.43; 95% CI 0.28, 0.68), those who breastfed their baby for 17–25 weeks (AOR = 0.60; 95% CI 0.37, 0.99), 26–51 weeks (AOR = 0.65; 95% CI 0.45, 0.94), and 52 weeks or more (AOR = 0.62; 95% CI 0.43, 0.90); and those who introduced solids between 17–25 weeks (AOR = 0.58; 95% CI 0.36, 0.91) and 26 weeks or more (AOR = 0.55; 95% CI 0.34, 0.91) had reduced odds of introducing SSBs early. Tailoring health promotion programs for these vulnerable groups may delay the introduction of SSBs.
